I visited Azalea Trail Nursing and Rehabilitation Center while researching care options for a family member. From the moment I arrived, I was impressed with how clean, quiet, and well-maintained the facility was. The environment felt calm and professional, and the staff greeted me warmly, even though I was just a visitor. I had the chance to observe a few resident interactions, and it was clear that the caregivers were attentive and genuinely kind. The facility had pleasant common areas, and I noticed a few residents enjoying time in a shared space, which gave me a sense of community and engagement. While I can’t speak to the care firsthand, my visit left me with a positive impression of how the center is run. It’s definitely a facility I would consider further when the time comes for long-term care.

I have parent in Azalea. Facility was clean and well kept. Staff was less than desirable. They treat family with very little respect.

My parents were at Azalea Trail Nursing and Rehab. The staff were wonderful and the rooms were very good. Their food is wonderful and in fact, they also cater food in town. They have bingo, arts and crafts, and a TV room for their residents to come together. They also have road trips. Their Alzheimer's unit is very secured.

Grand Saline Nursing and Rehabilitation Center is a small nursing home located in Grand Saline, Texas. I have visited this nursing home two times and both times I was impressed with the friendliness and caring attitude of the staff. The nursing home was clean and well taken care of. I seemed to feel very welcome upon entering the home and was greeted by the nursing home's administrator. This nursing home is not owned by a corporation but is family owned and treated like family owned home. The residents appeared well taken care of and I had an opportunity to speak with two of the residents. Both expressed gratitude towards how well they were taken care of. Azalea Trail has a different feel to it than most older nursing homes. It doesn't feel old and stale. The staff is friendly and you feel welcome. I would definately trust my loved ones to be taken care of in this home. For these reason I would give this nursing home five stars.
