I was very pleased with the services ( nurse and PT) that were assigned to me after my knee replacement surgery. The administrative staff was always available and responsive. When I called about a scheduling problem it was addressed immediately. Rebecca, the first nurse to come to evaluate was very warm, and supportive. The PT's provided good care and suggestions. Overall the care was very good and I felt I was in competent hands.

I was appointed Ismael as my in home therapist. He was absolutely amazing. He was patient and showed me some therapy I never had. He stayed with me making sure that I was doing everything correctly. He is a credit to your organization. He was always on time. I was taken off my walker and used my cane instead. He also showed me the correct way of going up and the stairs.

This agency is the worst. You are given no schedule of when the nurse will be arriving so you have to wait around all day waiting for them to call and say they are on the way. Got to the end of the contract and suddenly I had phone call after phone call demanding the names and numbers of my Doctors for no reason. Finally, one of their Nurses must have got the number and name of my primary Doctor from a pill bottle and they had the gall to try and get my other Doctors number so they could try to get the contract extended.

My dad was a patient with diabetic wounds on his leg, and Revival was our health care attendants. We were fortunate enough to have TATIANA as his nurse. Once a schedule was established by her to twice a week visits, she was always on time, always called prior to arriving and she always had the necessary supplies. She was absolutely awesome. She was caring, had a gentle hand and an absolutely sweet, impeccable and wonderful bedside manner. If she was concerned about him, she would always call me and voice those concerns. She treated my dad like a human being and not just a "job". Revival is lucky to have her, and we were lucky enough to have her take care of my dad. So Thank you!

Protest if the hospital says they are giving your case to Revival. One nurse insulted me as I was not answering the door and refused to believe that he was at the wrong address. The other nurses - never the same one - were competent. The Physical Therapist was fine for the few times she came. I never saw a Home Help Aide. Revival said they gave me care until almost end of November, but that was not true. I await the Medicare papers and shall challenge payment for totally inadequate assistance.

My mother is the patient, and she is recovering from a broken hip. We were told by the rehab hospital that Revival would provide nurse evaluations and then Physical Therapy. The nurse who came and the PT who came were excellent. But the company is AWFUL. There are separate departments for PT, Nursing, Occupational Therapy, and Insurance. The clinical case manager knew nothing about what the various departments were doing, was unable to give me any kind of schedule, and when I asked for a schedule and the name of the person who was coming, the answer I got was, "What difference does it make? The patient is homebound so isn't going anywhere." My jaw was on the floor. The one time PT came (and my mother has been scheduled for more than one visit) the person came at 6:00 PM, after we were waiting all day. The reason I'm giving one star is because the actual people who came to see my mother were very good. The business end of the business is terrible.

My father was recently on service and we were very happy with the care provided. The nurse went above and beyond to ensure my father had the care he needed. The staff were kind, professional, patient and really sensitive in the care provided. I would highly recommend this agency.

My mother-in-law was on service with Revival 4 different times. The service she received was impeccable. The nurses and therapists were all very knowledgeable, thorough, caring, patient and warm. They made our transition from hospital and rehab to home as seamless as possible. I would highly recommend this agency.

My dad had hip replacement and spent time recuperating in a local rehab center. Afterwards he was put in Revival’s capable care. He did very well and is recovered. Revival came through (again) for our family and my siblings and I couldn’t be happier. Thank you Revival!!

All I know if the hospital or facility does't offer you Revival upon discharge insist on it and push for it. My elderly father had a major infection which required hospitalization for antibiotics. The hospital wanted to transfer him to a facility to continue treatments. The family wanted to take him home and arrange the IV at home for the next 3 months. The hospital was against it. They refused to call Revival because they have their own agency they work with. I called Revival and a Revival intake specialist came to the hospital to make all the arrangements. The hospital only wanted to use their pharmacy to supply the IV. The copay was $700+. I found a pharmacy to supply the IV for much less. The hospital said this is not an authorized pharmacy. Revival went to work and made it an authorized pharmacy within 30 minutes. Any stumbling block the hospital tried Revival cleared it. We brought my father home and the first morning the Revival nurse was there setting up the IV and teaching me how to do it. Afterwards he came 3 times a week to check. Revival sent a PT twice a week and kept my father walking. I am afraid to think what would have happened if he went to a nursing home. Revival is Reliable

I previously used Revival Home Health Care. They're in my network with my insurance and provide home care, physical therapy, and occupational therapy. I had one aide, but she was frequently late, then one day, she didn't show at all. Originally I was told I could pick the hours, but it got switched to when the girl was available. That didn't always work because sometimes she would have another job the same day. Overall, I was very satisfied with Revival Home Health Care. In their defense, they tried to accommodate my wishes. I'm familiar with other agencies, so I think Revival is better for me. They care more, and the staff is very good. There's always communication where they would call me or I would call them if something is bothering me, and they would in turn try to fix it.

Do not use this company. Revival will not give you a nurse, PT, or aide appointment until the morning of the visit day. One of their nurses never showed up, and I had to call the company to find out there was a problem. The representative did find a replacement--who arrived 4 hours after the original appointment-- and then hung up on me. You'll have to pester them to get a response to your requests. They promise to call you back, but don't. The client communication and customer service is dismal and Revival is simply ill-equipped to handle the demands of the disable.

My uncle was a client of Revival CHHA. That is the worst company. No one knows how to take care of patients. Nurses and managers have no clue why they are. Managers are rude. This company clinicains have no idea what is infection control. I was trying to get the name of the director but apparently that is a top secret.

revival is the worst company I have seen. the nurse showed up 3 days late, she lied and claimed she called and did not answer phone. she never left a message and my phone showed no missed calls. when she finally showed she was over 2 hours late. I never got a home attendant , I made numerous calls only to be told various stories. when they finally promised to send one, I received a call 20 minutes before attendant was supposed to show claiming they could not get one to the following day. when I called revival I was given all sorts of excuses. when I asked about my physical therapist I was told they were working on it. I called back that afternoon and spoke to the clinical director, and complained about the nurse I was told that they had documentation I did not answer phone. at which point I told revival to go to hell and I would find a responsible agency which could provide good service do not use revival you will be sorry

A truly personal feeling health agency. They put my mother's first and always go out of their way to make sure that we are happy and satisfied. But more importantly they make sure she is happy and comfortable. The team I've worked with became like family to me.
