Rooms are large and don’t feel crowded with 2: of us. The interactions with the staff are all ways friendly and nice. Popular activities are bingo and card night. In-house laundry is handy, often same day service. Food is great and tailored to your diet. Outside smoking in the courtyard.

If I were you, I would not send a loved one here, the staff is very friendly but at they are not, some showed true care for my father that was there and others acted as if he was just another paycheck. out of a one month stay they forgot to feed him on three different occasions. the first time they forgot to give him dinner he was already there for a week, 6 days to be exact. how could they forget to feed someone dinner that has been in the system for a week already??? their excuse was that he wasn't in the kitchen system?? how could that be so when he was provided breakfast, lunch, and dinner for 6 other days. and then did that another 2 times to him. and the sad thing was that he wasn't the only one that happened to! they would scrape together a sandwich with one slice of ham or some kind of sandwich meat. the workers were so over worked that no one would come for about 30 min after my dad would ring the bell for assistance. my dad is a smoker and they allowed them to smoke, on the times they chose and would take his cigarettes away from as if he was a little kid. i don't find that fair at all. my dad said it was more like a prison than a rehabilitation center. so think twice before you send someone you love here!!

I simply cannot understand the excellent ratings people have given this place. My father is in this place for rehab. Outside of physical therapy - given only several times per week, he is expected to sit in a wheelchair or be in bed all day long. The nursing staff is totally indifferent. No one answers his call button and he ends up urinating in bed. Nursing is very slow to clean that up. The food is horrible. I wouldn't recommend this place to my worst enemy

My mother went to a rehab place called Haven Health Phoenix two years ago. We hated it. It was very clinical and not friendly. The care that she received was minimal like she would push her nurse button and no one would come for like eight hours. When she was first there, she couldn't get out of bed and she was completely bedridden. In the middle of the night, if she had to go to the bathroom, she would push the call button and someone would come and say, "What do you need?" and she'd say, "I need to go to the bathroom." They say, "Ok, we'll go get your nurse," and then no one ever came back. That happened multiple times. She ended up having to just pee on her bed. We hated that place. She was in rehab. There were a few people there that were very nice and helpful, but there was that same amount of people that were not nice or helpful. She was there for three-and-a-half weeks. Even the physical therapy or occupational therapy that she was supposed to receive every day happened maybe once a week. There was nothing there. They had a gym, but they never came and took her to it even though they were supposed to be working with her on physical therapy. It was a poor experience overall. There were no activities there.

I am in Haven Health Phoenix. It's awesome, it's clean, it smells good. The staff is very friendly and the food is really good. It tastes homemade. I have a small room, but it's always clean every day. They have bingo, they have someone come in and do nails, coloring if you wanna color. They provide physical therapy, occupational therapy. I saw some gym equipment, they have a couple of sitting areas if you wanna go out and sit. It's very well maintained.

This place has definitely changed and improved. They have new owenershio and are now called Haven of Phoenix. They have new directors and it has been a tremendous impact. They really cared about the ones that are there, whether they are living there or there for rehabilitation. All I want is to make sure worry my loved ones wre taken care of when I'm not able to be there. They exceeded my expectations. I have been to various ones and this has impressed me the most, with caring individuals

Our sister stayed here a little over a month here after being released from the hospital following a serious hip fracture & surgery. She did not have a pleasant stay, and I would definitely NOT recommend this place to anyone. One huge concern of ours: We were told that physical therapy would be done one or more times a day on at least 6 days each week, yet she only received 2 to 3 sessions a week, and this was the main reason she was there! Unfortunately, she made extremely little progress during her stay, & left with less confidence & independence than when she arrived!!! Other concerns / complaints we experienced at Grace Healthcare were: 1) Most of the hallways & patient rooms consistently stunk strongly of urine & feces, so much so that it made me physically sick when I first arrived each day to visit. 2) There were ALWAYS multiple patient call buttons beeping & flashing, often while staff stood around chatting & laughing with each other. 3) Our sister was often left sitting on the toilet for long periods of time with no help to get up & get back into bed, even though she her call button was pressed over & over. 4) On the day our sister arrived, her hospital bed was not working & the air conditioning was freezing cold, and although it only took a couple of days to get the room temperature to a comfortable level, the bed situation was not repaired for over a week! 5) Our sister's food was extremely salty & sugary, even though her doctors ordered a salt-free (kidney), sugar-free (diabetic) diet. 6) At one point, staff ran out of our sister's pain meds and some of her other meds, so that she had to go without until they could get more. 7) Often the housekeeping staff lost our sister's laundry, and even suggested it might have been stolen. 7) Rarely would anyone come to take our sister to activities (as promised), so she missed out on the socialization she sorely needed. Now I understand why Grace is listed as one of "The Worst Nursing Homes in Arizona" but I certainly wish we had researched this atrocity before we placed her there for skilled nursing / rehab post-operative care. Staff treated my family mom with kindness and compassion. They were knowledgeable and equipped to care for mom. I felt comfortable leaving her knowing she would be taken care of. The therapy she received was great, and again very kind and understanding. Would defiantly recommend this facility to others.

My family member was a patient and fell out of bed twice and did not receive his dialysis for days. They kept him heavily medicated as well. Then got sent back to the hospital to be told that if he hadn't been brought in he would have died. I would not recommend this place, period.

I would definitely not recommend Grace Health Care to anybody, at any time, for any reason. Some of the people that were caring for my sister were not qualified particularly in wound care. As a result, she was admitted to the hospital on 3 different occasions because she had infections. The place is old and in need of renovations. One day, I was talking to the director and the ceiling fell down. They didn't even have a water fountain. The maintenance guy wasn't really qualified to do electrical type of work. It was just awful. Also, the receptionist was rude and annoying. The only one we had a good experience with was with the admissions person. She was okay, but the main problem was the administrator himself, Michael Morris. You can complain all you want, and he shakes head and it's business as usual. Also, you could never register a complaint or get everything done because little do you know the staff are all related. My main issue with them is that they don't listen to you.

In our experience with our mother's stay, this place warehouses patients. There is little to no care. There were no therapies. Additionally, we were assured her insurance would cover her while there, however after a five day stay (during which we were trying to transfer her to a more humane nursing center) we were told she was not covered. The new center would not accept her because the "therapy report" said she was ambulatory - since she had to get out of bed (fall risk) and ask for towels to wash herself since they never bathed her. Since they did no therapy, we wondered what therapy report would even be valid. Please avoid this terrible place. It is filthy, run by incompetent, uncaring people and not fit for the elderly.

I was not very pleased with Grace Nursing Home. My grandfather was a resident here and i was not happy with the CNAs. They are not friendly, caring, and do not pay much attention to the residents. My Grandfather did not get the care that was promised. They frequently forgot to brush his teeth, change his briefs, give him water, and was ignored when he pressed his call light. It took at least 5 mins for someone to help him after pressing the call light. We took him out of this facility once we realized how poor his care was. I would not recommend this place to anyone. I was very disappointed at how this facility is run. I was upset most at how they did not provide him with enough water. This caused him to be greatly deydrated.
