Staff receives ongoing training from Dementia Care Matters. All new staff who join the team receive 8 full-day modules of “Being a Butterfly” training. Dementia Care Matters continues to be a resource for the team and are available for ongoing consultations.

Cobblestone at Park Springs has 56 suites and studios divided into 3 care levels. Member’s average age is 83. Our members enjoy weekly outings, art classes, exercise classes, game days, cocktail parties, current event discussion groups, outside time in the courtyard and walking around the lake, among others.
What programs or activities does Park Springs - A Life Plan Community offer to enrich residents' well-being?
Our residents are supported by caregivers, homemakers, dietician, and nurses. We also have a geriatric MD and Nurse Practitioner on staff. Activities are designed to suit each Member’s lifestyle, likes, abilities, and natural rhythms. We have art and social programs, a fitness center with an onsite fitness instructor and rehabilitation area, walking trails, a variety of outdoor areas and porches.

Cobblestone is on the campus of Park Springs, a 61-acre Lifeplan (formerly continuing care retirement) Community. There is a small store on site. There is also Pebblebrook that has short and long-term rehabilitation as well as Memory Care. Park Springs is surrounded by Stone Mountain Park that offers numerous amenities for Members and their families.

We have multiple busses and sedans for scheduled and personal transportation.
What makes the chef or dining program at Park Springs - A Life Plan Community exceptional? What kind of menu can assisted living residents expect?
Our Homemakers (Chefs) are professional culinary specialists and understand the personal tastes and needs of each member. Each homemaker understands the importance of nutritional value, preparation, service, and measurable results. Members are encouraged to participate in the planning and cooking of meals if they would like.
What safety features or security measures does Park Springs - A Life Plan Community offer to assisted living residents?
We are in a secured building in a gated community with 24-hour security. Additionally, there are nurse call lights in bathrooms and bedrooms. All suites and studios are one level, 36” doorways, zero-entry showers with safety bars.
How does Park Springs - A Life Plan Community keep residents' families informed about their loved ones?
Communication is provided through public postings of announcements and activities, as well as mail, text, and phone calls. Once a month, representatives from each department that oversee daily operations for Cobblestone attend a member council for a round table discussion of goal setting and performance feedback. At the end of the month, the care staff contact family members to give a wellness update on their loved ones along with progress reports towards individually set care plan goals.

We have an on-staff trainer and Assisted Living Guide (director) who maintain on-going staff in-service, training, and supervision. The trainer also listens to feedback and can make changes as needed.

Our residents reside in homes, cottages, and villas. We have a population of 460 with an average age of 78. Our Member activities are planned with input from the members. With over 65 special interest groups our members enjoy (but not limited to) gardening, art, sculpture, working in the community store, carpentry, physical fitness: equipment room, water volleyball, water yoga, water aerobics, and planned local outings and planned travel club outings.

Places nearby include local restaurants, shopping, community theatre, community symphony, volunteer opportunities, golf, trail walking. We are adjacent to Stone Mountain State Park that features a variety of seasonal and family themed activities.

Travel Club to Cuba, Europe, Panama Canal, Nova Scotia as well as continental US trips. Members attend theatre productions, symphonies, art exhibits, a monthly dinner club, and many more. The leisure calendar is designed by the members. Their recommendations drive the monthly schedule

Members enjoy 5 dining venues, complimentary carry out, and complimentary in-home delivery. The chefs are exceptionally trained in culinary arts for active adults. Menus vary from Mediterranean cuisine, Bistros, American Classics, and Pub fair.

Common areas include a 50,000 square foot clubhouse featuring (but not limited to) fitness center (15,000 sf), art studio, carpentry shop, greenhouse, walking trails, library, card rooms, community hall, private dining room, beauty shop, member-directed convenience store, and on-site banking.
What safety features or security measures does Park Springs - A Life Plan Community offer to independent living residents?
Every home has pull cords in bathrooms, daily check in service, touch dial to security from kitchen telephone, and 24/7 security (fully fenced and gated).
If Park Springs - A Life Plan Community accepts dogs or cats, what indoor/outdoor areas can they enjoy?
Dogs and Cats are welcome and ultimately the responsibility of the individual Member. We are a very animal-friendly community. Dogs are welcomed to enjoy outside walking and in the clubhouse while on a leash.
Does Park Springs - A Life Plan Community work with an agency or registry to provide nursing and medical assistance as needed?
We have an onsite health services which offers in-home care, assisted living, memory care, skilled nursing, rehab services, adult day care, and respite care. Also, a board-certified geriatrician and nurse practitioner are on site 5 days/week.
How many staffers are on duty in the daytime and overnight?
There are 3 Care Partners and one Homemaker available in each household during the day and evening times. There are 2 Care Partners available in each household from 11pm to 7am. There is a Licensed Practical Nurse available from 7am to 11pm daily and RN coverage inhouse 24 hours per day.

Our secured Memory Care households located at Pebblebrook are secured from the outside but offer an interior courtyard for easy access to the outside and an exit door that is key card entry accessible
What programs or activities does Park Springs - A Life Plan Community offer to keep residents engaged? How does the staff engage residents' long-term memories?
Having the luxury of the Homemaker in each household, who plans and prepares meals to be served when the member would like to eat “takes the schedule out of the day”. This allows Care Partners to maintain a more relaxed, home-like environment. Care Partners provide ongoing engagement opportunities for members throughout the day based on what the members truly want to do and are interested in doing. No two days are the same. The Care Partners are trained to identify the individual needs of each member and attempt to provide the right level of engagement to meet those needs. Butterflies change moments throughout the day. Engagement means different things to different people. Care Partners develop meaningful, supportive relationships with members and can tailor engagement to meet those individual needs.
Who assesses residents' health and cognitive functioning? How often is that assessment repeated?
The Memory Care Guide in collaboration with the nurse completes a quarterly assessment of the member’s overall wellbeing and cognitive status. Health and wellbeing are monitored on a daily basis by the Care Partners and they report any significant changes directly to the nurse on duty for follow up and treatment.
What techniques does Park Springs - A Life Plan Community use to support memory care residents during mealtimes?
Mealtimes are a very important social time of the day. This is a time when the family shares events of the day, spends valuable time together, laughs, and enjoys sharing good food together. Our Care Partners share meals with our members. They are encouraged to sit and share, further enhancing engagement and a sense of family.

The Memory Care Guide establishes a close relationship with family members and keeps them updated on the member’s current condition, any changes in condition and is available to follow up on any concerns the family might have.
