Your Home, Your Life, Your Choice. Many residents of Autumn Trace go about their lives with no assistance. At the same time, we do offer a wide variety of assistive services to our residents, upon request. Should you have a need for any of the services we provide, our Wellness Nurse will meet with you and your family to discuss and design a custom plan of care. While many senior communities utilize levels of care as a standard and show you where you fit in, Autumn Trace has always found it more beneficial to tailor a plan to you as an individual. At Autumn Trace every day is different. Our residents can stay as active as they desire, without even leaving their new home. They will have the opportunity to enjoy many daily, weekly and monthly activities, from cards, games, crafts, unique programs and classes, special dinners, movies in our in-house theater den, church services in our chapel, holiday and seasonal parties, exercise classes and regularly scheduled trips to various local and regional locations and activities. We want our residents to host their family and friends, not only in their apartments, but in our common areas and in the private dining room, which is theirs to have special gatherings in whenever they like.
