AVAILABLE SERVICES (linked by our Case Coordination Unit Case Managers): Homemaker: Homemaker's assist with household tasks such as house cleaning, laundry, shopping and errands. Assistance is also provided for personal care such as dressing, bathing and grooming, instruction in meal preparation, medication reminders, and escorting our clients to physician appointments. Home Delivered Meals: Under Title III of the Older Americans Act, seniors may be assessed for individual needs such as home delivered meals. If assessed as eligible and in need of a home delivered meal, the client will be allowed to participate in this government subsidized program. The home delivered meal provider assigned to you will discuss the start of service, special circumstances that may exist such as special needs diets, and will discuss a suggested donation amount which, together with Federal and State assistance, will meet the cost of producing and delivering each meal. Adult Day Services: Adult Day Care supervises and cares for clients in a group setting outside the client's home. Services may include counseling, exercise therapy, as well as social activities. "We're using American Patriot Home Care. They had a lady come in for most of the time, we started at four hours a day and we moved it up to six hours a day, six days a week. They took care of my wife, did meals, and just made sure everything was okay with her. They did an excellent job. They stayed on top of it all and if I had a problem with anything, they were very adaptable and did everything right. They worked with me always and helped me out. Sometimes when 1 or 2 of their ladies got sick themselves, they immediately had somebody else to get in here. They did a great job. We had one caregiver that was having some problem, but they replaced her and everyone that we had in here was friendly and treated my wife right. They're just good, friendly people who played games with her and did all kinds of things. On her birthday they brought, balloons and cards and things, and I can't recommend them more highly."
