Senior Care Consultants ease the burden by giving you advice and sharing our wealth of experience in handling a variety of unique situations and difficult questions. Our geriatric care managers can consult with you in your home to help evaluate your situation, listen to your goals and needs, and develop a specific plan of care for you or your loved one. Senior Care Managers also: Monitor the level of care given Coordinate household chores, meals, maintenance, bill paying and banking Assist with medication management Coordinate and/or accompany the client to doctors' appointments Assist in obtaining and coordinating home care, nursing home care, and care in retirement or extended care facilities Offer guidance to out-of-town family members and act as a liaison between the family, older adult, physician, attorney and others Support caregiving family Advocate and coordinate care while in hospital, nursing home, and/or rehab Advocate for clients' and families' needs Reassess and reevaluate your long-term health plan
