When someone enrolls into LifeCircles PACE program they agree to use our network of providers. If someone sees a provider that is not part of the LifeCircles network, or sees a provider that hasn’t been authorized in advance, the individual may be fully liable for payment to that provider. Emergency care does not need a prior authorization. Our Enrollment Coordinators can help you or a loved one find out of if they are eligible for Long Term Care Medicaid. They can walk beside you as you complete the application process. Seniors may be eligible for Long Term Care Medicaid Coverage if their monthly income is less than $2383. Contact us to learn more!
