"Mom required PT, OT and speech therapy after an extended hospital stay. Farmington Village was perfect. The hospital liason came to hospital to assess mom same day we called to inquire availability. Though the facility is older, it is spotless and well maintained. The staff was very attentive, treating mom with kindness and patience. The food was good, not at all bland hospital fare. The PT, OT and speech therapy was outstanding. I had peace of mind with mom staying there. The Christmas decorations were warm and cozy. The common area had tables and ample seating for us to work puzzles and play board games When she was ready to transfer to her Assisted living facility, the director, Phyllis, had paperwork promptly completed and transfer was seamless. If rehab or long term care is needed in the future we will go back to Farmington Village"

"My father is in Roseville Rehabilitation & Health Care. We chose this because hospice found this for us, and we were comfortable with the facility. Their communication with our family has just been absolutely wonderful. You know with COVID we can't go inside and visit, but we've been able to dowindow visits. And during the Holidays they were very accommodating when they didn't necessarily need to be, with the amount of people they have. They have activities, it's in a calendar. They will take him down so that he's a part of the group, so he's able to do activities at some point. It's extremely expensive, close to $5,500 a month. And I don't feel like, ever since my dad went in, that anyone took the time to sit down with us to talk to us about the options. He went from the hospital to the nursing home, so Medicare should have paid for that, and that was never offered to my mom, and I'm a little upset over that."

"My mother is at Toulon Rehab & Health Care Center. She goes through Medicare and Medicaid. Cleanliness is good. They keep it up. The staff is very nice and very friendly. My mother shares a room; she has a roommate. It's adequate. She has a closet, half of a dresser, and a bed. The main area is where they spend most of their time. She likes it. She still thinks she's living in her apartment, which is good. With regards to activities, my mother does a lot of walking, she colors, and they have crafts and stuff. But because she has dementia, they don't take them out on activities. The last time they did that, they had a very hard time getting them back in there. Plus, one of them wandered. He took off on them because he wouldn't go back in. They have a fenced-in back area with tables and chairs. I haven't tried their food myself, but I've been there when my mother was eating. It's a full-on course. They feed them very well. They're not skimpy. They give them a lot to eat. They have a dessert with the meal, either juice, milk, or whatever. Then they have regular snack times, too. The whole place is very good."

"The building was very clean, from the front visitors area to the cafeteria and room I visited. It was pleasantly quiet at the time. I happened to show up around lunchtime, so I sat with the resident during her meal. There was slight confusion during the serving of the meal, but it appeared to be well-prepared food and the resident said she enjoyed it. She was given her medication with her lunch and the nurse was very polite and made sure that all of the pills were taken. The resident's clothing appeared to be in good, clean shape. This nursing home had a hospital odor but not a foul smell that sometimes is in a nursing home. There were posters up for some civic choir that was going to be performing soon at the facility. I heard a couple of alarms go off while I was there, but they were tended to fairly quickly. When I left, I noticed that the grounds were well kept and the parking lot seemed to be in good shape. I thought that this nursing home was much nicer than others that I have visited in my life. I would consider this facility if I needed to shop for a place such as this for a loved one. "

"MY experience is I've visited attended many different activities here for the residents employees and community I was free to view the facility its clean warm friendly and caring.the employees are like family if you dont have any they welcome you. I'm a personal assistant I've had client's here temporarily due to stairs in the homes. In MY opinion this is a good healthy environment. If I had to stay or put a friend or relatives here I'd do it if I had to stay this is where I'd choose to stay. I love the staff night and day.the food that's offered is good also. Nothing like the past when food had to be blan or no taste. Housekeeping staff is friendly also. Rooms are clean quiet and personalized. This is the place to be when you are in need."

"Granted the home was not brand new or state of the art, but it was very clean and very well maintained. Staff cared deeply for Dad, this was evident when he passed away. 2 of the nurses that were closest to him, were very moved and cried at his passing. The food was freshly prepared, varied, presented well, tasted great. Activities were varied and were of the proper type for the audience that attended. In a nutshell the staff and facility put my brother and my mind at ease knowing that he was well taken care of. Fortunately he was a Veteran, his benefits and pension from his last job took care of everything. Transportation was provided for trips to Walmart or to parks for day trips and to give the people their choice of outings. Medical staff as well as the patients own private physicians were given 24/7 access, all meds were metered out if the patient was not able to maintain themselves. "
