I was a short term rehabilitation resident. I received excellent care during my stay. The Nurses were kind, caring, knowledgeable, competent, compassionate, and hard working. They would take the extra time to listen to any questions or concerns I had. They made sure I was well taken care of. The CNA's were Amazing! Friendly, kind, compassionate, always smiling and positive. They helped me without any complaints. They went the extra mile to make sure all my needs were met. The PhysicalvTherapy Team is Awesome! They made a treatment plan and made sure I understood all of the details of my rehabilitation plan. They were very knowledgeable, helpful, out going, kind, persistent and showed compassion. They made sure I did my therapy everyday. They have some creative ways to do therapy. Always willing to help in anyway. The dining staff made sure I knew what would be served. They went out of their way to prepare special requests for me. The social worker is wonderful. She made sure that everything was going well with my stay. She answered all my questions. She spent time talking with me She's wonderful! The Director is fantastic. He introduced himself to. me and was available I'd I needed him to be. He's very competent at his job. The office staff are Awesome! The housing staff made sure my room and my clothes were always I would highly recconrnend this facility.. A++++++ 5*****

Logan Nursing and Rehab Center has a rehab and long-term section. They were very helpful to my before she passed away. They had church on a speaker system, so she didn’t even have to go to the room, she could listen to services. She did sometimes go out to the dining room for food, and the food was OK. It's pretty cool that they feed you. They did a good job.

My girlfriends grandma was staying here while recovering from hip surgery. The facility catered to her needs very well. She is full navajo, and knows no english. The nurses there were very eager to learn phrases and words so that they could communicate with her. They also made arrangements for another nurse who was also navajo to be transferred from the hospital to help out with her as well. Overall I think this was the best place my girlfriends grandma could have been in the area that I live in.

Logan Nursing and Rehabilitation Center is an older facility but the only way you can tell is by the dated floors in the hallways and rooms and the ceiling lights. They are always painting and decorating rooms so that they are pleasing to the eye. They recently updated there front sign which looks great. This older facility has really tried to stay updated. The staff is wonderful many of the nurses have worked there for years and are still there because they love their job and taking care of the ones they love. The can's are a great help and are always willing to help if that means sitting down and talking for a minute with a patient or answering another call light with a smile. The main manager we remember being a friendly man that made us all feel at home. We were grateful for everyone's hospitality, kindness and patients.
