I was a patient at this facility after falling and breaking my hip. I had also been a patient four years ago and will say I saw much improvement from my first stay. I went back to this facility because I remembered the Therapist there and how much they help me. Before the Physical Therapist section had been crowded and I had seen the new facility after visiting a friend there and also seen two of the therapists still there that had helped me. I was trusting they were still there and also knowing the new facilities, knew it had to be really up to date. I appreciate so much the main three that worked with me. Fortunately the main one was one I had worked with before. I certainly appreciated the fact that the rooms were private and I did not have to worry about someone else in the room with me. At first I was a little disappointed in reaction time to some calls I made. It took several days to learn the ins and outs and how the procedure worked. I like everyone else, am sure, saw people around or heard some and wanted help. I took me several days to realize everyone had the things which they were able to do. Many did help with simple things like moving something for me. I had wonderful caring aids. Got to know many since I was there for three weeks and they all assisted me so well. It did take me a while to realize I needed to ask for a shower because it was not clear to me at first how the process worked. I will say, though, at first I was not getting along very well at all and needed more assistance. I was always provided the towels and wash cloths so that I could "sponge bath" in the room. The food was OK, however, as usual, just not homecooked. There were a number of meals I kind of enjoyed, however, I noticed when I ordered a sandwich one day there were no condiments. There was not anyplace on the menu that I noticed where you would list them. Also, I was not on a salt free diet, however at first I never got salt. Sometimes the salads came with the dressing and other times not. Again, did not see on the menu where you would request any. Blue Cheese dressing was sent to me on one occasion , and that is the one dressing I do not like. I asked for and got another dressing, but of course because they needed to finish getting the food to the residents, I was almost finished with the meal before I got it. I would like to suggest getting blankets that are a little softer, as the blankets provided are very hard and not really comforting. Other than these few things, I was taken care of well by the staff at Centre Pointe. I especially appreciated the receptionists at the entrance as their patients in dealing with so many things as they change daily because of COVID. Unfortunately, this has brought on change for all of us and change is hard -- especially as we get older and need more help. I do agree that the staff (and not sure what level) needs to be a little more understanding of communications. I had a doctor's appointment and there were good to get me to it. Noone had let my husband know, and of course that really was not necessary in my case, because I was very alert and able to understand these type of things. It was not until the driver was taking me that he asked if someone was meeting me there. I was not sure if that should have been arranged or what they did to make sure I was OK once I got there. A staff member did come and let me know when my appointment was but that was only after I asked several times because I had been told they would be setting an appointment up for me. Could not understand why I had to ask someone who then transferred the message to get the answer back to me. To me "Communications' is one of the hardest words to get. To me this is because there is so much more that needs to be communicated, and hard to word it in such a way that it is understood especially since we are using fewer words to say something. Overall I was very pleased with my care at the facility. Most people were very friendly and I truly felt they enjoyed helping people.

The employees are rude, they treat residents with little respect. They are loud talking and playing in the halls. They tell you if you need anything to press the call button, but when you do you might see someone within the next 2-4 hours. Be sure to keep an eye on the nurses. They gave me mine several times with either to many or not enough of what I was supposed to get. It was a nightmare. Don't go to or send anyone you care about to this place. The food was awful half the time.

My mother right now is at the Centre Pointe Rehab. I am very, very disappointed. I'm thinking about pulling her and trying to find a better facility for her. I don't think they have my mother's best interest at heart. I have been there on occasions where she has had the same shirt on for three days. I go visit her every other day and if I go on a Monday and she has a certain shirt, I pick up her laundry. I do her laundry and I bring her clothes back, and then I go on Wednesday and she has the same blouse on, but different pants. They're just not caring enough to say, "Let me help you with your shirt," or "Do you need help with putting on a different shirt?" or whatever. She has advanced stage dementia and she's hard of hearing and her memory is not good. She doesn't really know to put her clothes away. I'm looking for a place that I think will be concerned about my mother's care, but I don't think Centre Pointe is. My mother says the food is not good. She loves to eat, but when she can't eat it, I figure it's pretty bad. She's in the rehab part. I don't know about long term, but I heard that it was very good. The reason she went to the rehab was to speed up the process of getting her to the long term, but the rehab section, I don't think has my mother's best interests at heart. The rooms are spacious. In the rehab, they have a half bath. I went in there and housekeeping hadn't cleaned up the toilet yet. I went to visit her today. She was up at night like a lot of dementia people. She said that she stayed up all night, and she ate her breakfast this morning, took her medication, and went to bed, but they noticed that she had a cough. She had been through pneumonia while being out there, and I'm afraid it just hasn't gone away. I'm looking to take her out and get her to someplace that can take care of her. My mother has therapy from 10:00 to 12:00, for her arms, and exercise on her legs. I met the therapy lady one time and she said Mom was doing real good. I went to visit her one time and she had somebody else's pants on, so I said I'll just start doing her laundry. They brought me her laundry, and I bagged up her laundry not looking at it. I get home and go to do her laundry. There were two shirts that were not hers and a pair of pants that were not hers, and it happened twice. I marked their clothes and said, these are not my mom's. I told them I really didn't like her wearing someone else's clothes when she had clothes of her own, and I know that her clothes are clean because I wash them. I wouldn't recommend rehab there for anyone unless you've got a sound mind, and you can get around on your own. The way I know about Centre Pointe was word of mouth. Their experience was not in the rehab, it was in their Centre Pointe extended living, and they were very satisfied, but this lady was in her right mind. She didn't have dementia or anything, and she could get around, so it's a different situation for somebody like that.

My mother went in for physical therapy following heart surgery. When she was able to start her PT, the therapists told this woman who had been able to walk a little bit with a cane not two weeks prior that if she couldn't walk with no assistance on day 1 of therapy that she'd never be able to walk again. They also mentioned that they worked on a point-based pay system, which might help for staff motivation, but also sacrifices quality of care for patients. Therapists also seemed absolutely clueless as to the quality of care given to patients, especially visiting one day to see my mother in tears with bloody fingernails after PT (one of them fell out when we were talking that day). Nursing staff seemed nice to and in front of visitors, but appeared otherwise after seeing my mother in a stress-induced delirium, deprived of nearly everything connecting her to the outside world, and not being able to remember her own son (she's back to normal after being home for a week). Nurses also seemed to deprive her of toilet use and openly told her to wet the bed. Standard COVID protocol was fine, but when a nurse tested positive, the facility closed off inside access for visitors for "safety" reasons. This came off as peculiar not just due to their original requirement of visitors to be fully vaccinated, but also because it forced everyone into a bottleneck around the facility entrance and creating a viral hazard rather than a safety measure. My mother's paperwork says I was given two and a half weeks of notice for her discharge. I was actually given only one week notice, with roughly 36 hours to find a facility she could be transferred to. Because no facility responded, I had no choice but to do home care as a guy on the autism spectrum with no one else to help. They provided a wheelchair and the word "toileting" with nothing for actual toileting like a commode or even so much as a bedpan. In short, my mother came to Centre Pointe with some mobility issues, and came out four weeks later needing a medical bed and a mechanical lift. If you can do at-home care for your loved one, then do it, otherwise avoid this place like the plague.

If I could give them a zero I would! Mother-in-law was taken there on a Wednesday afternoon, we told them about her panic attacks and breathing before she became a patient. Dr notes were given to them at transfer. We got a 4am call the next morning telling us they were sending her back to the er. They either didn't listen to what we said or were ill prepared for her. My husband called them multiple times afterward.... we have yet to hear from them. Very restrictive about visiting and you have to make an appointment. She is now at encompass and doing well!

I was a patient at this facility and I found the rehab department to be excellent. I was comfortable there and the staff meet most of needs.The room and facility were beautiful. I was in during the COVID 19 concerns and I got my shots while I was there. Activities were not available nor was I allowed to have visitors because of COVID concerns but I felt it was necessary considering what we were dealing with. My husband visited at the window and we FaceTime by phone. The staff was good. I would consider going there again because of the excellent rehabilitations. I thrive while in the facility. I regain my strength.

My 88-year-old father has had dementia for two years. He is also physically disabled, unable to walk without a walker. On April 21, 2020, after being at Cap. Reg. Med. Center for three days due to dehydration-related unmanageable behavior, he was transferred to Centre Point for physical rehab. Of course, we weren’t allowed to go inside so were unable to help him settle in and make sure he was properly taken care of. I contacted CP by email, detailing my father’s needs and also spoke with several employees in person. Due to his dementia, contacting him by phone would add to his considerable confusion. We provided CP with plenty of clean clothes as well as his prescription glasses. For the following two weeks we had no contact with my father and had no idea how well he was being cared for or how very little care he was receiving. When the day came to pick him up, we were shocked by his appearance! He was unshaven, was wearing rumpled unclean clothes and had a catheter bag hanging loosely from the bottom of his pants. The bag was nasty, full and didn’t look like it had been attended to in some time. When we had him settled in the car and were on our way home, it was readily apparent that he had not had a bath or brushed his teeth in days, possibly the entire time he was there! When we opened his suitcase, there were all the clean clothes we had sent for him, still neatly folded just as we had packed them, minus three new t-shirts and his glasses. I’ve contacted the administrators on several occasions regarding his glasses, but they were never found. On May 26 I spoke with the head administrator about being reimbursed for the purchase of new glasses. She indicated that we should go ahead and get his new glasses, email her the receipt, and reimbursement would be taken care of in a timely manner. It is now August 1 and we are yet to be reimbursed. DON’T TAKE YOUR LOVED ONES TO THIS PLACE!!

My mother had hip surgery at TMH & was transferred to this facility in April 2020 after a fall that fractured her hip. My sister and I realize this was all during the pandemic so of course, we could not visit her at all. We have never ever even seen the inside of the place. We tried on numerous occasions to contact them to get updates on her care. No one would ever answer the phone, except the receptionist and we would ask for a nurse or the administrator. We never got them and had to leave voice messages. They would not return calls and we had to keep trying our Mom's direct room number. Mom would never answer the phone so we couldn't ask her how it was going. We thought the physical therapist would call & keep us updated on her progress, but NO ONE did! Since we were unable to visit her and get up dates in person, this was the least the staff could do. But, they wouldn't even return calls. Our mother lives in an assisted living community with memory care so even if we were able to speak on the phone to her, she wouldn't have been able to tell us anything. This facility staff is so unprofessional, we would never recommend it to anyone, that is if they love their person!!!!!!!

Both parents have stayed at Centre Pointe, numerous times, for rehab after surgery/medical complications. Physical Therapy is wonderful but the nursing staff is awful. Even with bringing in our own medications, they cannot administer them correctly. And this is after 5 days of coaching and giving handwritten schedule of frequency. Director of Admin nor the Head of Nursing care. And caring starts at the top, so, if they don't care, you know the nurses and aids don't care. This is our 4th dealing with Centre Pointe, and it will be our last! I advise not leaving your loved ones at this facility as they will not receive medications correctly nor timely, and doses will be missed. Use them ONLY for outpatient therapy. TMH is much better when it comes to keeping the medications straight. I gave one star for nursing, but would give PT five stars.

I was fortunate enough to be placed in one of Centre Points newest room, complete with a beautiful bathroom and walk in shower. The employees here from R.N.s to CNAs and all others were always puntual, understanding, and kind. I also like what may be a new implemented plan. If your red light is on indicating you need assistance, anyone who sees the light has to stop and come in. From administration, to anyone just passing by. Great way to meet people! Last, but definitely not least! Physical Therapy, and Occupational. These are the people who work extremely hard to get you to your former self if not better! These folks are the people that make the difference of you returning to your happy home, or sorry to say, staying here or someplace else as a resident. Thank You all for helping me!!! Don’t take your family or loved one here. Was told how great this place was just to have my family member ignored once admitted . Not given shower, no PT . They didn’t even try. Had my love one moved and is doing much better . Food my terrible Don’t take anyone you care about there

Horrible place. If you love your family member or friend don’t go here. My experience was pretty much neglect. No one seems to be interested in patients. I arrived late afternoon on Friday I was given two a bologna sandwich nothing on it and water as a meal. I asked for my pain medicine as my leg was broken they said it was lost. Later that night I rang to get help to the bathroom and my roommate rang and 30 mins later someone sacting discussed that I bothered her even though all night people gathered in the hall talking and on phones. I asked for the supervisor to see what happened to my pain medicine l was told she wasmn’t lthere and noth l be done and they could give me an aspirin. The next morning I called someone to come get meas I would be better at home. I think I inally talked to supervisor as someone came in and said they had talked to Dr I asked if I could talk to him but was told no. No one really seemed to care about the patients. The Dr need to know this and please don’t let a family member go there without someone to oversee their care. Never got my medicine.

Centre Pointe Health and Rehab was great. The staff was good, the rooms were nice, and my mom was very comfortable. She's pleased with the food. They were also doing some construction work, but the place was clean.

My husband was admitted here after cervical spinal fusion. From day one the entire team welcomed us and reassured us of his care and their commitment to treat him well. They did just what they said. I'm happy to? say their care is complete... Body, Mind, & Spirit! PT, OT, Nursing, Housekeeping, Food Services, Business Office--SPOT ON!

My mother was a patient at this facility for one month. Admitted for Sciatica, she was there for physical therapy. She was admitted on Morphine and Percocet, given around the clock. Family expressed concern at the type and amount of narcotics she was given. Mom was placed in diapers although not incontinent. When questioned the staff said it was in case she was not able to make it to the bathroom. Mom soon became lethargic and complained of not feeling well. Then she became confused. The family requested to speak with a physician but was denied by nursing staff. We shared our concerns with the PA and the Physical Therapy staff. After a steady decline mom complained of chills. The nurses added blankets and we were told "this is just what old people do sometimes". I found my mother alone in her room completely unresponsive and with a 104 fever. Staff wanted to argue that she had been fine and the only thing different about her was she stopped pressing the call button that day.! Mom was diagnosed with a UTI at TMH ER and it was caused by e Coli... A direct result of being allowed to lay in unsanitary conditions. She became septic as a result and developed cardiomyopathy. Sadly she passed away in February 2017. A series of unfortunate events began at Centrepointe when my mother was being treated for "Sciatica". The facility has not been held accountable but any nurse should have known the classic signs of UTI in the elderly. Not one time was a urinalysis ordered. Mom was reprimanded for using her call button too often and told she would have to wait her turn when she had to go to the bathroom. She began to hold her urine until she couldn't any longer. My biggest regret is that I didn't remove her from this facility when my instincts told me too. If it had, my mother would probably be here today. Don't trust the care of your loved ones to the physician or nursing staff at this facility.

I am recovering from a stroke . The doctor told my husband that I would not make it through the night; good for me I did. He then told my husband that I would be bedridden for the rest of my life. Fortunately for me, this was not to be my fate. I was placed in a local facility where the treatment of the patients was horrendous. The ratio of patient to physical therapist was one-to-how ever many patients they had. It was horrible. After a series of falls, Centre Point was recommended to me. Since I have been at Centre Point, as an outpatient, I have improved tremendously. My therapist Risa G., who is certified in the area of vestibular and concussion management, demonstrates premier competencies and utilizes a variety of professional. skills and accoutrements to improve my gross motor skills, increased range of motion, better balance, and enhanced muscle tone. The other therapists are also very helpful and encouraging. I have not had a fall since I began therapy at Centre Point and my outlook is very positive. Thanks to everyone at Centre Point for a wonderful rehabilitative experience. Centre Point rules.

I stayed at Centre Pointe for about two months to rehabilitate after my last major surgery, minus a week or two when they had to send me back into the hospital due to internal bleeding caused by the unfortunate combination of me having to take blood thinners, yet also being at risk of developing a blood clot due to having a pacemaker. I was initially bed ridden, throwing up whatever I tried to eat, and was generally a hot mess. Truth be told, after two major surgeries, I was entertaining thoughts that I would probably end up as one of their assisted living residents for the remainder of my days. To their credit, they cleaned up my poop like I was a baby until I was able to get up, with help, and get to the bathroom when I needed to. Their physical therapists are top notch, and they have invested a lot of money into their on-site physical therapy gym. They started me out in a wheel-chair [which someone else else to push], to being able to propel myself in that chair, to being able to walk with a walker, then to being able to go for short walks within the facility with a cane. From there, they got me to where I could return back home, with visits from a nurse and my son, then regularly scheduled out patient visits. They have a van and their driver would pick me up, take me to the facility, then bring me back home after out patient appointments. It is now over a year later. I will , never again, be a healthy person. But it is quite possible that I may never have to live out the remainder of my days in an assisted living facility as my mother did and as my older sister will. I have been on the other side, as a younger man, stressing with the decision of how best to help a relative who is no longer able to live alone on their own. You are faced with the terrifying reality that many assisted living facilities are just simply horrible places, little more than holding cells for seniors until they finally draw their last breath. It is important that I convey to you that Centre Pointe is one of the truly SUPERIOR facilities, not like the many bad ones. Nothing will take the place of a resident's own motivation to get better. If you have decided that all you want to do is sit in your wheel chair until you die, no facility can help you. But, at Centre Pointe, if you are even remotely motivated to get better, they can and will take you to a better place. I am living proof of that. Also, if your loved one gets into one of those inevitable funks when they just don't want to get up and do anything ... it happens. Be patient with them, but also it is an important part of your job to, gently, nudge them into just trying to do a little more each day. That is how my son motivated me. He kept visiting me, didn't scold me when I was having a bad day. He supported me. Just being there, visiting your loved one, is really important. Otherwise, they start thinking that everyone else has moved on with their lives and forgotten about them. And that leads to the aforementioned funk when all they want to do is just sit in their wheel chair and wait to die.

My Mother was admitted to Centre Point after falling and breaking her hip. The staff from the beginning (Karen S.) was so informative and helpful getting her placed in the facility for rehabilitation. The nurses, doctors and physical therapists were excellent. They really care about the patients rehabilitation and she was able to return home within a few months. We were so pleased with the care provided.

My husband was at Centre Pointe for rehab a couple of times. They weren’t so great. Their nurses weren’t up to par, but the rehab people were good. After awhile, I was in a hospital and ended up in rehab, so I went to the same one and he and I were in that room. It was large enough for two people. Some things, like the bathroom, were not as good as they should have been, but it wasn’t that bad.

While a patient at Centre Pointe,I received excellent care. Demetria, my nurse, was extremely efficient and was at all times, on top of both my medical and daily needs. Jennifer Thornton and Risa Green, my therapists, made sure I walked out of the facility on my own two legs. Having rehabilitation be a part of your treatment facility is definitely a plus. Both of my therapists made sure I did not skip a single day no matter how many excuses I came up wit!!h. Moreover, the additional care and love of Ms. Barbara C. and Ms. Sharon L was just extra icing on the cake. Thank you Centre Pointe!!!

My uncle and a long time family friend were patients at this facility recently and received excellent care. Could not ask for a better therapy team! Both received all disciplines of therapy specific to their needs. If not for the great team at CP, neither would have been able to safely return to their homes. Thank you!

My wife has been at this facility a few times for therapy and now resides here long term. The therapist here are awesome to work with and seem to be very knowledgeable and equipped to improving the patient's needs while remaining compassionate with their abilities. Many of the nurses and CNAs have been a big help with this transition period for our loved one and we have come to know several of them on a first name basis. The staff and administration here have been extremely accommodating to our needs and expectations as well. I would not hesitate to recommend Centre Pointe to anyone in need of assistance with their loved one.

I was extremely impressed by the care and compassion that my mother-in-law received at Centre Pointe. She fell in her home and had to have surgery in Pensacola. We had her transported to Centre Pointe from Sacred Heart where she received excellent therapy and nursing care! I will always recommend this facility to anyone in need of rehab.

My best friend's mother is in this facility and she might as well be living alone. She was sick and threw up and the lady told her to clean it up herself. They left here laying in urine and did not bother to change the sheets. It is awful that we have to have places that pretend to care but hire incompetent personnel.

My mom has been at Centre Pointe Health & Rehab for about 60 days. I felt that the staff are excellent. The room she was in was a double room, and it was clean. Each person had enough space. She had enough privacy. It was good enough that she had company. Their hallways were wide enough. They would contact me constantly if anything has changed. They have constant meetings with their staff.

This is the worst facility I have ever been to some staff is very helpful. On the other hand I can't not stand the supervisor throughout the week days by the name of Lenora, her and another nurse on the 3-11 shift was very rude and disrespectful to a cna the supervisor was sitting at the desk on her phone during the time of a call light was going off the cna was busy helping another paitent and was constantly asking the supervisor would she answer the light the supervisor ignored her that's very sad because for them to have a gold seal and a 5 star facility that should know how to work as a TEAM they treat the cna's very badly the LPNS don't pass medicine on time patients steady be on the call lights about pain medicine I have been there a couple of times and watched the cna's tell the lpns that patients wanted medicine and they didn't even bother to go in the patients rooms I will be calling state in the morning

If you love your family member do not take them to this facility. They make your family member sit in filth for days on end with no bath and no sheet change. I had to go to the facility to change her sheets myself only to have the cna lie and tell her boss that she changed them. They literaly have to beg to get a bath maybe once a week they get it and when you ask them about when medications were given they lie about it and tell you it was given when it was not and then won't even provide you with a printout of what's been given since the patient was there but they promise that they will. The nurses are extremely rude and uncaring and shoot dirty looks at you when you have complained about the care. I would not let these people care for my dog much less my mother and I will be taking her out of there tomorrow and change over to outpatient therapy since the physical therapists were the only ones doing anything for her.

All they are interested in is the money and keeping someone in the facility. The level of care is terrible especially on weekends. The communication with the doctor and nursing staff is almost none, Decisions are made without consulting family. If you can avoid the place do it!

The first night my mother was at Center Pointe she was to receive her blood pressure medication and a lidocaine patch at 11pm. I asked the nurse 3 times before I was told that they only put patches on in the morning (even though I had a copy of the orders from the hospital instructing that she is to receive meds at that time) and the nurse wasn't even aware she was suppose to get any medication! The next night I had to ask them again to give her the blood pressure medication. Apparently they don't read the patients chart. She was also getting 3 injections of heparin a day in her stomach. The fourth night she was there she mentioned that she did not receive a shot at all that day. I called the nurse and asked her about it. She checked her chart and said she had one at 2:20 that afternoon, however, my sister had been with my mother from 1:00 to 4:00 and a nurse had not been in the room at all. Hmmm. It takes the nurses forever to respond when my mom presses her button so sometimes she's unable to make it to the bathroom. She was there 3 days before they gave her a shower. She's been there 10 days now and has only spoken to the doctor a total of 10 minutes. She's been on oxygen since leaving the hospital and they have not tried to wean her off or down. My husband asked for a copy of the records the hospital sent over and they told him they couldn't give it to him because it would be too confusing. Haha ok. My husband is a doctor and I can't wait for the Department of Health to investigate. I feel sorry for anyone there with no family to look after them.

This is the worse place ever. They ignore their patients, leaving them in the beds all day. My mother was in here and I got her moved. They treat their patients like they are burdens. She was suppose to be getting rehab 3 times a day, we were lucky to get 1 time a day. When she needed to get up to go to the bathroom and pushed her button for help, nobody would come for up to an hour. Nurses would walk by and just say, thats not my patient. The nurses were constantly arguing among themselves. If you care about your loved one - do not send them to this place!

The experience we had was nice. They’d take the residents down to the dining room and had meals for them. Centre Pointe was a nice place. The staff seemed very nice. My wife had been there for a while before she passed. They'd take my wife out and take her to the dining room. They took care of me when I was there, just like they did her. Sometimes I think the staff is overworked. Some of them had too many people to take care of. I ate there, and I thought they had good meals. The dining room was a little bit crowded, but it was nice. They had entertainment there, too. I think they organized the place pretty well. It’s modern, and they keep it up pretty good. There are lovely flowers and trees around outside, kind of like home.

My family really likes the fact that she is at Centre Pointe because she still can have some independence. They provide transportation to get to their doctors and to visit friends family and even go to the movies. My aunt has her on room, so when family visits we are able to have quality time with her. The nurses there are very helpful and they over and beyond for their patients. The one thing I didn't like was that there was a pinesole smell and normally that means you are trying to hide the smell rather than terminate it. My aunt is very happy there and it is 24/7 care from great nurses, they even have people come on there own time and visit with the residents.

Kindness and Comfort are the only words to expresshow I felt when I went to visit Center Pointe. The staff was very pleasing and not only that, the facility itself has a variety of ammenities to keep everyone happy at this nursing home. Rehabilitation is guaranteed. My client was pleased.
