About Sentara PACE – Norfolk in Norfolk, Virginia
Sentara PACE is a health plan for seniors that provides a range of comprehensive health care and supportive services for seniors who wish to remain in their homes and community. Sentara PACE is an alternative to nursing home placement for those who qualify. Qualifications include being nursing home eligible, 55 years or older, able to live safely in the community with PACE support, and living within the service area which includes, Franklin City, Chesapeake, Isle of Wight, Norfolk, Portsmouth, Suffolk, Surry, and Virginia Beach. Our Sentara PACE team partners with families to provide a 24/7 plan of care for participants. Sentara PACE requires state approval for eligibility and serves those with Medicaid and Medicare. Eligibility is based on geographic residence, age (min. 55), ability to safely live in the community and eligibility for nursing facility level of care. https://www.sentara.com/PACE
