my 90 yr old mother was placed in this facilty for p/t and o/t rehab due to 4 falls in her home. no injuries were incurred . i came into the facilty noting to staff the reviews i had seen and the care i anticipated for my mother. My mother did not complain at all, she is of 85% sound mind, and stated had not been mistreated in any way. I did have to request they change the bedding and it was done same day at night. My mother did not complain of the food nor drinkables and the staff called her and roommate the "gold star" residents. probably since they didnt need much tending to. The NP on staff was very blase about reviewing my mothers health needs and went on to the next. That would be my only complaint. My mother was there for a total of 5 days.

I have to ask every day to have my IV removed. I walk around with it for hours every day and it’s only scheduled for 1 hour. My floor has been swept 1 time in the almost 4 weeks I’ve been here, I ask every day and every day it is ignored. Staff is very short handed. Some have very little bedside manner.

My stepdad was at Granite Creek Health and Rehabilitation. It was a terrible place. They did not change his beddings for a week. They had his call button and everything pinned up out of his reach, so he fell and he laid there with no help. His roommate had to use his cellphone to call for help. The med techs were very good. As far as them coming in and taking care of them, not so much. I had to go there and have them come and take care of things. They were not attentive. Their food was terrible. It was cold. He couldn't even chew it because it was hard. Their community room where they go watch TV, they were fitting the people in between tables and chairs that are stacked. It was like a storage room. The place was not clean. In fact, they were tearing up the floors in their hallway.

Worst place EVER! Do not go here! Do not put your loved ones in here! My mom was here for rehab because she hurt her back. She ended up getting Covid pneumonia on top of her other lung ailments and now she is dying in the hospital. AND when I went to pick up my mom's possession today her money from her wallet was gone! $170! Plus her Xanax medication. The money and Xanax were locked up when she was in the hospital so I assumed they would do the same at this place but apparently not and they claimed no responsibility. Really bad place, not good people and poorly managed.

My brother was admitted with pneumonia and was not given any physical therapy or cupping on his back. The food was bland to say the least. They seemed to be short staffed all hours of the day and night. He went into sepsis and was groaning at 12:00 pm when I spoke to him. I called and spoke with a nurse's aide and she said all his vitals were fine that morning. I asked her to check on him again because he was groaning badly. By 5:00 pm he was rushed by ambulance to Yavapai Regional Medical Center. The ER staff questioned why he had not been sent over sooner? He went into a coma and never regained consciousness. He was in intensive care and put on dialysis and life support. He passed away 12 hrs. after he was admitted. I feel the so called rehab center was negligent in their care and my only brother may still be with us if he had proper care. I DO NOT recommend this place to anyone with a loved one who needs proper care and rehabilitation.

My mother was a victim of Granite Creek making employees work with Covid. She died. They took horrible care of her and I have everything documented.

My mom was in Granite Creek Health & Rehab Center. That place was horrible. There's nothing I like about that place. The staff was horrible. There was no communication. One day, they took my mom to the doctor, and they did not even call or tell me that they were taking her. The food was terrible. I know it's COVID, and they're short-handed, but there's no reason for the lack of communication. It's in a nice area, there were lots of trees, and it looked neat and clean. They just did not take care of my mom at all.

This place is nice inside and they are remodeling in the front. The room has a great view and my uncle enjoys it. He likes the salads and really likes some of the nurses. It’s quiet a lot and he’s bored but he’s doing good.

Las Fuentes Care Center was absolutely atrocious. They were in the worst level of atrocious I can think of. They have horrible service. When we pulled my grandmother out of there, they had already completed the paperwork for her weight checks in advance.

The care from this place was fair, but the food was terrible. They also didn't provide as good a care as I had originally thought given that they have been in the community for as long as I can remember.
