I am a friend or relative of a current/past resident

Staff did not follow through on care instructions provided by hospital...relative ended up in hospital with stage 4 bedsore after wounds were not treated during a 7 day period...prescribed meds including blood thinner were not given during the 7 days spent here.

My mother is in ProMedica Skilled Nursing and Rehab. It is an older-looking facility that has a reputation for very good rehabilitation services, which is why we selected it. We came here because their visiting hours were a little more lenient than some of the other facilities. The visitation is from 9:00 until 3:00 pm. My only concern is that after hours, it's almost impossible to reach anybody. We've called and let the phone ring 20 or 30 times and not receive any answer from anybody working there. That is a negative to me. During the day, the staff has been very good and pretty attentive. The occupational therapy has been very good as is the physical therapy. The food is not good at all. it's bland and not good. We've been pretty much supplementing her diet. There are no activities. I've not seen any amenities, but with Covid, they've been restricting a lot, that we can't even take her for a walk in a wheelchair outside, so a lot of that is null and void. She's got her own room. Because of Covid, she has no roommate. She has a window, but no reclining chair. They've been very resistant to provide anything other than the wheelchair and her bed. They would benefit greatly from having more chairs available that were more comfortable and that has been something that has been bothering me. I've been trying to get her a chair that she can sit in aside from the wheelchair. Even a bigger wheelchair would be nice; they just will not provide it. Every time you ask for something, they claim Covid is the reason that they can't and I frankly don't believe it. I wish that they answer the phone after hours, have better chairs available, and get a little bit more flavor into their food.

After 8 days there, my mother wanted to commit suicide . Said it was the worst experience of her life. Because of Covid I nor my representative was not allowed in the facility. I get that. I would have expected nurses to return calls and in 8 days never received a call back from the so called Dr. Rahbar. Not sure if he even exists. The staff refused to listen to the meds she was taking prior to her stay. Always referring to the good Dr., that he had control of her meds. After being on strong pain killers for 7 yeas for severe arthitic conditions, they wouldn't give her any. Her doctor sent over a script that mysteriously was overlooked. I got her out of that place in a hurry. The only bright side was the Administrator stepped in and set up her release. Might be a good place to go and die, but for rehab, I'd take my chances elsewhere

I visited this facility

My wife had Spinal Fusion Surgery and needed Rehabilitation. Heartland Health on Mack in Grosse Pointe Woods was on the list so we went there. After about an hour my wife decided she could not take the smell and overall uncleanliness of the place. Most of the people were rude and when she called her Surgeon and he told her to leave. The Staff would not give us back the paper written prescriptions and also kept a 2 inch thick book the Hospital Supplied about all her Do's and Don'ts while healing. It was just pure meanest on "The Woman" in charge to keep our property and told us to go back to the Hospital and get a new one. This is about a $200.00 book on how to start your new life after Spinal Fusion Surgery. Don't risk you or a loved ones life by dealing with such a vindictive small minded person who was given a little power, your life may depend on it. This person was in charge on April 30th after 4:00 PM.

My mother was at Heartland Health Care Center. At the time she was there, she had a sitter because she is a fall risk. The service was great. I even tried to keep her there long-term. The staff was great. They did a great job with activities. The food was OK to me, but she didn't like it. I like the place, but the only thing about it is that I didn't see too many people cleaning.

My mother recently stayed at the facility, the therapy staff were wonderful and the nurses and aides were always friendly. My mother said she enjoyed the food and acactivities there. We would definitely recommend this facility to others.

I am/was a resident of this facility

First of all the rehab staff is very good. Who is management at 4 days in a row management told me that they will have my snack PB&J a pair for me because I don't want to eat me tonight and told me that it will be taken care of by dietary and four nights in a row dietary of dropped the ball dinner is served at 5 p.m. and only thing that you're giving to eat between 5 p.m. and 8 o'clock am the next morning is peanut butter crackers graham crackers animal crackers and juice that's the only options. And all I ask for is 2 four ounce 2% milk an two peanut butter and jelly sandwiches. And being in their facility you're at their Mercy.

We were at Heartland for about six days. Some of the nurses were good, and some were not. The doctors were good and attentive, but their orders were often not followed correctly by the nurses. I think they need their nurses or care workers to be better. The OT, PT staff was excellent. My mom fell out of bed on the last day and injured herself pretty badly after we had asked for a better mattress, and it was the very last day before I took her out. She fell at around 5:30 in the morning, and when I got there at 8:30 she had a new mattress. But after you fall out of bed is not the time to respond to the request for a new mattress. It was clean, and there were many things about it that were adequate or better, but overall our experience causes us still to have heart palpitations and to not want to go back. The facilities were more than adequate as far as the showers, the dining was fine, and the food was good. The activities director was very good, and the administration tried to help immediately and in any way that they could. Of course, I went immediately to them when I began to see that her medication was not administered properly and things like that. They really jumped right on it. I established a relationship with the social worker there and the floor manager. They were both very good, but of course they don’t have control over their day-to-day health care worker or nursing person; that is a personnel problem.

This was a Horrible Horrible place. my loved one left with wounds on her feet that nobody took care. When the caregivers at the new place took her socks off they stuck to her feet and the ulcer popped and her heals were black... Do not send your loved one here... no care the physical therapists were rude and had no patience for the patients. Terrible Terrible Terrible!!!!!!!!!!!!!

I am a friend or relative of resident

My initial impression was not positive at all. My husband was transferred on a Thursday evening by ambulance and I did not see him until 8:30 Friday am. This was after a 32 day hospitalization for my husband and we were both emotionally fragile. A large part of my first impression was a culture shock but then I came to realize my husband was one of those patients with a neck like a noodle (chin on his chest) and very low alertness. After 22 days of EXCELLENT therapy (Speech therapy, Occupational therapy and Physical Therapy) he is coming home tomorrow! Good therapy was the critical factor for us and Heartland delivered exceptionally. I also must say that the nurses and CNA's were very caring and responsive. We had a very good experience and, most importantly, an excellent outcome.

I went to Heartland Health Care Center - Georgian East. Their food was better, but I really didn't care to be there. The rooms were cold, they were not homey. There was no carpet on the floor, it was just hard floor. The wall painting was just bland. It didn't give you that comfort of home. Some of the staff was okay. They had social activities there, but I really didn't do much at Heartland because all I wanted to do was go home.

I visited this facility

My Gran stayed at Heartland and loved the surroundings. She was a Grosse Pointer to begin with and enjoyed being in the center of the town. There was also a Big Boys and gas station next door for the convenience of the guests and their family. These people seem to value opulence and luxury above all else. I've never seen an nursing home decorated so regally. The employees were for the most part fun and upbeat. They checked in on her more often than not and I felt comfortable trusting them to look after her when I could not. They were prompt in feeding her, changing her, and even came in just to change the temperature for her so she wouldn't be cold. When I asked her what they treated her like behind closed doors she said they were on average just as attentive as when I was there to ensure everything was ok. Gran enjoyed the food, and the Big Boys too! They also have lovely plush sitting areas and plenty of room for visiting family, of which we had plenty. I'd recommend Heartland to any demanding customer in the tri-city area.
