"My mother-in-law is at Bay Harbor Memory Care & Assisted Living. I know she gets good care there, but to me for being there, it could be a little bit cleaner. It's not a place where I would have my mother, but my mother-in-law is there. I'm not the one that put her there; my sister-in-law and husband did. It was one they could find at the time that would take her. My mother-in-law got one of the bigger rooms. It's not too bad. For her, it's okay. I have no idea if they have activities or not because all I see is the staff that's there. The dining area is not as nice. It's an older place, too. In terms of value for money, my mother-in-law pays more than my mother does, so I would say it's kind of overrated. To me, it's pretty high there."

"The room are absolutely beautiful! The staff is wonderful! This place took my breath away. Excellence from the minute you walk in the door. The staff interacts with the residents everyday with fun things to do. The food looks delicious and healthy. Clean clean clean! They have an arboretum with flowers and a raised bed garden that residents can work with the plants and even eat the food!"

"My father fits in very well at Prairie Gardens. He is in an apartment, and it's very nice and very clean. It's roomy enough. It has a kitchenette, a living area, and then a bedroom. The facility seems to be very clean, and I know the cleaning staff comes through a couple of times a day. The staff are all very good. I have been there while he is eating, and the dining room is very nice and very clean. The food has been very good. "

"We visited Sylvan Crossings at Hunter Ridge. I like the size of the rooms. The staff people seemed friendly, but I thought the place looked a little bit rundown. The beauty shop was very important for my mom. She was happy about that, and since she's never been in a communal living situation, she doesn't know what to expect. They showed us the menus, the activities, and the library. They were very accommodating. They gave us the sheet for the month of what was happening; it was bingo was one that stood out to me. The staff was so friendly, and I think my mother would get really good individual care there. I like the idea that she can get her nails done there, and the private room was nice. They also have a little church thing, it's nice on Sundays. The outside was gorgeous and the common areas were, but you could tell it was an older building."

"The assisted living was sort of new, well-kept, and pretty clean. There are low incidences of COVID, the staff seemed warm and helpful, and the residents seemed friendly and nice. It seemed like a nice facility. The apartments we saw were decent. They have 1-bedrooms and the efficiency apartments. They were unfurnished so you have to provide your own furnishings. The efficiency ones didn't have their own shower in some of them, so you had to go down the hall to the shower, but that was okay. They had a library, a TV room, and a piano room. They also have a cafeteria type of facility."

"We choose Sylvan Crossing at Westshire for my brother because it is a small community and a fairly new facility, but we have not move him in yet. The inside is gorgeous. They have 32 private rooms and they are absolutely beautiful, spotless and very generous space wise. The dining room, the gathering area, the rooms that are open and available and all of the different rooms, I am amazed on how big they are. They are also in very nice size, well light and they are all in outset and facing windows. We didn't eat there, but I did see on the board what they are having each meal and it looks nice and well balanced meal. They tell me that they fix everything from scratch.The only thing that I notice is that they have a kind of a porch on the outside that has spindles and the spindles can be re-painted, but other than, they have a lovely parking lot and it is close to the water. The one who assisted us is the assisted manager and her name is Carmen. She is very helpful. She isn't expecting us, so we come in there as a surprise, but she shows us the whole facility. They have a list of activities posted, very morning after breakfast they have exercises and after that, people have a choice of what they can do or if they don't want to do any of them, they can go visit in the lounge area, go to their rooms or go outside for a walk around the facility. They have a walking path around the entire facility. It is very pleasant, homey and very nice. The location is wonderful. It is like in a cove and there is a lake there. The staff is very happy. They are doing their job and they are smiling. I will recommend them. "

"Columbia Health Care Center is a nursing home, rehab, and long-term care facility. It's very clean and renovated. The staff was wonderful, nice, and devoted. "

"My overall impression of At Home Again is awesome. It would be a place I would strongly consider for my parents. What I like about it is that the appointment was set up and followed up with me to ensure that I was going to be there. The owner of the place took me on the tour and was very prepared. It was very organized, which is always a good first impression. They make the food there and bring in musical talent and organize people. It's a great idea that people are able to give feedback on things that are lacking. They also try to cater to the food requirements for the different people and have nursing staff 24 hours a day. They said they could accommodate both my parents, but they don't have any large rooms they could share. People seemed to be engaged and doing stuff. The staff seemed friendly and said hi, and the owner was wonderful."

"I chose Hamilton Park Place for my dad, and I think everything is great. His apartment is very nice, and he is very happy there. The staff members are all great, and they keep him busy. My dad said that the food is too good. Some of the activities that he enjoys there are the parties, trivia, yahtzee, bingo, and crafts."

"The facility was very clean and presentable. They had a lot of great things which the people living there could do such as a garden outside with fresh vegetables, as well as a green house which was accessible from inside the building. Something which is very important in a place that gets as cold as Wisconsin does in the winter. They had events going on in the evenings and seemed to want to keep the seniors entertained and engaged at the facility. My grandfather began on the side of the facility that was assisted living for the first year he was there. He moved into the full time nursing area for the last few months after he fell on the other side. His health declined rapidly when he was on the nursing side. It seemed that the nurses were stressed on that side. On the second to last day we were visiting we noticed that he had developed quite a diaper rash and his skin was starting to break down from being left in soiled diapers for so long. A few months after this visit he died from a respiratory infection. "
