This is the worst facility I have ever seen. The facility itself is old, dirty, cluttered and completely in disarray. The staff make continent patients use adult diapers if they cannot get to the bathroom without assistance since they are short staffed. My mother in law sat in a soiled adult diaper from about 6pm last night till we arrived today at 1130am. The whole facility smelled like urine so I know my MIL was not the only one. I immediately pressed the call bell which was never answered and in fact was still on when we left at 415pm today. Staff was less than helpful. Refused to give her a bed bath so I did it myself. My MIL hadn’t been out of bed or even offered to try to get out of bed since Friday afternoon when I saw her last which is almost 48 hours. My husband and I had to work with her on some PT we were taught at the hospital and were finally able to get her out of bed, into a wheelchair and took her outside. This place is a disgrace and I wouldn’t even take my worst enemy there!! Unfortunately, people are limited as to who will take their insurance but do yourself a favor and don’t choose this place just because they have a bed and take your insurance!!!

The rooms look NOTHING like the virtual tour, heck none of it looks like it. Definitely cleaned up for pictures purpose. My mom was at this facility and very unimpressed with this location. Weekends seem like a whole different company as no one asked me to check in or do any COVID procedures. My mom was left in her DIRTY diaper! Also was not given LUNCH OR DINNER, because they forgot. If basic needs such as food are not being met imagine what else is not. The shower was HORRIBLE!! looks more like a closet with old equipment in what are suppose to be shower. For my mom to access the toilet, I had to move around equipment to clear a path for her to walk to get to toilet. Nurse was sleeping at the nurses station on numerous occasions. Overall DO NOT take your loved one here, it is DEPRESSING!

I felt very comfortable with my Grandma being at this facility. She was well taking care of & the staff was very friendly, approachable, and was prompt with answering my concerns and meeting the needs of my Grandma.

I am/was a resident of this facility

I stayed at PruittHealth - Lanier. The room was fine, and it's very spacious. I had a two-bedroom, but I was by myself. They cleaned it every day. They kept it neat and clean, but I would rate the personal care a 1. The food wasn't very good at first, but at the end of my stay, the food got much better. They had good facilities, but nobody knew what to do with the facilities that they had. The statement came in the mail and they had all the charges down there for the length of my stay and it showed a zero balance. I think because that's where I got my wound. One of the nurses caused the wound and I asked her to fill out an unusual report because that's what she had done to me. I said, "You go to your charge nurse and you tell your supervisor that you need to report that you hurt me while taking care of me." I didn't have a wound until she snatched the potty pan. It had a rough edge on it and the rough edge cut my leg. I kept telling them that it needed to have care. Finally, they came and had seen it, but it had gotten infected and it was not very pretty.

May dad was at PH Lanier. The nurse are great, staff very helpful. Close to home. The food was pretty good. All new staff from the last stay. We really enjoyed our stay. Room spacious, Staff informative, Activities ok, but Dad is a loner.

Not very pleasant. Always has bad smells. Understaffed. Food is subpar . Most residwnts can't chew the food they serve. Residents sit with dirty depends for too long without attention. Employees are to busy with there phones to help residents. Only a hand full of good workers are there and they usually dont last long from doing most of the jobs. Could be a better place with the right employees and management that take pride in there job.

Had to call 911 tonight to my aunt the facility is disgusting with smell and left her uncleaned for over two hours. Yells at patients to get back in there rooms. When confronted nothing but attitudes and excuses that they are very busy when a lie sitting at desk on facebook or playing on phones. If you have family here I suggest you get them out

My family member is there and this facility comes off nice but the CNAs do not know what they are doing nor do they care. My family member suffered numerous bed sores as they were not turned at all. I would be there for three hours and no one came in the room. You can get a Nurse or CNA that cares but after that are not at work, it is over for your loved one. My family member went down hill in a matter of months who was once someone mobile upon entering. In my opinion, not just this one, but all nursing facilities are pretty much the same....there just is little hope when a loved one enters a nursing home unless you live across the street!

My mother is there and they are very nice on the surface. But the care is less than adequate. Meds are not given on time, test results have taken a week and it is definantely sub par. I would not recommend to anyone.

Think twice about putting your loved one in this facility. They are incompetent, uncaring, resentful and disrespectful. They have no regard for the health and safety of the residents and forget about resident's rights and dignity. If a patient is bedridden, they will be neglected. They will not be turned every two hours as the state law requires. Your loved one will be left soiled for hours without anyone checking on them. Our Mother suffered horribly in their hands, enduring multiple, unexplained injuries. If I could choose zero stars, I would.

My father is being cared for in this facility until we can place him in a private care home. Pruitt Lanier is very clean and I was impressed with the staff. The workers are friendly, caring and attentive to my father. The social workers and nurses have been very helpful each time I've asked questions or needed assistance. I would recommend the facility to anyone looking for placement.

I visited this facility

PruittHealth seemed very clean, and the staff seemed very nice, but I didn't see any of the people that lived there. I don't know if they were kept in their rooms or if they were at lunch, so I couldn't tell by looking at the patients if they liked it or not. The staff seemed nice, but then they all do when you first go in there. As for the room, they were not shown to me. They said it was all occupied, and they didn't have an empty one to show to me. The place is clean and bright though.

I visited this facility

I have only visited Dan once at PruittHealth. They do the rehab, and he is getting better some days, but some days he doesn’t. The rooms are very cramped. In Dan’s room in order to get to the bathroom, you have to move the dresser that has a television on it, and in order to get Dan out with a wheelchair, you have to move it back. You have to move furniture to move the patient or to get to the bathroom; that’s poor planning. They needed a wall TV. It's a double occupancy, and I guess it’s just the way it is. It was supposed to be short term. It’s smaller than a hospital room, but he has a lot of the hospital equipment like the fancy beds that go up and down. Everybody has their wheelchair and their clothes and chairs for visitors, and it’s just cramped. It’s a little irritating, but he is safe there, and apparently the staff doesn’t have any trouble; they just move things around and do their thing. It’s totally different from what I am used to because it’s a nursing home and a rehab center. My impression was that they have plenty of staff, and Dan said the food was good. I am very impressed with the care manager. She is very alert and on top of things.

My mother has been at the facility for 61 days. For the last week, her room has been without running water or the availability of bathroom facilities, including a toilet. I even had to ask the evening nurse on Saturday for the return of her water pitcher that they took 2 days earlier as my mither did not have even water available to her (her roommate gave her a bottle of water to drink). She is in there for rehab after a fall and surgery. Before the fall she was completely self-sufficient, now, she is wheelchair bound for 3 months. The rehab has NOT taught her the skills for daily living, trainied her to use the bathroom facilities or even worked with her on any skills for daily living. The staff takes a long time to respond to call belo, as they are very busy with their cell phoine calls (they like to hide behind the curtains in patient's rooms). Now against doctor's orders, they are wanting to release her (they even refused to look at the documentation provided by her surgeon). During our only (and last) family meeting, they tried to state that we have been having several face-to-face meetings which has not happened. If you love your loved one, please find another facility. We are trying very hard to find another place for my mother.

My mother has been here for 5 weeks and has been treated well. She has made as much improvement as she can and will be released soon. There was extensive communication between all parties and I am pleased.

My mom was recently at Unihealth Lanier, We received great Care and Service....We were given a welcome basket Upon admission that made mom feel special.. We had a 72 HR,meeting to discuss Careand benefits...Therapy worked mom to the point we left with no devices

I hope and pray you will read this before you send you loved one to this place. My dad was here for a total of 12 days. I don't even know where to begin, the nurse's or help would take at least 15-20 min. to ever respond. We stayed around the clock we were so afraid to leave him. We were in the process of moving him finally to another place when he had a bad spell. They would not call 911 because they use a private co. He stayed on the floor of his room for 45 min. while we waited on the ambulance. If I had it all to do over again I would have taken charge and dialed 911. Please if you care about your loved one at all do not send them here. We were warned by a fellow church member about this place. But they released my dad from the hospital and no one else had a bed for a male at that time.

My dad stayed for a couple of weeks at Unihealth Post-Acute Care. The place was dirty. The staff were uncooperative, unpleasant and they did not inform the family of his condition. I got my father out as soon as I discovered how horrible they treated the residents. I would never recommend this facility.

Unihealth did a lot of work. They really upgraded it. It was a nice improvement and looks much better. The intake girl, when we checked in was very nice and helpful. But communication, coordination between the staff, maybe the people in the office, the nurses on the floor, they don't communicate. It seems like from the day shift to the night shift. The one thing that disappointed us was we never had any time to sit down at all about dietary or the physical therapy like they had said we would do.

The facility was very nice. We had to move my husbands grandmother into a nursing home after she was released form the hospital and before she could go back to her assisted living facility. Laurel Baye was very clean and the staff very friendly and knowledgeable. They spent as much time as we needed showing us around and answering our questions. It was farther from our home than the facility first recommended to us but it was so much cleaner and the residents looked much happier so it was no question to go the extra distance. She needed physical therapy and the therapists there worked with her very well considering her attitude on the subject. It was a struggle and she kept telling everyone we had put her there against her wishes which was true but she needed help. The staff took it in stride and after a couple of weeks she had even made a couple of new friends.

My mother in law is a resident of this facility. Recently they have kept her so medicated that she is unable to eat and has lost a lot of weight. She is either cursing or crying when she is a little alert and I feel that this is also a result of the medications she is given. She has told us on several occasions that they are very rough with her especially when putting her to bed at night. Also, they are letting her sit in soiled diapers for long periods of time and every time we are there, we have to ask them to change her. She has very limited use of her arms and hands and needs assistance eating. They have told us that they assist her in eating, however; if we come in at meal time, she is sitting at the table with no help and most of the time is so medicated that she can't even eat.
