Element Care is a non-profit healthcare organization that administers the PACE program (Program of All-Inclusive Care for the Elderly), an innovative medical model of complete care that serves 51 towns on the North Shore & Merrimack Valley of Massachusetts. Headquartered in Lynn, we have provided health and home care services to nearly 10,000 frail elders since our founding in 1995. Although our clients can be as young as age 55, the impact of our services is also experienced by the thousands of caregivers and family members who say time and again, "I don't know what I would have done without PACE." Currently we provide services to over 1,000 older adults who are able to remain at home and living independently in the community who would otherwise be in a nursing home if it were not for our coordinated healthcare services & support systems. "Programs offers Social Day Care, Adult Day Health and Alzheimer's Day Care programs for frail elders and/or disabled people. Cooperative Elder Services has programs that are available Monday through Friday 8AM-4PM. Services include breakfeast, snacks and a noon time meal, counseling, recreational activities, door to door transportation and nursing services to those in the Adult Day Health program. State Home Care reimbursement may be available to some low income elders attending the programs. Cooperative Elder Services day programs have been approved by the Alzheimer's Association of Massachusetts. This C.E.S.I. Has received a grant from the Crossroads Foundation to initiate an ""Extended Day Respite Care Program"" at this Concord ADH center. It will be on Mondays, Wednesdays and Thursdays until 5:30 p.m. There is no cost to clients at this initial phase."

For over 30 Years, the Adult Day Program has been supporting families with seniors and those with disabilities. The program is an enjoyable and enriching daytime environment that is housed in the local Hudson Alvirne Highs school which provides opportunities for seniors to interact with the young adults as well. Our program provides a broad array of engaging activities that promote mobility, stimulation and friendship among the participants. Families benefit by receiving the opportunity to remain active members of the community. We are licensed by the State of NH.
