"Sylvan Crossings of Chapel Valley was the first place that had an opening for my friend. I have not been in it because of COVID, but it looked really clean. They've had trouble keeping people and a cook came down with COVID, but they were friendly and they treated him pretty good. It's nice outside and it's only 1-story, so it's comfortable and it has a lot of windows. The staff that I've talked to was very helpful and nice. The cook has been there for many years and he's really good, and he'll be coming back in a couple more days, so a couple of the women chip in and cook for 20 people. Sometimes, he likes the food and sometimes he doesn't. This place is comfortable, well-kept and has good parking."

"I toured Oak Park Place Nakoma. Sue, the director, was the most kind, caring, and generous with the time of any of the people. She was the most proactive in terms of follow-up and not pressuring you at all but just really being in it for the people. It's apparent that she cares about all the people involved and that they truly care about the residents and their families. It was a very good comprehensive tour. In terms of the condition of the place, I would say it was really nice and well-maintained. It's like a newer facility and it's nice looking. They have some nice communal areas. Currently, the average age of their residents is older than my father-in-law who has early-onset dementia. A lot of the patients there were in a little further and later stages of dementia, so that was a negative for us. Part of why we're looking at centers is so that my father-in-law can have socialization, but if you have a lot of bedridden or wheelchair-bound patients that aren't as social with the activities, that makes it harder. Their memory care unit isn't on the first floor, it's on the second floor, so their only outdoor space is a concrete balcony versus other places we toured that had memory care that had an outdoor patio with some green space. Since nature is a huge love of my father-in-law, that's a big thing for us. "

"My tour of Brookdale Madison West was really good. Heidi, who gave us the tour, was very nice and very knowledgeable and took the time to answer all our questions. The individual rooms were nice. We toured two of their different sizes, and we've toured a few other facilities and this one was just as nice as some of the other ones we toured as well. So that's good. The layout was good. It's a little different. They had good common areas and then two different hallways of the rooms. Overall, everything was clean and well-maintained. It wasn't the newest by any means, but overall, it was nice. You can tell that Heidi is passionate. She has been with Brookdale for over five years, and she's very embedded in the community. Like she leads Alzheimer's and dementia support group close by, that she told us about and invited us to. Not only does she work in the field, but she gives her personal time in the same field, and to me, that speaks volumes. We also got to meet the nurse whose office is in the memory care unit, and she seemed very nice and friendly. We saw support care staff people working with the patients, and they seemed to be engaged and on top of it. So that's good. I like that the memory care unit has a separate entrance from the overall assisted care. I like that it's covered and that it has a little bit of parking there, so if you want to take your resident out or come to visit, it's very easy to do so. But in one of the main rooms that we toured in memory care and Heidi acknowledged it, too, she's like, 'Somebody must have had an accident in here, and we haven't cleaned it yet' because it smelled of urine. So that would be like my one thing because there were some facilities that we toured that didn't have that or would have maybe already had it cleaned up. "

They ranged from large apartments to small efficiency. I saw a dining area, and everything seemed nice. I saw their activity schedule, which looked good too."

"Mom moved into Prairie Gardens. When we first took her there to tour the place, she immediately liked it. She liked the atmosphere, which was cozy and friendly. The food is wonderful. The plates and the food are always warm. The staff is very attentive, and they keep the call time to five minutes, ten minutes at the most. They come in and they ask if there's anything else that Mom needs help with. Anything we needed was gotten there right away. We are really happy and she's happy. Mom's room has a path for her wheelchair. It has a large bathroom that she could get into, and everything is arranged nicely. The facility is an older building, but is well kept. Within the one week that she's been there, they had a Halloween craft, bingo, and movies. They have a beauty shop on Thursdays."

"We choose Sylvan Crossing at Westshire for my brother because it is a small community and a fairly new facility, but we have not move him in yet. The inside is gorgeous. They have 32 private rooms and they are absolutely beautiful, spotless and very generous space wise. The dining room, the gathering area, the rooms that are open and available and all of the different rooms, I am amazed on how big they are. They are also in very nice size, well light and they are all in outset and facing windows. We didn't eat there, but I did see on the board what they are having each meal and it looks nice and well balanced meal. They tell me that they fix everything from scratch.The only thing that I notice is that they have a kind of a porch on the outside that has spindles and the spindles can be re-painted, but other than, they have a lovely parking lot and it is close to the water. The one who assisted us is the assisted manager and her name is Carmen. She is very helpful. She isn't expecting us, so we come in there as a surprise, but she shows us the whole facility. They have a list of activities posted, very morning after breakfast they have exercises and after that, people have a choice of what they can do or if they don't want to do any of them, they can go visit in the lounge area, go to their rooms or go outside for a walk around the facility. They have a walking path around the entire facility. It is very pleasant, homey and very nice. The location is wonderful. It is like in a cove and there is a lake there. The staff is very happy. They are doing their job and they are smiling. I will recommend them. "

"My father has been happy since 2017 at this facility. The facility has increased their care for him as his needs have grown. This is a relief rather than needing to him to move him to another facility. Importantly to me, the facilities' core staff have become like family for me in navigating the waters of an aging parent. They are an advocate for my father in dealing with his healthcare. This "home away from home" gave me a chance to know my father much better than I could have if he had stayed at his home. Plus, he gets the care he needs and I get the support to focus on loving him rather than worrying about him."

"The facility seemed adequate. I was not overly impressed with the common or private areas for those living there, but it seemed to provide them with everything necessary. "It was a nice facility. They have everything that we need. The only thing is their rooms are just bigger versions of a hospital room. It's just one area and it was small. It wasn't what we were looking for. If my aunt is going to live her life out there, I would prefer she would have a separate bedroom area and sitting area. For some people, that's not necessary though, so it was still a nice facility and they have a lot to offer. A lot of the residents were sitting out front when we got there. It's got a nice patio on the front. The lady showed us everything that they had and she talked about the facility."
