Staff are trained through ongoing online modules and tests to ensure dementia competency. Our Registered Nurses also provide in-services regarding dementia care and ongoing competency evaluations for our staff.
Describe the overall population of assisted living residents at Golden Horizons of Crosslake. What is their average age? What are their favorite things to do?
The average age for our residents is around 85, we have many residents in their middle 90's and some residents in their 60's, so there is a nice wide range or ages. Some of their favorite things to do are: Bingo, Morning Exercises each day, Happy Hour with live music and great food, Chair Travel, walks on our beautiful walkways surrounded by nature, gardening in our raised custom gardens, socializing with friends and family, puzzles, weekly cooking class, and crafts!
What programs or activities does Golden Horizons of Crosslake offer to enrich residents' well-being?
Our wellness program is called the Golden Wellness Program, a program designed to enrich our residents' lives with mind, body and spiritual exercises incorporated into our activities each day. We have a morning and afternoon exercise each day, church services, crafts ranging from painting to making decorations, cooking classes, food and beverage tastings, chair traveling to different countries, gardening out in our raised gardens, bingo with great prizes, trivia, and so much more! Our activities are tailored as well to meet or exceed expectations. If you enjoy more one on one time, our Activities Director accommodates that. If you enjoy socializing, we have plenty of options there as well.

Our Chefs and dining programs are unique because they are home-cooked and tailored to resident's preferences and suggestions. If a resident doesn't fancy the meal, they can ask for a substitute. Our Chefs put pride into creating meals that remind our residents of the home and of growing up. They also enjoy new recipes from residents they remember having as a child to share with the whole Golden Horizons family! We do not take celebrations lightly either, if it is a resident's birthday, their favorite cake is made, or for Holidays we create traditional feasts.

24/7 7 days a week trained care attendants with a LifeLine alert system that immediately sends to the caregivers working. Each Exit doorway is also connected to the same system so staff is aware of where residents are at all times.

We aim to continue to grow relationships with our families by frequent calls on changes in conditions and emergencies. We have an open-door policy to immediately communicate on changes or questions families may have. We have leadership on call 24/7 for any emergent needs that may come up outside of business hours. Care Conferences are scheduled with families and department heads to communicate how everything is going for that residents' care, socialization, eating habits, and well-being.

We accept cats and dogs and have specific policies regarding prices, sizes, and taking care of a loved furry friend. There are many beautiful outdoor spaces that pets can utilize.

We utilize our efficient Life Line system, Doorway alerts and locked doors in our Memory Care.

Our residents are included in activities stated in our AL information with our Assisted Living residents which include and aren't limited to: Happy Hour, Church services, Daily exercises, crafts, cooking classes, bingo and more! Our Activities Director and staff specialize individual programs and smaller group programs for our Memory Care residents to appropriately interact and provide proper stimulus for dementia care. Activities ranging from one-on-one trivia or conversations, bowling, balloon toss, dancing, folding cards and bags, gardening and more. These activities stimulate their long-term memory and keep them active-mentally and physically.
Who assesses residents' health and cognitive functioning? How often is that assessment repeated?
Our Registered Nurses! Assessments are done after 14 days, 30 days and 90 days of admission. When there is any change in condition, they are immediately assessed as well. Residents are constantly assessed by staff and they directly report changes to our Registered Nurse to assess further. Each year on their anniversary date, they are also assessed.

24/7 staff supervision, reminders, prompting and assisting with meals, and encouraging eating and drinking frequently to our residents to ensure proper hydration and nutrition.
