My family member was at Church Hill for almost 4 months. She had what I consider excellent care while she was there. They food is home cooking and the cheesecake was great. I watched my mother go from unable to get out of the bed to now being able to walk again and live independently again at home. The staff is very supportive and caring and relied on us for help build her plans so she could get back on her feet and home safe. If we ever have to go back to a skilled nursing care I hope they have a bed at Church Hill.

My family member almost died from Sepsis from UTI and Pneumonia. When she was finally sent out because she is a full code and the family demanded it she required IV antibiotics. We also have photos of the call light wrapped around her bed and she was in the chair. Order for mouth care every 2 hours because she can't eat or drink and has a feeding tube but lucky to get it done once a day. Today the staff dropped Mimi from a Hoyer onto her head. Resulting in an ER visit yet again. No where else to go. HELP state. And staff is rude to us because we have expectations. Requested a itemized bill as well and no one has given that either.

I visited Church Hill Nursing Home and Care Facility. I really did not care for it. I don't know how I would grade it because, in my opinion, it wasn't safe for residents that were up walking around or for visitors that were coming in and out. They were having difficulty when visitors would leave, because the patients would want to go, too. It was really confusing and I would have not left a relative there. I would have been uncomfortable leaving, not knowing that they were going to be watched over. From what I saw, it looked very clean, and the people that were working were doing as good as they could with the ones that they were having to deal with and take care of. The staff members weren't very efficient. They were there, but they weren't there. They were there, but you couldn't find them and you couldn't get them when you needed them. It was several minutes before they would arrive or even answer the call system, so that was frustrating for me as well as the patient.

My mom was a resident here for 4 years. She had dementia. They allowed her to fall and break her hip and didn’t do therapy to help her to walk again! Another time she fell and had 20 stitches in her face! Another time they allowed her to fall into a split, she couldnt get her legs apart after that! She had many other falls! They didn’t feed her often and most times I visited, her hands and fingers were covered in feces! The”workers” there go out to smoke every few minutes! The activities director is totally worthless! Bad place!!

They are very short staffed. The staff they have is sub-par. They do not work together as a team.

My father went to Church Hill Care & Rehab Center. It was very personal, and the best parts were the exercises and the rehab itself. The staff was very efficient. The rooms were small with two people in each room, so it was very crowded. They went outside for cake and watermelon during the summer. There were church services several times a week, music from different groups came in to sing, and bingo once a week. They also had a free barber/beauty shop on-site, which was real handy.
