As a result we have created an exclusive training and programming system called MOSAIC by Azura, which ensures that our Team provides not just high quality care, but exceptional personalized engagements that result in magical MOSAIC Moments and radiant smiles all day long!

Held-up as a standard in dementia care, MOSAIC honors the personhood of each individual by understanding that dementia is a disability in only specific areas of the brain, not the whole person. As a result, our strengths-based programming works to utilize the parts of the brain that are still functioning properly leading to engagement and enhanced quality of life. When you visit Azura you will witness our everyday use of MOSAIC programming through creative expression, small group peer interactions, special interest clubs, one-on-one engagements with caregivers, outdoor activities, small group outings, daily themes, church services, supervised baking, music and much more! In addition, you will be visiting a home that was specifically designed to support and engage those with memory loss throughout their disease process.

We offer a wide range of Alzheimer's, dementia and memory care services at Azura Memory Care of Eau Claire including: 24-hour memory care; on-site respite care; high caregiver to resident ratio; on-site beautician services; electronic charting system; medication management; diabetic management, including sliding scale insulin; registered nursing services; in-home podiatry and dental services and offer on-site physician oversight. In addition, our home works in collaboration with area home health and hospice providers to assist with higher medical needs including wound care, physical therapy, occupational therapy, speech/swallow therapy and end-of-life care. Azura also provides daily behavioral and medical monitoring and is sit-to-stand and Hoyer lift capable.

I visited this facility

I loved my tour of Azura Memory Care of Eau Claire, but there was no opening at the time. It was beautiful, very nice, and friendly, and that would have been my first choice. When I was there, they were watching the Hamilton play, which was very interesting because both the men and the women seemed to enjoy it. They also had a beauty parlor.

I visited this facility

We went to Azura Memory Care of Eau Claire. The manager was not there, so just one of the staff members showed us around. The tour was OK. He was friendly, but it was not as good as the other place. There was a nice outside area and it was completely fenced, so they couldn't get out. At the time we were there, they were eating. The place didn't impress me that much. It was nice enough.

I visited this facility

The tour we had with Azura Memory Care of Eau Claire was great. They're the ones that had three different room sizes. It's also nice that they had a lot of activities. The residents can do some of their own cooking there if they want and the layout was nice. The residents could walk around without getting lost or anything. They also had nice patios, gardens, and a nice activity room. The staff answered all of our questions and was very informative.

I went to Azura Memory Care of Eau Claire. The staff seemed very friendly, the people seemed pretty happy there, and the facility was nice. The staff was very good. The building was very well maintained.

I visited this facility

I toured Azura Memory Care of Eau Claire. It was a beautiful and very nice facility. It was perfect for my mother, but not my dad. I liked the way that all of the hallways led common areas, there were no dead ends, there were little places to sit, I love the colors they painted in there, it was so peaceful and calming, and I could have laid down and taken a nap there. The people were very nice, I saw the residents dressed appropriately, their faces were clean, and they looked happy. The facility was very clean, and well-maintained. The staff sat down one to one with me and probably talked to me for 45 minutes, answered all of my questions, was very knowledegable about their programs, and I learned a lot from her. It was a very pleasant atmosphere, and they had consulted with somebody that specializes in decor for memory care facilities.

I am a friend or relative of a current/past resident

My dad and mom lived at Azura and the experience was truly amazing! The caregivers are so well trained and compassionate they made all of us feel like family. We had originally put mom elsewhere, but it was quickly apparent they had no idea what Alzheimer's was and expected her to remember to take her medications and wouldn't do simple things like cut up her meat and allowed her to not eat. At Azura things were so different, the girls always introduced themselves when they spoke to her and spent time talking with her, holding her hands and event painting her nails. My dad can get gruff at times and they always found a way to make him smile and come out of his room. When we asked our doctor and others where to take them everyone said that Azura was the place, but my brother was worried because they don't have bathrooms in their rooms and that's why mom went some place else first. At that place she never came out of her room and within weeks they locked the door because she might fall or use it in appropriately. Needless to say we moved her and now she is always out of her room, smiling, singing, baking, doing exercises. When we really thought about it a person only spends like 30 - 45 minutes in the bathroom and maybe 8 - 10 asleep the other 13+ hours we wanted them to be surrounded by people who love them. That's what we found at Azura, people who love them and treat everyone like family. If I could I would live there!

I am a friend or relative of a current/past resident

My family had my mother at Azura for about 7 months and did not have a good experience. The training they say their staff receives was not evident to us in some cases. If I had to do it over, I would visit several facilities and ask a lot of questions about expectations, behavioral and medical interventions and costs.

Provider Response

Thank you for your review and we agree that families need to visit multiple homes to ensure that the fit for their loved one and they work. In addition, families need to be upfront about the care needed for their loved one and then utilize the expertise of that home to help them. Did you know that in the State of Wisconsin to be classified as a memory care home you only need to provide “client group” training with no direction as to topic or length? At Azura Memory Care we believe our residents and their families deserve better, which is why we have developed and provide an advanced dementia training program, called MOSAIC, to all of our Team Members. In fact, this training includes over 90 hours of hands-on, computer based and classroom training on the disease process, proper ways to approach, communicate, engage and holistically care for those with memory loss. Please note, this is triple the amount of training found in any other Memory Care home in our area and focuses specifically on helping those experiencing behavioral expressions. In fact, Azura Memory Care is known as dementia experts, not only in the Eau Claire community, but across the State. We are continually asked by other assisted living, memory care and skilled nursing facilities to provide this knowledge with their staff through consultations and trainings. Each person who walks this journey of dementia does so differently and while we tailor our approach to each individual, the expectations and ability to accept the realities of the disease by those who know and love them can be very hard to hear, see, understand and believe. We apologize, if in this instance, you felt your experience with our care was not what you expected. When our more than 20 years of expertise in dementia and advanced training is utilized the outcome is typically very good for that individual and their family. However, when this knowledge is completely disregarded even normal expectations would be hard to meet given these constraints.

Harbor House is a very caring environment!

Harbor House is a small home that helps those suffering from memory loss (aka dementia and Alzheimer's disease). Every time I go there I see and experience how caring the caregivers are and all of the residents seem very happy and comfortable. The employees there never turnover and always seem to be such wonderful moods. I always know that they are caring with their hearts! Also, the activity programming is the best I have ever seen. I didn't realize how much those with memory loss could still remember and interact, it's helped me find a whole new way to connect with all of those I love that suffer with dementia or Alzheimer's disease.
