Welcome to CenterLight Healthcare Pace Center! CenterLight Healthcare, a member of CenterLight Health System, is a not-for-profit New York State Managed Long Term Care organization. Founded in 1985 to provide home healthcare services to the elderly, disabled and chronically ill in the New York metropolitan area, CenterLight Healthcare today is recognized as an innovator in the managed care field.
Operating with the mission of providing individualized care that enriches the lives of those we serve and helps them live safely in their homes, our Program of All-Inclusive Care for the Elderly (PACE) is one of the oldest and is the largest such program in the nation.
One of the distinguishing features of CenterLight Healthcare’s care model is our team-based approach. At our 12 PACE Centers and many alternative care settings serving the New York Metropolitan region, our team of healthcare professionals work side by side to provide knowledgeable, attentive and culturally sensitive care to our members. Indeed, we are fluent in both the languages and customs of our members.
Whether we’re providing care in a member’s home or in one of our day centers, our goal is always the same: to help our members enjoy the highest possible quality of life while living safely in their homes and communities.
