My Dad was taken here for hospice care. This facility is so run down I don’t know how they stay open. The public bathroom door would not close. The floors were dirty. The door you walked into the facility had bird droppings all over the place due to a bird nest sitting above the door. My Dads personal blanket disappeared. My Dad could not feed himself, they would just drop a tray of food off. He never had water to drink. I had to ask them to give him a sponge bath. He passed away after 4 days. To this day I regret my Dad had to go tothis facility but Sanford Hospital said there was no other place available. It just breaks my heart my Dads last days were spent in this rundown facility. 😢

DON'T PUT YOUR LOVED ON HERE!!!! My husband was place here by the VA. He was in a room no bigger than a closet. The BR was so small that the CA's had little room to assist. His hearing aid was run through the laundry. On 3 occasions he wasn't given his meals. I on every night had to take his tray to the kitchen to heat his food. The tray came cold. He had Parkinson Disease and was having difficulty swallowing. The served him a slice of pizza and a bad of Corn Chips. If I weren't there to put him to bed. My daughter found find him in bed in the am in his clothes on top of the spread. On the day he passed away my daughter found him not responding almost falling out of his chair. It took them forever to answer his light and we witnessed them turning it off and walking out. Many of the aids were foreign speaking, it was hard to communicate. Dr. ordered meds were often not ordered. I could go on and on. Short staff. Arguments among aids and one or two would leave. Making the staffing even worse. Head Nurse and Administrator on extended Holiday at the same time.

the rooms are small and the bathrooms even smaller. Staff is pleasant but very overwhelmed and short staffed more so than other nursing facilities. 1 nurse to 50 patients and 1 to 2 CNA's. Only one staff person in their locked memory care unable to keep watch on everyone while in a residents room doing their cares. Activities are your run of the mill games and Bingo nothing really special. No real amenities either.

I first want to say what a joy it is having Mom staying somewhere people actually care. I'm very pleased with the staff and glad it's so close to home. I know there are a lot of options in Sioux Falls... just remember when choosing a facility find one that has people with caring hearts and Southridge has that.

This is the second relative I have had here at this facility. Last year, my Aunt was here and it was because of my recommendation to the family. I knew the Director at the time, he was a younger guy with a lot of energy and the staff really liked him. Everything was run smoothly. Now I have my Mom at Southridge and I am looking for a new place. Completely unorganized and the young director is gone. I hope they can change it around for the safety of the patients. I am not sure who owns the nursing home, but I hope someone like good sam buys it and flips it around.

DO NOT USE THIS FACILITY!! Understaffed, incompetent, negligent,and the staff are not friendly. Social Workers are no help and worthless. The care is poor and residents are not attended to in a timely manner. It took an 1hr 1/2 for somebody to respond for water request, the use of the bathroom, help to get up, and the list goes on. Unsanitary conditions, personal items missing and many more discrepancies. This review of the facility is not the opinion of one, but from several friends and family. There are better choices.

I visit this nursing home a lot and there seems to never be enough staff to help the residents. The cna's are friendly but overwhelmed. Sometimes the call light can be on for close to an hour before help arrives. There is a lot of turn over with the aids

My mother was a resident of this facility for approx 4 yrs and has not been ambulatory for about 10 yrs. Just before Christmas this year (2014) we received a call that she had fallen during a shower and was sent to the hospital. Hospital MD noted several rib fractures, fractured clavicle, & black eye as well as 2 broken vertebrae that were partially healed (prior fall injury). She died as a result of complications from these injuries about one week later. This was by no means the first time that we received a call that she had fallen. It seems their standards for basic patient safety are very low. I would rate them zero stars if I could.

I visited this facility several times in the recent months, the nurses were very polite and professional although noticeably overwhelmed. Not one time in my several visits was this place properly staffed. I do not know the regulations for this sort of thing but I can't imagine that one nurse and two cnas is a decent about for almost one hundred residents. I also can't imagine being that short staffed would allow the time for the staff to do their jobs properly.
