I highly recommend Tanbark Rehabilitation and will go again, if needed, without reservation. I was at Tanbark Rehab following a shoulder replacement. The staff is wonderful! They helped with everything I needed. I was unable to shower by myself and the assistants that helped me made me comfortable. The nurses were professional and caring. The rehabilitation staff were amazing. The PT and OT staff were knowledgeable and engaging. I looked forward to that part of my day. Everyone I meet from the doctors to housekeeping, front office, administrative staff and dietary staff made me feel comfortable and cared for. My only complaint is their medication system. Their pharmacy is in Louisville. Enough said.

My father was recently at Tanbark after having covid. He was treated with great care and respect. The room was comfortable and the staff treating my Dad was exceptional. The social worker, Latoya, did an amazing job of making sure all of his needs were met. Sandy, Dawn and the two Mikes as well as Mathew the Physical Therapist should be recognized as well. The food was good, as I was offered a guest tray during visits. Tanbark also provided transportation for Dad so that he could visit his doctor during his stay. They also brought a doctor to his room when he was having issues with a wound. Overall we are very happy with the care he was given.

My mother has been at Tanbark for 3 years total. She was in the rehab section for 3 months and in the personal care side the remaining time. The apt is large , well lit and has a large window to see out of. Except when she was isolated due to Covid restrictions we have been very pleased with the care she has received . Some of the staff and caregivers have been there for several years. The food is very good and you can’t have a better food staff than Brad and his crew. I go see my mom most every day except when I’m in florida. When I’m here away from her, Tammy Smith sends me updates and pictures of mother doing activities. She treats her like family. They have a new activities director who is doing wonders getting the residents to participate in doing crafts . All in all the Ayres family has been pleased with our mom being here. Our dad was also here for his last 4 months and loved Britany dearly who also is very kind to my mom. Her rooms are kept clean. We all love the hair stylist on board now that Covid restrictions are less and loves my mom.

My aunt was in Tanbark Health Care Center for rehab. It was fine. (My mom was also there three years ago.) They had wonderful physical therapy, the nursing staff was very nice, and the rooms were very large and clean. Overall it was very nice. It's located in a residential area, it's well maintained, and the grass and everything was well kept. It fits well into the area that it's in. It's not homey. It's small, but it's definitely a rehab center. They have lots of patience; my mom had vision problems, and they were very kind and helpful. My aunt had very extensive physical therapy, and they worked with her really well.

My mom stayed in Tanbark Health Care Center for two weeks. I was not pleased at all. I've gotten an excellent review on that. I would not take my mother back there, and even my mother, she was not happy at all. They were not organized, they lost her clothes; it even had her name on them, and the food was not good. They told me that I could come and do a window visit, but I wasn't able to even look in her room because there was a courtyard. I don't think they did any activities, and my mother was not pleased with the food at all. I didn't feel the care and love there.

I was here for respid care for PT and while my townhouse was putting in the necessary changes after an injury. All of the staff were fantastic, great facilities, awesome amenities (with the beauty salon which is great) and wonderful PT. I was definitely not the typical resident (at 28yo) and I was treated wonderfully by all staff and residents. My aunt ran nursing homes/assisted living and we visited quite often so I have been in and around nursing home facilities for most of my life and this is definitely one of the best.

Mom (age87) was in the Rehab unit after breaking her femur. The facility is always neat and clean. The staff is very personable, helpful, & patient. Mom's rehab therapy was exactly what she needed, building up her arm strength and increasing her ability to use a walker and gradually increase weight bearing on her leg as it heals. We have no complaints and are grateful to the whole staff!

I checked out Signature HealthCARE at Tanbark Rehab & Wellness Center. They were too hands on, more like a nursing home than a senior community, and I don't need all that help. They come around your room every two hours and check on you, and if you want that much help then they're really good. It's a beautiful place and it's gorgeous, but it's a lot more money than my current community (like $1,500 a month more(. The staff was very helpful and very nice. They're gorgeous, but they're kind of too elegant for me. It's a fancy place with plush carpets.

My mom was sent there after open heart surgery and from the minute we arrived she was treated like a family member. She has received exceptional care both physically and mentally. She had a small relapse and went back to the hospital for 2 days and without question she returned to Tanbark to finish her therapy. Every single nurse, nutritionist, cleaning and maintenance employees, and support staff are highly knowledgeable and if they don't know something they seek out the answer immediately....not hours but immediately. I recommend Tanbark to anyone seeking rehabilitation in an atmosphere where they want to feel loved and helped.

My wife was in Tanbark because she had Bell's palsy and was falling every day. Tanbark did everything they could, but they couldn’t be with her all of the time. They had very professional staff and a doctor that came in to see her once a month. The food was good. The room was nice and had two rooms: the bedroom and the living room. It had a walk-in closet, a nice bathroom, and a small kitchenette. It was a very nice place and served my wife’s purpose at that time.

***Be aware of several things: Upon admission, they do NOT tell you that upon the 21st day of rehab, you will be charged an additional $150.00 per day because Medicare co-payment is reduced beginning day 21. If they "feel like" notifying you, they might, but in my case they waited until 18 days later to tell me they had been charging this amount to my bill. When I called to talk with 1 of the corporate vice presidents, he said "they're supposed to", but they feel no responsibility or conscience to make sure it IS done. Be advised to check and DOUBLE-CHECK the medications they put your family member on and the Care Plan that is posted in the room. Question it and make sure they FOLLOW it! I had to make sure a private caregiver was there most of the time when I couldn't be there because for the first 10 days, no one knew what the other one was doing. I suggest you RECORD your admission interview and do not sign anything unless you read every single word, even though they'll say, "this basically says...". Lastly, the current administrator is sadly lacking in people skills. Ask any person and/or employee there. Now, for something positive...the rehab OTs and PTs do a great job and are truly committed to rehabbing your family member. For them, I give 5 STARS.

My father was actually at Tanbark Health Care Center for rehab, and it was very nice. The facilities were wonderful, and the food was wonderful, but I don't think the care was nearly as good as the amenities. Some of the staff were wonderful. I'm 50-50 on that place; you were never greeted when you came in, and the people sitting at the door didn't even acknowledge you. The aides and some of the nurses were great, and some of the physical therapists were great, but I think overall there was a disconnection with the people that work there.

We visited Tanbark Health Care Center when we looked for a place for my mother-in-law. She would have a bedroom, small living room, microwave and sink. It was very nice but they did not have a secured dementia unit.

The staff was very friendly and helpful. They were able to answer all my questions and were very reassuring of their facilities accommodations. The administrator, Conjuna Collier, was very informative and even let me meet some of the staff that would be caring for my grandmother. The only negative thing I can say about this facility is cost but who doesn't complain about the cost of living in a facility such as this one!

We just moved our grandmother into Tanbark on the corner of Tates Creek and Man O War in Lexington, Kentucky. She has advancing Alzheimer's but is still able to somewhat care for herself. Everyone has been very nice and the facilities are beautiful. This is a great option for our family because as she advances in her illness we can move her to a higher level of care within the same facility.
