I am a resident of Avalon Care Center - Bountiful. They are doing everything possible to help me out. I go to therapy twice a week and it's excellent. The staff was very caring and very thoughtful. I've got a room with a TV, a hospital bed, and my own bath. The staff is very cognizant of your situation, and each person's situation is different. They tried hard to make sure you get the attention that you need.

We were told by the discharging hospital that Medicare considers this facility to be a 5-star facility. It absolutely is not. In the five days my MIL has been here, she has received incredibly subpar care, if any care at all. The first time we had any discussion with administration about her care plan was when we requested to discharge my MIL into another facility’s care. Of which they’re fighting. My MIL has not been showered in the five days she’s been here, nor has she had her teeth or hair brushed. We had her transitioned to this facility for catheter, toileting, and physical therapy care as in interim between hospital and return to assisted living (at another facility). Five days in there is still no PT or care plan. Yesterday we arrived to find my MIL half naked with the door wide open, stuck in a wheelchair having a delirium episode. The night nurse (through another agency) told us that the previous aid couldn’t tell her if my MIL had been changed or toileted, or the catheter clean. The day before another night nurse told us the day nurse had not done either. The food is inedible, if the staff even ensures that the patient can eat or reach their meal.

Avalon Care Center was the worst of the places we'd seen. It was a very sterile, very clinical room, the doors were all open, you could smell all the smells, you could tell that the people or residents required more medical care. It's like going to the hospital. The staff seemed good. It is what it is.

This list a joke. I seen what my friend has gone through with her loved one. The staff are rude and unprofessional especially the lady up front. She is the most unhappy person there. The staff are not nice at all. It's all about the money. My friend's mom can't even call her own daughter and they refuse to help her get a phone to call her loved ones. I'm glad she's looking for another facility. Place settings like pee and the food is sloppy and cold.

My mother stayed at Avalon Care Center Bountiful for one month and it was horrible. It was dirty, the food was crabby, and the staff was not helpful. There was nothing good about that place. The room was big, but it seemed unsanitary.

There’s a reason that we took my mother out of Avalon. It was very expensive, the food was horrible, and I didn’t like the environment. The people were very nice, but the room is like in a hospital.
