Cooperative Elder Services- Concord

"Programs offers Social Day Care, Adult Day Health and Alzheimer's Day Care programs for frail elders and/or disabled people. Cooperative Elder Services has programs that are available Monday through Friday 8AM-4PM. Services include breakfeast, snacks and a noon time meal, counseling, recreational activities, door to door transportation and nursing services to those in the Adult Day Health program. State Home Care reimbursement may be available to some low income elders attending the programs. Cooperative Elder Services day programs have been approved by the Alzheimer's Association of Massachusetts. This C.E.S.I. Has received a grant from the Crossroads Foundation to initiate an ""Extended Day Respite Care Program"" at this Concord ADH center. It will be on Mondays, Wednesdays and Thursdays until 5:30 p.m. There is no cost to clients at this initial phase."
