I cannot recommend The Citadel at St. Anne Place based on my family’s experience. A close family member lived there during the 2025 management/operational transition. After that change, we saw a noticeable decline in responsiveness, communication, and follow-through. Basic care concerns were not isolated; they became a pattern. We raised specific concerns with staff about hydration, toileting/incontinence care, timely changing after bowel movements, call-light responsiveness, and overall decline. Those concerns were acknowledged, and we were told care would improve and that a care plan or follow-up response would be provided. No follow-up or care plan response was ever provided to me. Within days, we remained concerned about her condition and care. She was later sent to the hospital and died the same day. Afterward, we requested records to understand what care had actually been documented and provided. The facility supplied parts of the chart, but not the kind of operational records that would show real-time care delivery, such as detailed CNA documentation, call-light response records, hydration/intake records, meal delivery records, rounding records, or staffing/assignment records. Later, the administrator stated through the Ombudsman’s office that they had supplied everything they had. That is deeply troubling. Families should not have to wonder whether basic care was provided, whether warning signs were noticed, or whether concerns were taken seriously. If you are considering this facility, ask very specific questions about staffing, call-light response, hydration, toileting, meal assistance, escalation procedures, and how care is documented beyond summary chart notes. I personally would not place a loved one here again.
