Helping people stay at home We specialize in caring for people who have complex chronic medical or cognitive conditions, and we take care of them in their home environment. Whether their care needs are simple or challenging, we pride ourselves on the ability to help families face many situations they will face as caregivers. Professionals at every level SeniorBridge's Care Management Team advises families about elder life medical, legal and financial issues that are important in helping people live safely and comfortably as needs change. Our team will help you evaluate how to manage finances, make a home safe or select alternate housing, access insurance benefits, and identify helpful community resources. Professional care managers are responsible for monitoring costs and advocating for access to health and social benefits. SeniorBridge Licensed Nurses and Social Workers provide professional oversight, planning, coordination and implementation of care plans. They make regular home visits, ensure doctor’s orders are followed, reconcile and monitor medications and home safety, support family decision making, and address mood and behavioral problems. SeniorBridge clinical professionals are on-call 24 hours 7 days a week. SeniorBridge Certified Home Health Aides (CHHAs), Certified Nursing Assistants (CNAs) Licensed Practical Nurses (LPNs) and Specialty Companions perform hourly and live-in support services such as: Companionship, meal preparation, light housekeeping, shopping and errands, personal care, bathing and hygiene assistance, transferring, and transportation. All SeniorBridge professionals are rigorously screened for criminal and credit histories. Each one is trained and carefully supervised. We assume all liability, workers compensation and related insurance expenses.
