I am a friend or relative of a current/past resident

The staff treat family members as if the have some kind of authority over us and we have to stop bothering them so they can get their reports done, especially a nurse named Barbara on Nightengale. My wife has waited hours to get catheterized to empty her bladder. The staff ignore the call bell. It might as well not be there. The food is as bland as sawdust. Very few of the staff acts like they care. Physical therapist start sessions they don't finish. Not a place to leave anyone you care about.

My mother was at Oak Manor Center for Rehabilitation and Healthcare. They provided the meals and they took care of her. The front staff is very welcoming. There are certain staff there that are wonderful, and then there are some staff that are not that wonderful. I would go visit my mother every day, and I would have to say they kept the facility clean, and they took very seriously the COVID checks like you get checked every three days for COVID before entering the facility. They were very understanding about what we were going through with my mother. They're sympathetic. I was also so happy with the room my mother was in. The room was a two-people room. When she first got there, she had a roommate and then the roommate left, and then she got a roommate later on. Even though my mother was long-term care, the roommate that she had was like a rehab roommate, so they weren't there too long. She had a private room and that way the family could come visit because they knew my mother had like six months to live. I never ate there. Their menu was not that extensive. But the good thing is that the residents always had an option if they didn't want what was on the menu. They could get like a grilled cheese sandwich or something else. The place was very clean, too.

I am/was a resident of this facility

I recently was at Oak Manor for a period of weeks following release from a hospital where I was treated for a dangerous infection. Oak Manor was supposed to continue a course of antibiotic treatment by infusion pump. In my opinion, medical care at Oak Manor seemed dangerously inadequate. (1) Nurses lacked the skill or training to operate the infusion pump. Some assigned nurses refused to try, and as I result, I was not receiving antibiotics as prescribed for days at a time. I tried to complain, but communication was difficult, and it was several days before I could count on receiving antibiotics correctly. (2) I also could not count on receiving other oral medications in the morning or evening as I should have. More than once, nurses skipped my medications. In one instance, a nurse saw me and said she would return shortly with my nighttime medication, but after an hour or two she did not. When I tried to get the medications, the nighttime nurse then on duty told me that the nurse who should have given me the nighttime medications had already left and that she, the nighttime nurse, refused to give me my medications then because she didn’t what “to get in trouble.” (3) In my opinion, a nurse or doctor should have monitored my condition at least daily, but they did not. As a result, nobody diagnosed a deep vein thrombosis that I experienced while in Oak Manor’s care for a period of days. I was examined only after I complained that my condition was not improving. (4) Shortly after I was admitted to Oak Manor, I received a wound assessment for open wounds connected to the infection. A nursing order was entered to dress the wounds. But for several days, nurses ignored it – my wounds not receive any dressings until days later. In my opinion, poor medical care at Oak Manor endangered my health and life.

I am a friend or relative of a current/past resident

My mom has been there a little over two years. I would visit two to three times a week and talk to her clean up her nightstand and making sure she had her goodies. I would change out her seasonal clothes if needed. When the shut down happened because of COVID 19 I was devastated not being able to see her. I was put in touch with Sharda and Jackie the Activities Department, she was so nice and positive. Helping me work thru my anxiety by not being able to visit my mom, But facetiming my mom caught on quickly, Sharda was so nice, I would schedule times and she would sit with my mom and we talk about bingo, trivia and other games they had played. She kept me up to date on her snack needs also. Thank you. I would also like to comment about housekeeping. I have a several occasions asked for something for my mom. And on the times it was the same person Tresha She not only got the items for me and with a smile and a yes mam and then picked other stuff up off the floor. Such an incredible worker. Thank you To the new Administer Mr. Johnson fantastic job kudos. Thank you. Ms. Sara the best receptionist most knowledgeable person to meet. She is polite, sweat , nice, professional great asset. Thank you. Thank you at all the wonderful nurses I am horrible with names but you all are the front end. when I call that I am taking my mom to an appointment she's ready, that's so important to be on time. My mom has only said nice things about all of you. Thank you. To everyone Thank you I cant say it enough.

I am a friend or relative of a current/past resident

My dad was in the rehab center at Oak Manor Healthcare Center. It's a decent place, but it's not a five-star place. They need more staffing. Most of the staff members were very professional and nice. The food was below adequate. The room was not very modern the one that my dad was in. It was just basic stuff like a wooden cabinet, wooden dresser, and 19-inch TV. The window had one shade and the other shade was missing. It wasn't a very up-to-date room. They had good activities for the people. They put out a schedule of activities for the residents, but my dad was bedridden, so he didn't get to do any activities. The activity person did come by daily and read to him. They had a chapel, a gym, and a dining room.

This facility is the awful but I must be fair and say that it does have a few jewels in the place. My mom was admitted on 6/11/20 after being taken off a ventilator and things started off pretty good. My mom complained to me and other staff that she was left in her waste for over ten hours on two different occasions. She asked them to change her and then said they would be back and shut the door. She was not changed until after 6am the next day. I did file a grievance on 6/20 and received a follow up call from the unit manager Sophia on 6/26. Some of the staff need to work on their attitude, empathy, customer service and overall care. They need to realize we miss our parents and we care about them. Today My mom called me in a panic asking for me to come get her, she was freezing and afraid she would get pneumonia again because she was put next to an air conditioner. She also informed me that she was not given dinner on June 25, 2020 which is concerning since she wasn't eating too much. This is an awful place but I had great pleasure of speaking with Nasira one of her nurses. I also want to give recognition to Patricia the Social Worker who went and spoke with my mother and made her so comfortable she agreed to stay. Patrice, Isaac, Michelle and Bryan were great too. The administrator Margaret needs to work on improving her level of communication, empathy and at least act like she cares even if she doesn't. She is the worse. Imagine your love ones experience under her management. It starts from the head down but again those few jewels did allow me to get some sleep and not worry as much. I will be making an official complaint with the Corporate office, CMS and DOH. This place could be better if they had a better leader. Five stars to Nasiria, Michelle and Patricia!!!

Get caregivers in the facility that actually care and love their work and people. What you have now are people who just want a check regardless of how good they work. Instead of my relative’s health improving it was deteriorating. The relative was dehydrating the IV was near the bed, detached and broken.

I visited this facility

I will mainly talk about the admissions staff. I went with a friend to scope out places for nursing care. The admissions lady i think her name is Melissa or Lisa? toured us and was so fantastic with her knowledge of the facility and about nursing care in general. Out of many of the places we toured she was the most warm and professional. We asked a lot of questions and she was just going with it and answering everything. I personally would have chose the place just because of her. My friend did not chose this place as her parent died in hospital. I would say its worth looking into make sure you ask for admissions Lisa or Melissa I believe, sorry if I got the name wrong!

I am/was a resident of this facility

I was a patient there for Rehab. This past Sept.03 Through Sept.25th. I came fr. Cressant cittes which was a hell old. Now when I got to Oat Manor , it was still under the name of Holly Cross satuanary. I then met the new owner/CEO. he came to my room because I was crying and very upset. He came with is administrator. Because I was a patient there two years befor he Brought Holly Cross I told him how bad things were. We met through my Breakfast which was very bad he then called in the Kitchen Manger as Well as hi Assiistant. I displayed the Cold Horrible Food. the kitchen new me because I am a graduate Food service manger/ Cook. the meeting went so well I was given my own personal hot craft the GNA's was told to make sure I have hot coffee all DAY as soon as the kitchen staff comes on my sure I get my coffee. all changes were made in the entire building to make things better.I left feeling Very good about Oak Manor and since I have to have my right hip replacement surgury I will be going back to Oak Manor for my Rehab. I sincerly Recommend any one to come and see What I see. THE OWNER WORKED WITH ME he made sure all my needs were met. Check it out

I visited this facility

they make you choose a star - they don't deserve one. They gave my father high blood pressure med and 2 pain meds and wondered why his bp bottomed out. they discharged him with an infection that needs surgery. He had to go to the hospital from this place because he was so dehydrated. They said they provided counseling - yet he never saw a counselor. The place is clean - that is the only good thing about this place. The employees act like they don't want to be there - they shouldn't be there. I saw patients who have no visitors and the staff were talking about them in the hallway and not saying nice things. Maybe the employees need to be treated the way they treat the patients and they would see how it feels to get no respect, to be shown no care, to be stripped of even more dignity. I can only hope that oneday they will experience the same mistreatment.

I am a friend or relative of a resident

My mom is in Sanctuary at Holy Cross. The staff is engaged while she is there. She receives physical and occupational therapy and hasn't had any complaints. She is dealing with her dietary requirements, and when we express a need or a concern, they act on it. They seem to be attentive to the needs of my mother and other senior citizens. They have activities for them to engage in like bingo. My mom is not always cooperative with eating, but the staff member is very supportive in getting her to eat. In fact, she responds better to them than to me when I am trying to make her eat something. She is in a semi-private room about the size of a hospital room. She has a special bed because she is pretty much bedridden. She has something inflatable in her bed that works with her condition and reduces the potential for her developing bed sores.

Sanctuary at Holy Cross is a wonderful facility. My parents are both in there. They moved there two years ago. Their staff is very professional and very caring. They check in on them regularly.

Think Twice

Right now I would think twice about sending your family member to Holy Cross. While some of the staff is very dedicated, most are not. I believe that the facility has deteriorated during the last year. Food is unappetizing, often cold, and sometimes served more than one hour late. The hallway smells. The rehab facility is too small and overcrowded. Patients have long waits for diaper changes. I believe that the physicians are uncaring, unavailable, and second rate. Having said all that, there are some positive attributes. The decor is fine. It is not a depressing facility. Some wings are actually quite nice. I have not seen any signs of patient abuse and I don't hear any patients cry out from neglect. I strongly recommend that you visit other facilities before sending a family member here.
