The ElderCare Resource Center is an Alzheimer's/dementia specific geriatric care management firm. We are highly specialized and experienced in working closely with memory impaired patients, their families, caregivers, and members of their health care team. Our goal is to keep our patient's in the place they most want to beÂ…their own homeÂ… Utilizing our Integrated Care Management (ICM)® model, we coordinate, implement, and monitor all of the services necessary to improve the quality of care and the quality of life of the people we serve. I live out of state and the geriatric care managers at the ElderCare Resource Center made my life so much easier. They provided services to me and my family for over a year and recently assisted us with placing our dad. I highly recommend them!
