"We're very happy with Autumn Fields Adult Community. My mom is in the assisted living there. You wouldn't know it's an assisted living community, it's like an executive retreat. It's very clean, nice, the people are very friendly and we're very pleased so far. The staff is very helpful, they know who we are, that we're the family, and they're very caring for my mother. The dining area is clean and the food is good, my family and I have had 2 meals there. My mom is doing crafts, playing games, they took the residents for a movie, they took them out to see the Christmas lights and they have entertainment come in specially at Christmas."

"I'm trying to get my sister move into Heritage Health Hoopeston. It's close to me, and it was very nice and really clean. On the scale of 1 to 10, it's at least a 9 or a 10. The staff who assisted me was very friendly. The living atmosphere was good. There were people sitting around, smiling and talking, and it just seemed really nice. When I was there, the residents were working on some kind of craft. It's clean, it's friendly, and the patients looked well cared for. The dining area was a big room with a big table and lots of chairs around it. It's very pleasant looking."

"My dad moved to Autumn Trace Rensselaer. It's excellent. It's very clean. Everyone's very friendly. My dad loves it here. The food is good. The building is brand new. It's beautiful. He has adequate living space. It's perfect for him. They have combat coffee for retired veterans. They have bingo. They have various outings throughout the month and several social activities available."

"My grand mother is a resident at the friendship house. I am extremely grateful for the security measures they take to keep all residents safe. I also like the fact the backdoors require a key or can only be opened from the inside.The building is always clean and well maintained. The staff are always helpful and friendly, which is also important. I feel like they go out of the way to make everyone feel comfortable.The commons area also has separate rooms for bingo, cards, and games to keep the residents occupied. It gives the elderly a safe place to mingle with other adults and have fun.I appreciate the fact that they give residents their personal space but also check on them if they haven't been heard from in a few days. To also keep a safe environment they put pull cords which alerts staff if there is a problem in the residents rooms, also very important to me.The friendship house has been a God send to my family and me. We know our grand mother is very well taken care of while maintaining her independance. "

"My mother's stay in Cumberland Pointe has been fairly positive so far. Of all the facilities I visited, it is the only one that I saw where residents were socializing in the common areas. I thought that was a good sign. The activity program is okay. They have two field trips a week that my mother likes. The room that she is in is fine. The food they are serving is good and healthy. However, I'm not thrilled with the level of medical care they're giving my mother. I just learned that they stopped giving my mother physical therapy last June and didn't tell me about it. I'm not happy about that. Also their medical director is on leave and I think right now this is the biggest deficit because the doctors who are covering for him are not doing a very good job. I can't rely on them and I have to take my mother to outside doctors for everything. Also this facility doesn't offer enough transportation so I'm concerned about how my mother would get to her appointment if I'm unavailable. "

"It's beautiful over there at Autumn Trace of Attica. My sister loves it. It's brand new, and it's very clean. They keep it immaculate, and the women all seem like they're so happy. Each of them has a studio apartment in there. My sister has a studio apartment and it's beautiful. It's just like living in an apartment building instead of being a nursing home. I had thought about selling my home and going over there, but it's a little more expensive. They have a game room where they go in and work on puzzles and do different things. They have a beautiful lot outside that they can go out and sit. They have several areas like screened-in porches, that when it's nice, they could go out and sit in there with their families. I've never seen something like that before, it's just beautiful. All the staff seemed nice. When you walk in, they greet you. If you go into a nursing home, you have to sign in, but this is just like they have their own apartments. You could just go in any time, you don't have to sign in, and you could go to their door and knock. They're allowed to come out and take you into a different room if you want or into their apartment. It's just like an apartment house. So, the people that are in there feel more comfortable. It's not like they're in a nursing facility. The staff is all young and very concerned. They have bingo, they have live entertainment coming in, and they have church services. They have puzzles, and you could go in and work puzzles all day if you want. They have a TV. Each one is allowed to have their own TV of course, but then they have a big TV out in the foyer for everyone. They are allowed to go into the dining room to socialize with each other, and that's nice."

"Horrible experience this time on the rehab wing,they let my pain meds run out,told staff my bladder n left kidney were really hurting(I've had kidney failure twice,I suffer from chronic kidney disease)requested an ambulance they denied me told me to calm down there was nothing wrong they done blood work and a urine test came back positive for a urinary tract infection n bladder infection,my husband had to empty my trash strip my bed n put clean sheets on it for me,had a male aide that was drunk and he touched me inappropriately one nite this place needs shut down Pray I NEVER have to come back here"

"My mother is in Watseka Rehabilitation and Health Care Center. This is the only place that had an opening in the Alzheimer's unit and took Medicaid. It's a fine place. Everybody is very friendly and very helpful. For the most part, she seems happy there. We have not had any trouble with any of them. The facility is very nice. They've got a couple of patios, a gazebo, and a lot of yards. Once it gets nice, we can take her out. The Alzheimer's unit has its own dining room. It's more like a restaurant. I had lunch with her one day and the food was very good. They have activities for them. They do her laundry, and they have a beauty shop. They offer three meals, and there's a nursing staff."

"I love the fact that they have 3 healthy meals a day, clean my apartment once a week, will do your laundry if you need them to, 24 hr on call assistants, VERY friendly staff!! Bingo 3 times a week, entertainment (when possible due to Covid-19), pets are allowed!! The rooms are one or two bedrooms and easy to get in showers!"

"My husband was at St. Anthony Health Care for 3 weeks, and they seem to do a good job for this rehab. They were understanding of his problems at the time. I understand it wasn't so great a couple of months with Covid. They were pretty good, the only problem was that sometimes it took a while to get help for him. I thought the rooms were pretty nice, including the one that he was in. It was a room for 2 but he was in there by himself. So for 1 person, it was pretty nice. It was okay, the hallways were spacious and they had a nice dining room area. The staff was wonderful and the food was pretty good. Medicare was paying for it, I really don't know how much we were spending everyday because we never got a bill."

"My mom has been living at Signature HealthCARE for almost a year now. The administrative staff is very friendly and very helpful. However, we have found that they don't seem to have enough nurses and aides to really go around. We are not entirely satisfied with my mom's care there. They're very friendly, but it is just we don't feel there's enough people working there. The rooms are very typical nursing home rooms. There are usually two residents to a room. Each has a bed, a night stand, half of the closet, and a place to put a TV if they have one. They have a decent amount of activities too. I know they do crafts, they bring in animals to visit the residents, and they can also provide transportation to doctor's appointments and to the store, though I don't know how often they do that. The dining room is very beautiful. It is big and open with lots of tables and a whole lot of windows, which makes it very bright. They also have a lady who comes and plays piano, which my mom really enjoys. "

"I liked Glasswater Creek of Lafayette a lot. It was big and roomy. It's a three-story building. No matter which floor you're on, they had gathering rooms. The rooms were bigger than the other place, and the rooms had a lot of storage. The dining room was nice, and they're not as restrictive. When we toured, we met everyone. The people that were living there, and the staff that walked by were very friendly. They have 24-hour medical staff, rehabilitation rooms, and exercise rooms. They just offered more than the other place did."
