I am a friend or relative of a resident

This facility is in Topton, Pennsylvania. If you have not been to Topton, it is relatively remote and isolated. But it is a nice quiet town in farm country. The facility itself is great. It is a very clean facility with both assisted living and a nursing home area. The facility is relatively small and personal. They are very flexible about visitation and notifying families if there are problems. They have quite a few activities for residents and they the rooms are spacious and comfortable. There were some complaints about the food the facility serves, but I imagine this is not a unique complaint for these types of facilities. Overall, I would give the Topton facility and above average rating. The staff seems very knowledgeable, and I was impressed with how caring the nurses were. I also thought management did an excellent job keeping the facility clean and organized. I would recommend this facility for other families.

We have an always available menu so if they do not like either of the two entrée choices they can always order off of that. All meals served restaurant style in an upscale dining room

All residents in traditional PC wear a pendant and have call bells in their rooms. We also offer a 26 bed secure dementia unit (has a waiting list)

Families are updated when ever there is a change in condition or medications are changed. Made aware of falls, and any type of injury.

All mandatory training as well as diabetic teaching, medication certification, demetia care

Reminiscing, talking about their past with them. sing alongs, old music shows like Lawrence Welk or Dean Martin, Devotions

Our Unit manager or Administrator do all assessments. It is repeated yearly or if there is a significant change in condition
