My brother is staying at Southeast Rehab and is getting excellent care. He says even the food is pretty good! I've been in contact with the case manager there and am very impressed with her actions regarding my brother's treatment, her consideration of keeping me in the loop, and her compassion for his recovery. Well this place almost killed my father in less than 24 hours. He was admitted at 2pm and I was there from 3pm-5pm. I was immediately concerned when I asked the guy at the front desk to tell me what room he was in - he said he didn't know how to look that up, didn't work there, and was just filling in for somebody that called in. He actually just kept repeating our last name and making fun of it - it was really odd. He suggested we walk up and down the halls and look into the rooms until we found him. Thankfully the nurse's station was able to tell us what room he was in. Everything seemed fine during that visit beyond the front desk, though his dinner, which you cannot pick, was a huge roll with one piece of ham in it. There is no option (we did see an "alternate" option on the menu but think that may just be for people with dietary needs as he wasn't give any option). But at 10pm my father called me saying he had been pressing the button for a nurse since 9pm (when he is supposed to take his meds). A nurse once popped her head in to say she would get his meds then never returned, and nobody responded to his call for a nurse after that. I called the nurse's station numerous times over the next hour but nobody picked up. At midnight he confirmed to me he still hadn't seen anyone with his meds. I called the cops who responded (apparently if I tell you what they said it is hearsay and they won't post my review so assume what you want, I'll just say they knew the place), and I drove up there myself. I got there at 12:30 and they had just given him some of his meds, but didn't have some of the meds he is supposed to take (including his morphine). Turns out they hadn't even ordered his meds that day. At 3:30am, he called me to say he couldn't breathe and didn't think he was going to make it through the night. They wouldn't give him an inhaler because they didn't have a prescription for it - note my father has been on an inhaler with a prescription for 7 years. They could have called the hospital to get that info but didn't. They gave him a medication to see if that would help and he decided to give that a chance. I called him the next morning and he was 100 times worse. I called the nurse's station to call an ambulance...after I was put on hold for 15 minutes waiting for his nurse to pick up the phone, I hung up and called the ambulance myself. By the time he got to the hospital he was in very bad shape - barely conscious, couldn't talk. Now that he has been in the hospital for a couple days he is doing much better (thankfully). To top things off, we realized after a couple days they had put his shoes in the closet at the rehab facility and we didn't have them. I called numerous times that night but you guessed it - no answer. I finally just went there and picked them up (I will say the day nurse that was there when I picked up his shoes seemed much friendlier than the night nurse). Long story short - I would never, ever take another loved one to this facility again. I truly think that had we not gotten him out that morning, he would have died there.

This facility is the most piss poor excuse for a nurse care facility, the staff is miserable, CNAs and nurses are very unpleasant and uncooperative to deal with, people are often left uncleaned or given showers when they're supposed to get them, healthcare or being able to get appointments correct or booked correct is nearly impossible, as a paraplegic they filled my ears with nothing but lies when I first came here and told me they had all the equipment I needed and they had zero, I owe the ambassador program which was created to solve this problem a great debt of gratitude for forcing the issues to get me the better care and equipment I needed, Paula and Nei I thank you so much for all of your help and I am so grateful you guys came to this establishment, if it wasn't for you guys just care and time this place would have never done anything for me, you guys will be greatly missed and it is such a sad day that you guys have to leave, without people like you it is a share effect that these CNAs and nurses will simply fall back into their old habits and this establishment will go back downhill without you you guys were the greatest thing that ever could have happened to this place, thank you so much for your help, this place has so much capability of offering so much more but the help is awful

My father died of COVID contracted in this facility, one of at least 20 dead/50+ infected. The staff showed complete disregard of human life in their response to the pandemic. They called in the National Guard and only tested 4 residents when they had a chance to do something.

The facility is understaffed. Have visited all shifts, Day and Eve and weekend the worst. Very little interraction between staff and patients with dementia. Staff always in a hurry

We transferred my father to Southeast Rehab. I thought they were great. The place was clean. Some places smell, but they never did. The staff was very receptive to the family's needs as well as my father's needs, both physically and health-wise. My father was treated very well, and that was the bottom line. They did a good job with my dad. They kept us updated and were always in contact with us. I never had any problem getting in touch with anybody. They returned my calls, and they had his best interests in mind. I was happy with that response. We found them to be reasonable, and they were very accommodating from the start and right through the very end.
