I was home alone, casted with a broken ankle, and medically unstable. Centura came ONE time for the intake and never returned. They had orders for a nurse, HHA, PT, OT, CNA, and LCSW but no one came for 8 days. I phoned 2x a day, but was given a rude answer by the supposed case managers. Do not request this place. There is no follow through and they leave patients with no care.
