My mother currently resides at Beaumont and she's been there about 2 years. We love the staff, from the front desk person to the nurses to the cleaning staff. We visit several times and week and see the interaction that staff has with my mother. She is very well cared and there are many activities during the day to keep busy. The rooms are fine, I believe they are being painted soon, the food is OK. I think the food is better on the second floor than on the first but my mother never complains about.

Horrible facility, I would give it a zero if I could. Care is subpar, call lights on for hours with no response. Majority of aides poor at best and the good ones are horribly overworked. Poor communication by nursing staff, dietician and physical therapy. My relative is supposed to get out of bed every day but that frequently doesn’t happen. Rarely dressed and food is horrible per my relative. Usually poor staff is related to administration at the top.

My relative is at the rehab center and she is alone 24/7. No one comes to talk to her and no one takes her out of her room. I have to constantly call to have them check on her and provide her with some companionship. It rarely happens. Very disapointed.

Mom was here for several weeks for rehab. Staff is not friendly. (one exception: one of the PTs). Many ignore the call lights for a long time. She often had to wait to be helped out of bed. They seem to want to keep patients in their PJs since it is easier. You really have to insist to have clothes put on. One day I went to find mom in a johnny in the late morning. I asked what happened and she said that she thinks it was easier to put that on her than her pajamas so that is what staff did (mom walks with a walker so she is not so compromised that she can't assist the staff in getting PJs/ clothes on/off! Mom said the food was "ok" but I think she was being kind, judging by how it looked and the amount she left behind on her plate. Rooms are very small and configured in an odd way. If someone wants to visit someone in the back half of the room, they basically have to climb over the person in the front half. Mom was never offered to be taken to any activities.

Horrible. My father in law was there for 3 1/2 months. Waited forever to be cared for, sat in soiled clothes daily, ghostown at times, no staff to be found. Avoid!!!!

10/12/18 DO NOT be fooled by the quote below from the Salmon Health and Retirement website, https://www.salmonhealth.com/us/ “We respect the privacy, the views and the decisions of the people we serve. From this respect comes extraordinary compassion and caring, as we strive to help them achieve their best quality of life. And we never lose sight of our most basic responsibility—to treat each individual as we would want to be treated ourselves.” Residents wander into rooms where they do not belong... residents up all hours of the night... residents yelling for help. Seems to be always understaffed. Call lights are ignored. I watched one CNA ignore a resident call light on for over 1-1/2 hours!!! The person never got help until the following work shift arrived!!! Management ignores the issues, don’t return phone calls, and actually seem to blame the resident for the problem. The place was reported to the Department of Public Health. Nothing has changed. I would NEVER have a member of my family be a resident in this facility.

One of the worst places I have ever been! Got my loved one out...took time. So PLEASE go elsewhere. Google this place and read about the horrors. Patients up till all hours of the night... always understaffed... call lights being ignored. Residents sitting in dining room soiled... urine on floor around them. Residents yelling for help. I turned my mom's call light on a couple of times... no one ever came. I left it on for over an hour! I reported this place to Dept of Public Health. I don't know if they ever went out. PLEASE DON'T PUT SOMEONE YOU CARE ABOUT HERE!

The staff at Beaumont Rehab & Skilled Nursing Center was absolutely incredible. The staff and the care were exceptional. The main dining room was lovely, and the food was sometimes quite good. There's aren't any comfortable "sit and talk" places there, which would be my only complaint. There was no place to visit outside any of the rooms. Every available space was used for community activity space. The food was tasty, well balanced, nutritional, hearty, and well put together. We were exceptionally pleased with his treatment, the front office staff, and the office work staff.

Poorly managed, large turnover of staff. Significant issues over last year which were well documented and reflect how the facility is managed day to day
