I am a friend or relative of a current/past resident

My spouse was at the facility in July 2022 for rehab. and as a social worker I chose this facility because of knowing of its excellence. This only proved to be true while my husband was there. The Doctor and staff were outstanding. The care and attention he received and interaction with me was superb. I continue to refer others to the facility. Although the facility may be older what is important is the excellent care it provides.

My brother was in Holt Senior Care & Rehab Center for several weeks, it's wonderful. The staff was outstanding and the care was excellent. The food was okay, but my brother is a very picky eater but they helped him. It's clean and it's attractive, it has good halls, and they encourage people to get out of their rooms and move around. They offer quite a few activities, they have movie time and they have bingo and some other things like that. They offer two activities every day, one in the morning and one in the afternoon, they had entertainment, they had speakers and they had games. The health insurance paid for it.

My mother is in a nursing home at Holt Senior Care & Rehab Center. The nursing staff members have been excellent to my mother there. They treat her like family, and it's always very clean. When I go in there, they're always very prompt with letting me see her. My mother seems happy. I know that she was part of a singing group, and like a games group where they do games like cards and different games. Then there was another group that was like a music group. She's in a wheelchair, doesn't do dancing. But they listen to different music, so it was like music therapy. The food seems like just healthy food. She never really liked to eat, so the fact that she has gained weight while she was there is a really good sign that she's eating well. It's definitely worth the money.

I am a friend or relative of a current/past resident

My mother is currently a resident on Oak Hall and I am familiar with other in-patient rehab facilities. This is the best I have ever seen! The rooms are very clean and provide ample space to move wheelchairs and walkers around. They are bright and cheerful. Staff are just absolutely incredible! They are without a doubt smiling and helpful, each and everyone, all the time. They go above and beyond to make sure my mother is well taken care of. There are many different areas in the facility which my mother can wheel to for diversion and activities. I think the negative comments I've read are from individuals who do not truly understand the definition of rehab. My mother is there to regain her strength so she can go back to her apartment. She is not there to be coddled. These people constantly encourage her and gently push her to do her very best. My sister calls them "miracle workers" and I agree. Highly recommend this place for those serious about rehabbing.

Terrible facility. While some of the aid staff is pleasant and a pleasure to interact with, others leave a lot to desire and clearly do not want to be there or do their jobs. Small, dark, cramped rooms. The food was barely edible and we had several issues with my relative not eating because of the food being so bad. The person who does the insurance billing and such, Bob I believe his name is, is very standoffish, and treats patients families as though they are stupid and have zero knowledge of medical billing/insurance procedures. It was like pulling teeth to get a copy of the patient billing from admittance to release, so we could go over the billing with a fine tooth comb and make sure we were billed correctly. Apparently, he does not know how to print a patient statement record, showing all charges, writeoffs, etc, it has to come from corporate, yet he can bill the patients account??? Just now receiving a bill for over $1,000, 3 months after the patient was released, with some lame excuse about waiting for insurance to pay and not knowing the co-pay. Really because he didn't have an issue billing us ahead of time while she was there, he magically knew that number. Ridiculous. My relative will not be back to this facility, we are choosing one a bit farther from home, but with much better reviews and a much better staff, who have kindly answered ALL our questions and been wonderful.

I am a friend or relative of resident

My mother is currently there and has been for almost 3 yrs. The first 2 yrs. I really liked the staff and her aids were great with her. She is in a different wing now because of her dementia that is progressing and needs more attention than before, but clearly the care is worse. They have done away with any alarms, nurse call buttons go to pagers or cell phones, the chair alarms so if a patient falls out of their wheelchair, there is no alarm that signals anyone they have fallen. They do call you to let you know they have fallen and that there will be bruises, but they have also stated that they didn't know how long she has laid there. Not something you want to hear. See she can't walk or get up after a fall by herself. The room she shares with another resident is so small and poorly built for two people to occupy, that she can't even get out of her room without assistance because it's so narrow and has 3 doors that all open in different directions. The bathroom is not big enough for her wheelchair to get in the door so she is helped by 2 assistants and her pants and diaper are then pulled down out in the doorway then transferred onto the toilet. She should be entitled to a have a handicap bathroom that her chair fits inside for some privacy. I'm trying to find her a different facility now the care she is receiving is not good, I know it is hard taking care of these patients, but they still need to be treated like human being's and somebodies loved one. This place has really lost my respect and I fear for my mom being there.

If you live out of state and a family member has dementia, be advised that you may never be allowed to talk to them again. I called on my fathers birthday and the staff would not even take 2 minutes to let me talk to him. Clearly the staff could care less that there is still a person in there. His cognitave function may have declined, but he still knows who he is and who is family is. If any of the staff had family in a facility out of state, I guarantee they would not be happy if they received the same treatment as they are providing to me and my family.

The staff are not helpful allowing me to talk with my Mother, they give all of her friends and me the 'run around' when we either call the main phone number or my mother's personal phone. Mom had two strokes so she cannot move to answer the phone independently and her personal phone is typically out of her reach or 'unplugged.' This is the poorest excuse for 'continuity of care' that I have ever seen. Further, Mom's wedding rings were removed from her finger by staff and placed in a bag and somehow 'put through a shredder.' Either way, she was not there a month and her wedding rings of 63 years are now gone. Holt Senior Care & Rehab is anything but, it is very dismal.

My mom was at this facility. I loved her room. It was homey, spacious & lots of privacy. Some of the aids were outstanding; but one or 2 were deplorable. My mom, at the time had very serious health issuies, cardiac problems, an unusable left arm due to a doctor, diabetes, asthma. One night, New Year's eve actually, I told mom I would stay till a bit after midnight so we could usher the new year together. Mom was acting not good at all that evening & I was actually afraid if I left her something bad might happen health wise. This bossy, cocky aid kept telling me around 10pm I had to go. I expressed my concerns about my mom. I was told I could stay another ?20min or so & if I did not leave then, she would call the police. I did not leave as I was worried abt my mom. Sure enough she called the police & they came & made me leave. How horrible! Though most places have general visiting hours; this is the only place ( & since then mom has been in a few other places, due to doing 2 hospital & bk to rehab place a couple of times) that has ever given me any problem or any other visitor with other patients any hassle about staying late. The next day I arrived early in the morning & mom was not much better. A nurse came in & was ckg her wound/incision & said it looked infected. They had mom taken to the hospital where she was for several days, and ended up having surgery again on this pacemaker incision/wound. They said she had infection in her system & it was making her very groggy. Also when my mom needed breathing treatments for her asthma, several different aids and a couple of nurses reacted very slowly & uncaringly tho my mom was having lots of trouble breathing. The physical therapy people seemed nice & Dennis especially was very kind to my mother.

Staff are very nice and they do their best with what they have. This is an older facility and it needs a major make-over. The patient rooms are small and they aren't private. It feels very cramped and dark. On the plus side though, they have a beautiful bird aviary in the lobby that is fun to watch.

i thought it was surprisingly comfortable for a hospital type setting. the stafff, for the most part was warm and helpful. my mother, who was the resident, had no complaints and all her problems were handled in a timely fashion.
