Visionary Home Health Care is a locally owned Home Health Care agency. We offer our home healthcare services in the following counties: Collin, Cooke, Grayson, Fannin, Denton, and Hunt. Our Services Include: Physical Therapy Physical therapy assists patients in regaining physical motion and strength. Many therapies can be provided at home to improve recovery from illness, surgery, or injury. Our licensed physical therapists will evaluate the need for services and create a plan of care that allows clients to develop independence with daily activities. Medical Social Worker: Our social work team conducts assessments of social and emotional factors related to the patients injury or illness, provides counseling based on the assessment, and assists with community resources. They are an invaluable resource in helping the patient and family cope with the stresses of an injury, illness or major life change, and offer guidance for how to cope with a variety of psychosocial issues. This may involve assisting with practical tasks such as arranging meals and transportation after a hospital stay. Medical social workers courageously step into these situations and mobilize all available resources to help patients recover and/or live the remainder of their lives with the greatest possible degree of dignity and independence. Occupational Therapy: Home occupational therapy is a rehabilitative service for people with mental, physical, developmental, and/or emotional impairments. It helps restore or maintain the patient’s ability to perform tasks used in daily living, often through developing ways to modify or adapt activities. After hospitalization or a recent surgery, our occupational therapists can help you regain your self-care skills with rehabilitative treatment in your home. Our therapists act as a guide, teaching patients how to regain their strength and independence through exercise programs, personal care and daily activity task modification, including energy conservation techniques. Skilled Nursing: Our licensed nurses will perform a comprehensive health assessment and work directly with your physician and our interdisciplinary team to develop a plan of care that ensures that all needs are addressed. They provide treatment and education to patients and their caregivers on the disease or injury process, treatment plan, medications, diet and wound care. They also consult with your physician on a regular basis, and provide updates on your progress. Hospital Discharge and Transfers: After surgery or hospitalization, home health care may be needed on a temporary basis. We regularly work with hospitals, assisted living, long-term care facilities, rehab centers, and private care providers to coordinate safe and seamless transitions for patients when they go from one location to another. Our qualified team of admission nurses will collaborate with all providers involved to manage multiple complex medical conditions and ensure a smooth transition for the patient and the family. Medication Management: Medications prescribed by your physician, when taken correctly, support the healing process. Knowledge and education about medications is key to maximizing their safety and effectiveness. Understanding side effects, allergy sensitivities and contraindications are part of our medication management program. Speech Therapy: Speech therapists work with patients to improve speech, memory, and swallowing disorders, as well as assist staff and family members in communicating effectively with the patient.
