"I visited Azalea Trail Nursing and Rehabilitation Center while researching care options for a family member. From the moment I arrived, I was impressed with how clean, quiet, and well-maintained the facility was. The environment felt calm and professional, and the staff greeted me warmly, even though I was just a visitor. I had the chance to observe a few resident interactions, and it was clear that the caregivers were attentive and genuinely kind. The facility had pleasant common areas, and I noticed a few residents enjoying time in a shared space, which gave me a sense of community and engagement. While I can’t speak to the care firsthand, my visit left me with a positive impression of how the center is run. It’s definitely a facility I would consider further when the time comes for long-term care."

"We have had several members of our family reside at this facility. We have been somewhat satisfied by the care given. Most of the caregivers are very good. And give adequate care. Facility is old the residant rooms are small and could be cleaner. Problems have for the most part been addressed to our satisfaction, but some have not. Food could be a lot better, not very appetizing "

"The home may be old, but the staff are VERY friendly. Everyone greets you as you enter and all are eager to help in any way. The food is the best I have ever since in a nursing home and they are always happy to offer a guest tray when I would visit. The facility is clean and the care is exceptional. The activities are wonderful and range from the traditional Bingo to silly string wars and hallway bowling. They even have a monthly painting party and the residents get to paint a canvas with the direction of an artist. It is a joy to watch this activity. They are active in our community and the staff and residents attend most of the parades and events and have participated in the multiple floats in our local parades. The rooms are small but are very homelike with personal items and decorations. They have multiple outdoor areas for residents and family to visit and the flowerbeds are beautiful I would highly recommend this facility to anyone with a loved one in need!!"

"I have visited this facility approximately 15 times in the past three years. I would rate this nursing home as "slightly above average" for the following reasons. Positives: It is a relatively small nursing home in a rural community. Many of the residents have lived in Wood County for many years and people tend to know each other from the past. Familiarity is a good thing. For the most part staff is friendly and I believe they take good care of their residents. This is most important. For the most part staff seem accessible for questions. Negative: Though looks of a nursing home are not nearly as important as care the nursing home doesn't have a comfortabl feel to it in my opinion. It doesn't feel like a "homely" atmosphere. Despite this I would feel reasonably comfortable if a loved one lives here and would feel comfortable recommending this nursing home to others. Unfortunately there is only two choices for nursing homes in Mineola. I think I would feel comfortable with both of the homes. "

"My husband is in Mineola. It is small and clean. I have been very pleased with the care that my husband is getting. They help him into his wheelchair. He gets the immediate personal care that he needs. They have bingo games and entertainment. I think they had a country or western band yesterday that he was telling me about. Their food is like school cafeteria food. It take them less than five minutes to get to my husband when he rings. He is always clean, and they always give him a nice razor shave, so he is feeling better because he looks better."

"They did a good job with healthy food and cleanliness, but their interaction with residents was not what I would desire. Also, they weren't good about calling the doctor, hospital, or next of kin when medical care was needed. They would wait for local nurses to look over patients and try healing them for about a week before calling anyone. Because of this my grandfather's health ended up failing. The doctor who was finally called in said he wouldn't have lost his life if he would have received proper treatment sooner. "
