Accepted Answer

We do not provide transportation services. Our staff are more than willing to assist in finding transportation through any local services. We have a bus that is utilized for scheduled outings and activities occasionally.

What safety features or security measures does Elysian Senior Homes of Duluth offer to assisted living residents?

Accepted Answer

All exterior doors are locked and secure after hours (6pm to 7am). We have security cameras on all exits and common areas of the building to ensure safety.

Who assesses residents' health and cognitive functioning? How often is that assessment repeated?

Accepted Answer

We have two Registered Nurses that complete all assessments and continuous monitoring measures for our residents. They complete an initial admission assessment, another assessment after the first 14 days in our community, and then ongoing assessments every 90 days.

Accepted Answer

When the building is fully occupied, we have a direct care staff-to-resident ratio of 1:10 in Assisted Living, and 1:8 in Dementia Care during the hours of 6am to 10pm. Overnight (10pm-6am), the direct care staff-to-resident ratio is 1:20 in Assisted Living and 1:16 in Dementia Care. All of our direct care staff have the ability to move between Assisted Living and Dementia Care to provide extra assistance in one or the other as needed.

If Elysian Senior Homes of Duluth accepts dogs or cats, what indoor/outdoor areas can they enjoy?

Accepted Answer

We do not accept pets. Family members and other visitors are welcome to bring any pets to come visit the building at any time. There are grassy areas around the building for pets to enjoy. We ask that all visitors please clean up after their pets when they visit.

What makes the chef or dining program at Elysian Senior Homes of Duluth exceptional? What kind of menu can assisted living residents expect?

Accepted Answer

Our meals are prepared fresh, daily. We follow from-scratch recipes and take pride in providing a home-cooked meal to our residents. Our menu includes a variety of meals including soups, salads, sandwiches, pasta, casseroles, potatoes (of all forms), chicken, beef, pork, and many more; all prepared in a variety of ways. We also have an Always Available menu including soups, salads, sandwiches, burgers, eggs, and more. This option is available for any residents who would prefer something different than what is being prepared for a meal that day.

Elysian offers Lifestyle Enrichment programming tailored to our residents' interests and needs. Our Lifestyle Enrichment Coordinator and Care & Wellness Assistants work together to provide and facilitate entertainment, exercise, games, crafts and artwork, baking, and other fulfilling activities throughout the day. Elysian also has a bus that we utilize for outings such as scenic rides, picnics in the park, shopping trips, and so on.

Elysian utilizes Care Conferences scheduled with residents and their families to keep everyone informed of any changes observed in level of care or to discuss any questions or concerns that need addressing. Elysian has an open-door policy and welcome residents and their families to come to members of administration or nursing with any questions or concerns at any time.

When significant changes in care occur, or upon any incidents (falls, etc.), our nursing team will contact the designated family member or guardian as soon as possible following the change in condition or incident to keep them informed. We have on-call nursing staff that will also inform family or guardians after hours as deemed necessary.

All of our staff utilize language and other techniques that encourage residents to come out to the dining room for all meals. Our staff closely monitor all residents' dietary needs and provide assistance through cueing, hand-over-hand movements, or provide full assistance with feeding. All staff will provide redirection as needed for residents who tend to leave the table often, or bring the meal to them if they'd rather not eat at the table. We accommodate dietary requirements such as, mechanical soft diets, pureed diets, thickened liquids, etc.

Is Elysian Senior Homes of Duluth affiliated with a hospital if more care is needed?

Accepted Answer

We are not affiliated with a hospital, though we work very closely with Essentia, St. Luke's, and Twin Cities Physicians. We have Eldercare (through Essentia) and Twin Cities Physicians as rounding physicians within our community. We also have lab services through St. Luke's and Essentia.

We also work very closely with a variety of Hospice agencies, Home Health agencies, PT/OT/SLP services, and more.
