"I was at Rancho Mesa Care Center. The physical therapy was good. I got a lot stronger and better. The food was not very good because I'm so picky. They kept giving me the kind of things that I don't like. Even though I told them over and over, they'd still give me what I didn't like. I was trying to get my appetite back, and so that wasn't helpful. The CNAs were very great. They worked very hard. They tried to get me a wheelchair, but my insurance wouldn't cover it. I was in one room with three other people, and they all spoke Spanish all the time. They were really loud and noisy. Then I was moved to another room, where it was just one other person. It was nice, quiet, and peaceful. I like that. They have rooms where it's two or three or four in a room. Because of COVID, we had to stay in our room. The activity would be coloring, or they let us go outside sometimes. That was nice because then you get exposed to the nice trees, birds, and the mountains."

"My mother is at Upland Rehabilitation and Care Center for rehab because she broke her right femur. It's a clean place. It was redone since the last time I was there years ago. There are three people in a room. My mother is taken care of by a CNA, an LPN, and an RN. She has a wound care nurse that treats her wounds every day, and a wound care doctor that visits her once a week on a Thursday. The care is diligent. They do their very best that they can. It's an older facility. They do well keeping it up. Every skilled nursing facility that you go to is always going to have a strange smell compared to what you come from at home, because there are a lot of people being taken care of in those places. So it's not going to have the natural, fresh smell that you have at home. It's got old people in it. You have to take that into consideration. The people who are in charge of the activities are always coming to get them to do activities. As far as value for the money, they get their money's worth."

"This is a small skilled nursing facility tucked into a residential neighborhood. The grounds are quaint, but there's not much parking. The rooms were clean and are semi-private. I didn't see too many patients out in the hallways and the ones I saw looked clean; I didn't detect any odors. The menu included typical hospital fare and I don't think the food is made from scratch. The lobby was recently remodeled as were the grounds so that patients can use them for rehab. The staff I encountered were friendly. "

"I had a friend who used to live at Heritage Court long time and since then it has been remodeled and everything is really nice. When I went there for dinner before for my friend, their rooms are double rooms, but now they are all single rooms and each of them has their own microwave and refrigerator which is really nice and it's fairly spacious. My mother-in law is there right now and she has a nice view out of the window where she sees some trees and grass. She also likes to be outside and they have a really nice patio with flowers and a fountain. They have a really huge living room with a huge TV and they do activities there and they have another activity room. They have a lot of activities. She likes their Bible study and they also have church activities on Sunday. They also have trivia, contest or games. The food is good and she likes the food. The people that worked there seemed competent and nice. They are very welcoming to visitors. We think that the care she is getting is good. The residents are also friendly and they were pretty much mentally alert. The nice thing about the facility also is that they have a nursing facility next door. One time she broke her hip and she went into their nursing facility and after that she went back to their assisted living facility so that was an easy transition. Their cost was also reasonable compared with the other facilities that we've looked at. The place is very clean and nicely decorated. They have a good air conditioning system. Overall, we've been very pleased with it. "

"I was able to speak with a speech therapist who was working at this facility and told me that Plott facilities place a big emphasis on activities for rehab. I did see a lot of patients gathered in the activity room and dining room. The facility itself is an older building and could use a facelift; the vinyl floor tiles were cracked. While the rooms appeared clean, I was able to detect a slight odor. The patients looked clean and seemed content as they gathered around one of the TV rooms. I liked that the building has lots of commercial windows to let light in and a receptionist desk at the entrance. "

"Overall the facility was OK. The rooms where basic but met my needs. I had three roommates while I was there. The Physical Therapy team was awesome and knowledgeable. The food needed some help. The nursing staff were great. They were kind and very patient (and you need a lot of that in this type of work). If I had to go back or recommend a facility for anyone I would. "

"Clean but small rooms Food balanced and good taste Staff was excellent, friendly, attentive, very efficient and good at their jobs. Krista was the best at wound care and could not be more dedicated. Carlos in physical therapy made it possible for my husband to transfer home. He made him enjoy getting mobile! Can't begin to name them all Adora Adora, Alyssa, Jennifer, Diane, Jot all excellent at their jobs and really care. Ontario Health Care Center is what you want a nursing facility to be but hardly ever find. Highly recommended! Thank you."

"My mother-in-law has been in Montclair Manor. The people who works there are very nice. The management was very informative and caring about her costs and things like that. We had a little bit of trouble getting it accurate in the end. There were more bills that came in that we didn't understand. They overcharged us but they reimbursed us and so we were happy. The rooms were adequate and mom was a fall-risk person so they put mats on the floor and they put the bed down and they had adequate stuff for her to prevent any type of fall. The food was good. It was good that they served her a lot of snacks. She did very well there. The facility was old but clean. They needed a lot of maintenance there like painting and some things that needed to be replaced. "

"We’ve often heard how good the care is at Claremont Manor but they were always full. This past summer, when our mother again needed assistance, Claremont Manor had an opening and we jumped at the opportunity. Everything we had heard about Claremont Manor was true. It’s a very safe and welcoming environment. The management, staff and care team are accommodating, attentive, friendly, and helpful. The communication was informative and very thorough. The physical therapy team was helpful and patient. We are so thankful to have chosen Claremont Manor."

"This is a 99 bed skilled nursing home with a nice big green lawn and grounds that are well manicured that is owned by a corporation. The surrounding neighborhood was nice. The interior was nice and clean as were the rooms I saw; private and semi-private. I didn’t detect any obnoxious odors as I walked around. The patients were clean and seemed as content as possible; some were seated in wheelchairs along the hallway. The food that I saw was not fresh, but the posted menu was varied. There were a lot of activities posted for patient rehab. "

"My husband was at Pilgrim Place Health Services Center. It had occupational therapy and physical therapy. They had activities there. It's really a good skilled nursing facility. They have a huge assisted living facility there for seniors. It took a while to get his medicine figured out. He has Parkinson's, so it's very specific, like he has to have his medicine six times a day, and the medicine people are just used to three times a day, so it kind of threw them off. Sometimes, he didn't get his medicine on time. That was probably the biggest complaint, but everything else was good. The food was good. He loved his physical therapy. He liked the activities. It's clean. It feels like a home, even though it's skilled nursing. It's nicely designed with artwork, and the staff is great. The nurses are great. It's a great place. They had art, movies, singing, and exercise. They had a lot of activities. They have a courtyard. When you first walk into Pilgrim Place, there's a huge patio to sit outside. They have a nice room for family, like a big dining room for people to come visit. They have another room for church. Medicare paid for it."

"I greatly enjoy visiting my grandfather at this nursing home. He is renting an apartment in this senior center. My grandmother and grandfather purchased together, but she passed away while living here. The staff was extremely accommodating toward our family during this very difficult time. We were able to use the community center to host her memorial service reception. It came with a kitchen area and everything we needed for this event. My second comment is that I am very pleased that Inland Christian has a health monitoring system where the resident must push a button inside their unit to let them know their health is doing ok. The apartment is beautifully designed with a kitchen, living room, 2 bed rooms, and 2 bathrooms. They even have a patio and laundry room right inside their unit. This senior center/nursing home is a great place for any elder to retire. The only negative comment I have is in regard to parking. The parking is great for a resident as they are able to park in a covered garage. However, my grandfather has very bad knees so the walk can be very long when carrying in groceries. The parking is much worse for a visitor. A visitor has to park behind the building, and then walk to the main entrance where they take the elevator or stairs to reach the appropriate level. The day I visited it was raining so I was soaking wet by the time I reached the apartment building. Overall, I have been very satisfied with Inland Christian Home. A great staff and a wonderful place to call home has made my grandfather very happy. I highly recommend it! "

"Mt. San Antonio Gardens is near where I live. It's a lifecare community and it is very pleasant. It has a central dining hall, library, and community dining room. They have apartment type facilities and a lot of small duplexes/cottages. The grounds are very clean. They have a couple of open houses and information sessions a couple of times a year and I've tasted their food. It was delicious. The dining area was clean, neat, and didn't smell at all."

"I am a patient here for the third time due to various leg surgeries. I have progressed so much in the last month. I am now walking, climbing stairs, transferring to a regular chair, and to the car. I have spent 3 years in a wheelchair. I truly believe my vast improvement is due to the staff in general. I find all to be encouraging, caring, helpful, and knowledgeable. I came back this time due to above reasons. I have recommended Claremont Care Center to others due my satisfaction. I have been in other facilities and did not like them. "
