I cannot say enough good things about Oak Creek Terrace. The staff here are absolutely wonderful. From the moment we arrived, we felt welcomed and cared for. The level of care my mother received was exceptional. The therapy services are top-notch. The therapists are skilled, patient, and truly dedicated to helping residents improve their mobility and well-being. My mother made significant progress thanks to their expertise and encouragement. What stands out the most is the caring nature of everyone at this facility. The staff genuinely care about the residents and go above and beyond to make them feel comfortable and valued. My mother always spoke highly of the kindness and compassion she experienced every day. Overall, I am incredibly grateful for the great care provided by Oak Creek Terrace. It is reassuring to know that there are such dedicated and caring professionals looking after our loved ones. Thank you to everyone at Oak Creek Terrace for making such a positive difference in our lives.

Oak Creek Terrace has been serving seniors in Kettering, Oakwood, Centerville, Bellbrook, and surrounding Dayton, Ohio communities since 1986. We are conveniently located near Bigger Road and Rahn Road with easy access to I-675, I-35, and I-75. We are family-owned and operated by Caring Place Healthcare Group, which represents three generations of family members and over 50 years in senior living and skilled nursing facilities in Cincinnati and Dayton. Our building offers Assisted Living apartments, skilled nursing for long-term care or short-term rehabilitation, and a secure Memory Care unit. We recently renovated the entire building to a state of modern elegance.

On the whole I really enjoyed this location. I actually had to go twice and chose to go there the 2nd time (first time it was essentially chosen for me). It is not fancy, but I like that. Very comfortable/homelike feel to it. Rooms are large with a sofa and chair in each room for guests. The PT and OT staff were wonderful! and on the whole the administrative staff was wonderful as well. The problem I did have is they got my medications wrong. One nurse actually attempted to give me my next door neighbors meds. I stop it because I did not recognize them and asked what they were. Something similar happened another time. I reported this issue and was assured it would be all straightened out....and then it happened again. I was physically challenged but not mentally challenged... luckily for me. But how about those that would not recognize their medications and thus they would take incorrect meds. Serious! problem. When I reported this to the manager in charge, I was told I would get a report on how this was handled. Never heard anything. And when I called the night staff out on this, when there was a 3rd attempt that got my meds wrong, I was essentially scolded. Exact words were "well, you are going home tomorrow (true) and you can then just handle it yourself." I am so sorry it came to this because I so liked them in the beginning.

My father is at Oak Creek Terrace Nursing, Rehab & Assisted Living. His transition has been very smooth. They've worked with us to accommodate all his needs. He's gotten excellent care there. He is there for rehab. He is in a single room. It's a spacious room, and he has his own bathroom. They have a daily newsletter and daily activities including religious activities as well as social activities. The staff have been excellent. They've been receptive to working with any of his special requests and provided good care.

I am unclear how this facility is rated 5 star. My grandmother was in the facility for “rehabilitation” to go movement and strength back, where as ultimately she declined in a manner we had never seen before. The staff, not all members, seem rushed and uncaring. She had cuts, bruises and even fully ripped off toe nail, that now one could seem to answer when or how and we were never notified until we went to see her and found these. They sit the meal trays down and leave the room not caring if patients eat or can eat on their own. She also contracted covid at this facility, we are aware of them only testing her two times within 29 day period, yet we were tested twice a week to visit? She was sent home covid positive which was not addressed by the staff in any manner, and passed away roughly 8 days after being released from this facility. The decline in her was surreal to our family. I do NOT recommend this provider

My dad had to spend about 2 months in the dementia unit after he fell and broke his hip, I have to say the first couple days were rough, but after they fired an aide, that was causing dads night time issues, it was smooth sailing. the place is clean, the staff are caring and everyone was welcoming , the only thing I would change is to cook the vegetable longer so they can chew them

I was not very impressed with Oak Creek Terrace. It’s in an old school that has been converted, and I thought it was pretty antiquated. I’m sure their nursing staff and aides are all excellent, and I saw a lot of workers. I would suggest they redecorate and update their decorations.

Oak Creek was OK. The rooms were not as nice and they were quite small. I think they were getting to have ice cream social when we were there, and it was nice.
