I am/was a resident of this facility

I was at Canyon Vista twice.Rooms are large and cleaned on a regular basis. Staff is friendly and attentive. Food is good. I would recommend Canyon Vista Post Acute.

This facility serves very disgusting food. Better have money to have food delivered once in a while. Next, they often have only large size diapers. Started buying my own 3x. Therapy is a joke. Gaylen's idea of therapy is have u sit on side of bed doing exercises while he disappears for at least a half an hour. Finally asked Humana doctor how much therapy I was supposed to get and he said 5 hrs a week. Pissed them off when I told Gaylen that. They then threw me out. Canyon Vista owned by a family so if I complain they protect that person.

patient neglect is common in this place. they are under staffed and the workers who are there dont care about patient care or comfort. medicare needs to take a really close look at this place

My Mother was at the dumb for 2 weeks. I came in found her unresponsive no one knew anything. I demanded they call 911 or I would. My Mother was septic, fluid in her lungs. I would come everyday not a person would come in change or care for her. You hit the call button hours go by before anyone comes in. I put in so many complaints short handed not the patients problem. I’ve filed a complaint about lack of care to the State of Nevada. My heart breaks for those admitted with no family or friends. They must be so neglected anyone reading this don’t send your love one to this facility.

My mother was there 08/17/20 until she caught COVID -19 around 08/28/20 was in destress sent to hospital, where when she got there they said she was malnourished, had sepsis, and in acute destress. When she went into Canyon Vista I told them that she could NOT feed herself!!! She ultimately PASSED away on 09/04/20.

My cousin is in your facility and has told me that she is very satisfied with the care that she is given and also stated she is treated with respect and dignity by all of the staff. also stated that facility is kept clean and there are many things to do to keep the residents busy. I am very pleased that she is very happy and taken care of well.

my husband was brought here after a fall. The hosp gave them the wrong dosage on two meds. it took a bit of time, but i got that straightened out. our ins only paid for 20 days of rehab. i spoke to the case manager and she said i needed to sign papers while he is here for medicaid. i couldn't because of a medical issue. they could have sent a messenger to me to sign the papers, but they didn't. so, he didn't get it and i have gotten 3 bills for $1,000 in less than a month. they released him with uti and no nurse. he fell again due to weakness from a uti, hospitalized and now in another rehab. no one is returning my calls and i am mad. except for not checking the uti and making sure would follow up the 2nd day he was home, i have no complaints with the nurses. it's the one who work in the offices i have complaints about. case manager said i WOULDN'T SIGN PAPERS. untrue. i said I COULDN'T SIGN THEM DUE TO A MEDICAL PROBLEM. there is a big difference between the two. again, a messenger could have come to the house for me to sign the paperwork.

My father was a resident there. The staff are horrible. They left my father lie in his urine and stool for hours. The facility has a strong urine odor. Staff do not practice good hand hygiene. My father developed serious life threatening urine and blood infections.

Excellant case management & referrals for home care once discharge was approved. Eleanor & Kathy, case managers, were wonderful, knowledgeable & had effectively communicated patient care needs. Nursing staff very good.......rooms very clean..only negative is dietary choices/food prep.

I am really disappointed with the case manager. My mom is there for a broken hip and she has breast cancer. She was put into the facility on Tues and I informed them of an appointment she has Thursday with the oncologist. I was told the info would be given to the case manager and I would be getting a call. I have left 5 messages for Rae the case manager. who has yet to call me back and let me know if they will take her or if I need to.

My Mother in Law was sent here from Spring Valley Hospital after a stroke. She wasn't able to swallow regular food. This place gave her regular food. She now has asperation pneumonia and bed sores. She had to be brought to hospital.

I visited this facility

Content: After a back injury I was transferred to Canyon Vista Post Acute. My stay there was one of the worst experiences in my life. I only saw the doctor for 90 seconds the day I was admitted. They were careless and the only medication I was given was oxycodone acetaminophen 5-325, as needed. My pain wasn't going away. I never saw a doctor for the rest of my stay there. I kept asking for a crème or a patch to apply on my back. They only gave me a couple of patches at the end of my stay. I was also given Baclofen on my last day there, which helped, but I was still in pain. Here is the outrage. On October 23rd, 2017 I was told that I would be discharged on October 25th, 2017 and I had to file a claim if I wanted to stay past that day. I filed a claim. On October 25th 2017 at 8:18pm I was notified via a phone call that my claim was denied. At that time I was told by Canyon Vista personnel that although the claim was denied I did not have to leave right away and that I would be covered until the next day when I was arranged a ride. In December, 2017 I started receiving phone calls and a paper bill for $275.00 from Canyon Vista. They told me that one day of my stay (October 25) was not covered by my insurance. I have a letter stating that I was covered by October 26. I was also assured and told by multiple personnel and by my insurance that my stay at the facility would be covered 100% for up to 21 days. I only stayed there for 9 and I was basically discharged with the same back pain I was transferred there with. I also have Multiple Sclerosis which may have made my recovery from the back pain injury more difficult and time consuming. That is conjunction with the personnel at Canyon Vista doing nothing to speed up my recovery. As if their ignorance during my stay wasn't enough, I started receiving calls from the management who was belligerent, rude and disrespectful, asking me to pay $275. This back and forth is absolutely unacceptable. They continue bothering me with letters, threats, invoices to pay that bill, that should be covered by the insurance, as it stated in the letter. This harassment and constant pressure ruins my health more. This is supposed to be a medical facility caring about the people! Who cares?

My dad is in rehab at Canyon Vista. This place is exceptional. It is caring and loving; I would never want to move my dad from there. It's awesome. It pleases us that they care about their patients and are always welcoming. My dad has his own room and it is a perfect size for him. It is decent and has a nice odor.

I visited this facility

We weren't too thrilled with Brookdale Fisher's Landing. My dad had people that would come in for physical therapy, but he'd have a dirty diaper on so they wouldn't start therapy until he'd been changed. To make matters worse, the people who were supposed to change him took a long time, so they labeled him as someone who refuses physical therapy and they refused to extend his stay because of that. He also has colitis and they were feeding him spicy and greasy food which didn't help his condition. The rooms were very clean and the facility was nice, but a lot of the people there weren't great.
