My friend was taken to this facility for physical therapy after surgery. She was never briefed on the therapy plan. They had her putting pegs in holes and coloring on paper when she was in for therapy after repairing a broken femur. The staff ignored her requests for aid, the food was horrendous and caused her diarrhea. One morning a breakfast tray was brought and she could not eat the food, yet the tray remained in the room all day. That night I took it to the lunch room. The alarms are constantly sounding with no response. The staff purposely ignore her, and they get impatient and hostile when she had a migraine that prevented her from doing her therapy. After 7 days, still no shower. Pulled her out and got home health to do her pt.

I Do not recommend this place to anyone who cares about the welfare of their loved ones. My father in law is paralyzed and was left sitting on the side of the bed while staff walked to the kitchen....needless to say he fell. His bed has no side rails and he was yelling I'm falling. He has developed bed sores and only received one shower during July, 3 total in the 2 months he has been there. He has been injured while moving him and his room smells of urine so bad that it's hard to even breath. He was left outside in his wheel chair until 12:30 am...remember he's paralyzed and can't wheel himself. He was in pain and begged for staff to call his son or 911 and they pulled his curtain and shut his door. After several hours of him yelling help me, help me, please call 911, they called. We were not notified when he was moved to the hospital. We found out after a family member went to visit and was told he was not there. He was admitted into icu with severe urinary infection, low blood pressure and pneumonia. We have had several conversations with staff who listened but did not make any changes that had not already been promised. We had to suggest a sign sheet for those who turned him. Beyond disappointed and hurts to hear about how my loved one was treated. He will not be returning when leaving the hospital.

poor staff help adminestrater was not helpful and unconcerned with theft from resident when i reported this to her staff seemed unconcerned about there job i would highly recomend looking elsewhere for better place i would not entrust this place with my family

I have visited this nursing home on approximately twenty-five occasions over the past three years. Positives: This nursing home has a diverse patient population. More sepcifically, they tend to provide care for residents with behavior issues and are willing to take on challenging situations. Also, the home will often accept those who have applied for Medicaid but don't yet have it. I have met resident's loved ones that were pleased with the care provided. Negatives: On most visits I noticed at least one resident who had food on their clothing and on their faces. It is not a "nice looking" nursing home. Seems very sterile and unhomelike. It doesn't present as a comfy type atmosphere. The upper level management does not seem accessible. Also, the home seems crowded and with little space. It presents as a little chaotic during lunch times and during the typical visiting hours.Overall, I would rate this nursing home as a "two out of five".

Pinecrest Nursing Home is in a great location in Tyler since it is not located on a busy or main road. The ground are beautifully sculpted and the front lobby reminds you more of a hotel than an assisted living facility. The nurse's and staff were very friendly, but seemed abrupt at times. My only downfall of the facility was the lack of interaction between the staff and patients. Many of the patients were wandering up and down the hallways or seemed parked in front of a television. There were patients in the activity room, but no one was there to lead them in something. Some just sat at a dining table, reading a book or staring off into space. While the staff was friendly to myself and my husband, I was hoping to see more of it go towards their residents.
