"After being admitted to this facility, we could not be more pleased with how they treated my family member. My father-in-law was admitted here and we were always welcomed by the nurses and the physician always took the time to stay in the room to explain what was going on. The treatment plan was on point and they advised us immediately if any problems came up. It is a place I would recommend to other people. "

"I know that every facility has its problem but this is one of the best facility in Decatur. The staff really tries hard to please everyone even with the nursing shortage that is currently sweeping the country. My brother had a car accident and could not walk broke many bones but I am happy to say that with the nursing staff and therapy he was able to walk out of there 100% better than when he came in. I am grateful for everyone that works there, the place is clean and there are no odors. the building is accessible to marta, they have a great wound team, they even have a wound np that visit weekly. the activity department is great it is always something to do there, the food is really good I have heard about issues in the pass but I think they have resolved that they have rn/lpn and cna's that are really good with the patient especially the staff on short term unit. the facility needs more renovation some rooms are great while others still need repair. sometimes the cable don't work but the maintenance men are really good about fixing things."

"My experience with Traditions Nursing Home was a good experience for the most part. All of the staff members knew the patient and their respective families by name. The director was very friendly and would speak to you whenever she saw you. This facility was a very good facility. The facility's staff would make sure that they abided by doctor's orders and would keep a watchful eye on patients who would try to escape from the facility. I only visit this facility a few times a month but, whenever I visit I am welcomed with smiles and stories of what is going on with my grandfather. The entire staff is great at keeping you updated on all information regarding your loved one. This facility has great activities for your family member to attend: from church, to bingo, to sitting outside and enjoying the sunshine or the chirp of birds and the fluttering of bees. I am really impressed by the professionalism of the entire staff. "

"The facility has done good job during these trying times. The facilities have been clean according to my resident brother. The staff is always helpful and courteous when discussing care for my brother. AND - communication about Covid status has been OUTSTANDING, with calls daily when needed. (this was not the case under the previous ownership, who didn’t return calls even in pre- Covid times). In regards to food, my brother, a picky eater, is satisfied with it. No complaints! After I gave my brother a walker for Christmas, the staff worked with him to ensure he was able to use it properly. I have quarterly phone meetings with staff on his care. Activities are obviously limited during these Covid times. But my brother is still in good spirits because of the staff treating him nicely. As my brother’s guardian I MUST be attuned to this facility. The new Condor ownership has made improvements in all categories of care and I am glad my brother is under their care. "

"There is one immediate drawback to this place: It's located in a high crime and drug use area; there's a liquor store at the bottom of the hill to one side"”and housing projects to the other. But when you get closer to the facility, it looks like a cute brick house with rockers on the porch and flowering trees outside. Inside on the left are offices; on the right is a sitting room with a big screen TV and good furniture. There are centralized nurses' stations for the 69 beds here"”with only one or two private rooms. There is very little choice about what to eat. And residents don't seem to be taken outside the building for activities much. But on the plus side, it is very clean"”there are rarely odors. There is low staff turnover; the administrator, who is an RN, has been there at least 10 years. And the activities director, who has also been there a long time, is awesome"”always thinking about fun and engaging things to do. The quality of care is great"”there are RNs and LPNs, and even a nurse specifically dedicated to wound care; because there are few beds, they put more time in with residents than at many other facilities. And the administrator takes the time and effort to come out of her office often and monitor the patients. "

"My aunt is at the Fulton Center for Rehabilitation. Everyone seems nice and attentive to their needs. They have a nice one-story building with residents of different ages. It's clean, and they have people giving therapy and making sure that the patients' needs are met. She's being fed by the nurses. They have a rehabilitation area and a nice big dining area."

"My mother is in rehab at Crestview Health & Rehabilitation Center. We are familiar with the staff because she has been there once before, so she was quite pleased to be able to get back into Crestview. The staff has sort of been accommodating as best they can since she has a cell phone, and I've been able to communicate with her through that, and they've assisted in that communications with her. She seems to be very pleased with the staff and the support and help that she's gotten in the rehabilitation. We were pleased with the room. She does have a roommate, and she seemed to be pleased with that."

"My aunt is staying in Westbury. The staff is very good. They are on top of things, and they are really good with the patients there. They check in on them. They are wonderful. I have eaten lunch with my aunt there. The dining room set up, and the food is very good. The room has two beds, and it is all built in. It is small but sufficient, and there are two patients in each room. They have activities like games, bingo, church services, and cards. However, they can improve by having a little more variety in their menus. "

"Bonterra Nursing Center is very clean, and well kept up. They've been very nice and helpful. They are really on top of his personal care and medication. He has a room he shares with another guy but it's very spacious, very clean and well kept. The staff seemed to be pretty good. They get in touch with the family. I'm very impressed with their services. "

"My husband fell about 3 weeks ago and is in Conyers Rehab right now. He's in recovery and pretty much on his own. He just need assistance with walking. He does all his physical therapies at daytime to strengthen him. They take good care of my husband. The facility is clean and the food is excellent. It's just dynamic. I really like it. "

"My mom has been undergoing PT at RHC and so far everything has been very good. Starting out with Tammy in Admissions very nice and informative. Nurse Jameika is very kind and helpful. CNA Melissa...OMG she is phenomenal!!! She really takes very good care of her patients. Cynthia and Authrine are very good too. The cafeteria staff feeds them very good food 👌 The only negative is during the day it takes too long for staff to respond to the patient service call even after light flashing for a very long time. Please hire more staff and recommend checking on patients at least every hour. The patient rooms need a little paint touch-up on the walls. Another suggestion is to maybe have recliner sleep chairs for family to stay overnight if needed. I think Physical Therapy could be twice a day one in the room and one with equipment in PT room "

"My mother has been a resident here for several months, and we have both been very pleased with her quality of care. The nursing staff is personal and attentive, the activities department is enthusiastic and phenomenal, and everyone I've encountered in the facility from the business office to the janitorial staff is friendly and helpful. I've made lots of surprise visits at all hours, and I have never witnessed anything but calm, competent care. We both really like this place! "

"My best friend and I were visiting her grandfather that lived in Christian City. She was a little nervous to visit him in the nursing home, because she had the worst images in her mind of living in a nursing home. But after we got there her mind was easily put at rest. We saw her grandfather doing exercises in the activity room with the other residents. He was happy and laughing, even though he had to do his exercises from the wheel chair.The nurses came up and told us about the classes and how they do them each week and then they have a great social hour where they all drink coffee and eat cookies and doughnuts. We stayed for the social hour. Her grandfather seemed happy and had lots of the females attention and they were all so sweet together. We went back and visited him a few more times before he passed away. But I know he was happy living there for the short time he did. "
