I am a friend or relative of a current/past resident
My father was a resident here. Right before Christmas in 2017, I went to transfer my father to a closer facility so family could visit him more often. When I got there, one of the care takers told me he was laying down because he had a bad cold. I found him laying with his head at the foot board of the bed. He was fully clothed, and there was blood, urine, and feces all over the side of his bed, as well as his clothes. He had a fever of over 103. I took him to the hospital in Albany where he was diagnosed with MRSA pneumonia, and a staph infection throughout his entire body. He stayed in ICU for about a week. My family had decided to move him to hospice. The day after they took him off of oxygen in the ICU and moved him to a regular room to be transferred, he died. On the day that I had taken him to the hospital, when I returned to Bridgecreek to gather his belongings, one of the attendants asked me how my dad was doing. Her statement to me was that they thought my dad had pneumonia. They never called the family, they never called the doctor. They let my father lay there and suffer, doing absolutely nothing. This company, as well as the caretakers, are a joke and should not be in operation at all.

I think all the reviews I read and their website is not an accurate representation of Bridgecreek.
Anything and everything of value in Mothers room was taken. Other residents have shared these concerns with me. Staff has no response.
She was a smoker and when they took her out to smoke they did not remove her oxygen, the lady was on her cell phone, and mom smoked on oxygen and there was a fire on her face. She was in the hospital for a week or so. They were fined. It could have been much worse.
Seven months later they tried to move her to their sister facility, Southtowne, in another city. They did not inform us. I visited on a Friday and was told by the receptionist that she was moving to Southtowne on Tuesday. I spoke with administrator and said no. I had toured that place. Several people had died because of scabies and neglect. People went to jail. I did not want her to go there, they did it anyway over our objections. The administrator said Mom was her own person and could decide to go, despite her dementia and Alzheimer's disease, according to their own records. She was later moved back to Bridgecreek as Mom did not like it there. The first night someone came in her room and turned off her oxygen.
We have various stories of neglect.
Staff lacked knowledge in ability to operate and put in use her CPAP machine, it was stored under her bed, unplugged.
Staff did not know how to connect oxygen tanks or have the ability to discern if they were empty.
When Mom was dying they were so inept Hospice had to remove her because she was in such distress, according to the hospice nurse. They never came and gave her the medications unless we called and then it would take around 20 minutes to come.
The bathroom smelled terrible soiled depends were thrown on the floor and feces on the toilet. I complained they moved a waste basket next to the toilet.
If you love your family member...go somewhere else.

I'd like to thank Andrew and Casey for all their help getting my Mom in a room so fast. My Mom likes it and always has good things to say about the Staff, they are very helpful and nice to her. I would have liked her to have a private room but that is not an option with Medicaid. My Mom doesn't seem to mind much, she likes her roommate. She does need to share a jack-n-Jill bathroom with 3 others, not ideal. I feel they should have a better space for her tooth brush, towels and bath supplies.
I found you really need to ask questions how things are done, like Med list, showers, teeth brushing and laundry, so to better understand how your loved one is getting care. It might not be as good as home, but the next best thing.

I'd like to take this time to thank all of the staff in Lebanon for the great care and keeping me well informed with my moms condition. The new addition to the facility is great! Very concerned staff, very open rooms and entry way. Just love the care they give my mom! Thank all of you! And Doris loves all of them!

I visited this facility
Bridgecreek is a nice facility. They just recently remodeled. They have three dining rooms where they can segregate people. The people like my wife, who is a little bit higher functioning, are in one dining room, and then a next dining room which is kind of middle, and the third is for people who are really getting toward the end of memory care. So instead of one large dining room, they got three small rooms. There are a few residents that are high-functioning that actually get together and do a little card playing, or play Solitaire. My wife’s only been there for a week. My interaction with the staff is absolutely 100% positive. They’re very caring and just right on top of things, and they keep me informed.
