"Our grandparents stayed in Alden Meadow Park for four years. We have a relative that works there, too. It's an older place, but it's local. The facility is close to my dad's house and my dad could take the dog and walk down there. "

"My grandma has been a resident for a little while now. She has received excellent care from the staff. They taught her how to use some of her prescribed devices, and made sure she understood what her medication was for. Also, while not a resident, my grandpa goes quite often to visit and they are more than accommodating. Finally, something that I didn't really think of until I visited, they have great security. Lots of locks and passkeys to get in and out of the facility. This helps keep the residents safe. I have been more than pleased with the care my family member has received. "

"Evansville Manor is a beautiful, clean, welcoming place. They had a really nice arts and crafts room where they can do crocheting and painting, and a game room for bingo nights. Everything looked and smelled good. The staff wasn't stuck up and speak to you and ask if they can help."

"My mom has lived here about a month, and we are very happy with the facility and the staff. Everyone has been very kind, helpful and accommodating. Mom has her own room, which is small but perfect size for her. She shares a bathroom with a neighboring room, which is empty now, so she has it to herself for now. Vintage has a very cozy atmosphere, very home-like, with a library, an activity room (called "parrot room" because there is a parrot in a cage) and comfy decor. There are several pianos, which the residents play, as well as guests. They keep mom very busy, which was a challenge at her last assisted living facility. Here she has exercise class, bingo and a lot of entertainment. The food is really good. Best of all, the staff members seem happy to be there and are very attentive to her needs. I would give this place 5 stars except the building is a little out of the way, in farm land, and old enough that you can see the need for a few minor repairs, such as a bathroom cabinet that does not close all the way. "

"My sister is all settled in at Hearthstone by Fairhaven and it seems to be working out OK. It's about a 30-minute trip from where I'm living. We have friends that have been living there for 17 years, so we knew it had a very good reputation. She is now in their memory care, which seems to be serving her quite well. She has had some outbursts and combativeness, and they've been very tolerant in working with it. It's a nice atmosphere. She complains that they're all mean, but that comes with the territory. I haven't really seen anything happen that would want me to have her leave there. They don't do many activities with the people, and that's my only complaint. She might be happier if she had more stimulation. It's overall a very nice place. My sister has a tendency to fall, and they seem to be really careful in keeping an eye on her when she's walking. Overall, we're satisfied with everything. She has access to the building that has the apartments and the assisted living. She likes the food, and if she wants to go and eat in their dining room, she can do that without any problem. She can go any place in the main building for anything. For what she gets, it's more economical than other places. Initially when we walked through Fairhaven into the main part of the building where she was for a while, there were people in there down by the dining room, in their lounge area, and in the smaller lounges. Each floor has its own lounges. Every week, they have tea and things going on in the afternoon down in the main lodge, so that you could get to meet other people."

"Hospice Services at Willowfield Nursing & Rehabilitation Center receive an overall rating of above average. Willowfield Nursing & Rehabilitation Center, in Delevan, Wisconsin provides hospice care. Hospice workers are educated, maintain professionalism, as well as compassion for the client, family and friends,. I met with a staff who demonstrated her level of care and compassion in the way she described her work. They provide care for the client, but also support to the family, during and after. I believe they provide wonderful care. I would recommend Hospice through Willowfield Nursing & Rehabilitation Center. "

"Both my mother-in-law and my father-in-law did rehab here after a broken hip. The staff is very caring and considerate. They went out of their way to treat the residents with respect. It is a large facility, but kept in good condition. The rehab unit is newer and the rooms are larger but the rest of the building is nice also. They have a chapel which is nice for those who like to attend services on Sunday. There is a small room with tables and chairs and free coffee and cookies where families can gather. There is also a room which any family can reserve for a gathering free of charge. We used it for Father's Day and there was china and silverware and free coffee. The staff cleaned up the room after us. The Physical Therapy staff is available even on weekends to hasten your rehab. Medicare paid for services during this rehab, but because it is skilled nursing, Skaalen can get expensive once the resident is on private pay. They do accept Medicaid, however, once a resident is out of funding. The facility has a unique feel -- located in the Norwegian community of Stoughton, the common rooms and hallways are decorated with Norwegian rosemaling, making it look very homey and welcoming. "

"My mom came to Nazareth following a long hospital stay and hip surgery. This was my/our first interaction with any kind of skilled nursing facility and I was nervous about the kind of care and facilities where my mom would be placed. I have been so amazed by the graciousness and patience of all the staff I have interacted with. They show all the residents compassion, make them smile and laugh, and care for their needs. I feel very lucky that my mom ended up here."

"My daughter moved into River Bluff Nursing Home. The staff seems to have a genuine interest in the patients. My daughter feels very comfortable and relaxed. That's where she wants to be, a nursing home. It is an environment where she feels safe. The rooms look quite regular. It's not a new place. It's an old place, but it's not dilapidated. It's just kind of like a hospital. My daughter said the food is very good. They do have activities. My daughter participated in a baseball game. It's a card game. I don't know exactly what it was. And then, yesterday, she participated in a game called Wheel of Fortune."

"Wesley Willows was an older facility but kept up very nicely. The people we talked to were friendly. The facility itself is spread out very large, like 2 or 3 different buildings. We toured one of the rooms. It was a studio and it was very small but when we got down to talking about the price it was so much higher. The staff was well informed but seemed to hold back when we asked about the price. We did not see a common area. They didn't offer to show us anything."

"My parents used to be residents of East Bank Center. They were both there and they forgot to give my mom her thyroid medication and so she went way downhill. That place was awful. I had to call the nursing home government to report them because she could have died or gone into a coma without having her thyroid meds. She changed into a different person. She was fighting and biting them and she's never done that before. It took them three weeks to figure it out, so I was not happy. They also didn't notice the bed sores, so when she got to the new facility, they saw the bed sores. She had fallen a lot at East Bank too. At the new facility they had her bed down on a mat so if she fell it was not very far. At East Bank they didn't do that, I guess, so when she fell she'd end up in a hospital. She had a big cast on her head. They also didn't keep it clean so it was an awful experience. It was the worst place that I ever had to deal with. The staff was very friendly, there might have been a couple that weren't, but for the most part there were a couple that were very friendly. They shared rooms there. It is such a small place that I did not see anything around there. There was a little spot used for the hugging booth, like a plywood thing that you could put your arms through and you got to put these long gloves on, this was right outside the cafeteria. That's the only place that we could really see them."

"My father was at Asta Care Center. The staff was great. His room was spacious and clean. He even was allowed to bring his own big-screened TV and everything. He is a little diabetic, so he doesn't want to walk around too much, but they had him going to therapy. He didn't have a terrible time there because he stayed there for quite a while. They're great there. They were friends to me, and they were friends to my father. He was well taken care of; that's why he was released, and he was released because they actually did their job. However, there were only a few things that weren't right like the patients going into his room and stealing his stuff. "

"I wouldn't advise anyone to take their loved one anywhere else. Are you guaranteed a private room? No, but if there are issues they address them right away. I have been so impressed with the care the residents receive. The outpatient therapy is such a great addition to have once your mom or dad goes home, they can continue on with the same therapists. The kitchen offers to make you anything you want even if its not on the menu, and they monitor weight loss and have nutritionists that work with the residents. The Social Service director is so kind, she makes sure everyone who goes home has everything they would need. The administrator is so involved and knows everything that happens and really works to make sure each resident is successful and has everything they need. I have never encountered more caring nurses and CNA's. They have an amazing DON who is also a wound care specialists and the in house Nurse Practitioner is available any time you need her, she always takes the time to speak to residents and families and answers all their questions. I like that they invite the families to care meetings along with their loved one and they have them as often as needed. I couldn't say enough about the place, its really great. "

"I visited Fairhaven. The staff was very friendly and very helpful. They had a washer and dryer in each place. Each one had their own home, and you could make your own food. We liked the independence and that they had separate housing. "
