Pathways Hospice is a community based, nonprofit organization serving individuals and families in the San Francisco, East Bay, and South Bay-Peninsula Counties for over 30 years. Care is provided by a team of professionals who are experts in pain and symptom management. RNs are available to their patients and love ones 24 hours a day and will make personal visits after office hours when needed. What makes Pathways Hospice different from other hospice agencies??? Pathway Hospice care is directed by the patient's own doctor, and completely guided by the patient and family wishes. Also, each patient have the benefit of the agency team of medical directors - physicians with specialties such as geriatrics, oncology, hematology, family medicine, and long-term care medicine. In addition, Pathways has a nationally recognized pharmacist to ensure your medications are optimally managed. Other unique innovative services include; massage therapy, pet therapy, aroma therapy, guided imagery, and therapeutic touch. As a former Director of Social Services at a Sub-acute inpatient Skilled Nursing Facility (SNF) I referred patients and love ones to Pathways fro hospice services. Furthermore, many of my colleagues recommend and utilize Pathways for their patients.
