Not a great place. Call bells often go unanswered. Showers are not given as often as they should be. Briefs are not checked or changed like they should. You do not get too far by complaining to anyone. If you are looking for a good nursing home look on do not settle for this one.

I want to say thank you for the care that you give my Mom at Evergreens Commons! I want to give a special thanks to the maintenance crew for the care & compaction that they give to our parents in their time of need! I love you all Thank you! I don't think we give enough thanks to all at Evergreen Commons from the Supervisors to the people that physically serve our parents everyday! Even the front desk that are first to take our call's to get us to whom we need to talk to the kitchen staff that makes their meals Thank you. Well I am so Thankful for your continued care giving. Also the aides Thank you from the bottom of my heart you make my Mom smile & laugh. Believe me before Mom came to Evergreen Commons she was not doing well at all. Now Mom is up and about going to bingo & functions. Just Thank you! If you want a place for your parent to call home get them in Evergreen Commons Rehabilitation & Nursing East Greenbush NY. I write this because this is the only way to thank them & give someone out their a place to turn to if they need a real home for Mom or Dad Happy Memorial Day

Prescribing wrong meds for elderly with cardiac conditions. Aides cared- just not enough of them. No one answers phones at nurses desk- never. Passing off a doctorally prepared nurse practitioner as a medical Dr. Food is sub par for good protein or nutrient intake. Very carby. Nurse manager yells at you if you ask questions when something is wrong in meds. Bed and nurse buttons constantly put out of reach Very clean though and the main nurses and aides do care but their hands are tied. A lot of part time hours for those who write prescriptions. No recliner in room for patient so it’s the bed or uncomfortable chair. Need more wound care beds. Seems like you will get zero care for 2 hour windows while they are doing meals. Plan accordingly. One more person per unit, even just handing out food would go a long way in better care. Seems like a lot of group activities so that’s good if you are a joiner.

Evergreen Commons Rehab is a great place. Nursing is good, and the Activities department is very nice. Social work is great wind they work with my daughter for everything.

I had a great recovery at Evergreen Commons Rehab. My aide, Mike made me feel very comfortable throughout the stay. I will miss everyone here, they were all amazing.

The staff at Evergreen Commons Rehab are great. Nursing was kind and respectful. Housekeeping and laundry kept my room clean every day. The Activties team always had something for us to do.

I was a patient at this place in East Greenbush, NY. The food at every meal was cold and not given what was ordered. The staff in most cases had rude aids and some rude nurses.The facility at times did not have clean bathrooms and patients room had dirty floors. Clothes got lost in the laundry. Patients sometimes had to wait 1-3 hours to get help for different things. I would not recommend this place for anyone. Not enough staff and patients were mistreated. Therapy was good.

The staff, regardless of roles were consummate professionals! Their expertise combined with both compassion and totally positive attitudes made for a very successful and comforting experience! Rooms were comfortable and immmaculate.

The nurses at Evergreen are very good. Everyone is so helpful. The housekeeping does a very good job at keeping everything very clean.

Sorry. The only choices were Assisted Living, Nursing Home, or Memory Care. My good friend was there for short-term rehab. And, based on the POOR care she received as an 89-year-old woman with full faculties, I’d be very circumspect of this place. Her aides and nurses left her in her filth for over 6 hours! They did not respond to her calls. Her daughter asked them to do a UTI diagnosis on Wednesday and today, after getting her removed on Saturday from there to her daughter’s, they just contacted her and told her she has a UTI and possible sepsis. And the doctors, OMG, when she went in prescribed her meds that anyone with a heart issue or possible heart issue should never take. Please do not subject a loved one to this abhorrent facility.

I went for rehabilitation and found it to be very good. The therapist really worked hard with me and really made progress. The housekeeping was very kind and left everything clean.

my mom was a patient at evergreen n. facility for 11 months, on 1 schodack way. she absolutely loved her nurses and aides who were good to her. she absolutely loved the food and she enjoyed her activities and guest singers. she also loved her church on sundays as she was always a devoted christian. she often spoke highly of her weekly trip to the hair salon with her stylist shaquita, whom she loved. my mom was a nurse for 40plus years and she loved her work. at 90 years old we had a Birthday party for mom in December at the nursing home with all her roommates and nursing staff. she was so happy. on March 22, 2020 i recieved that dreaded phone call that sunday morning. my moms heart was shutting down. even during the pandemic i was allowed to be with my mom at that time. i held her in my arms as she left our world and went to be with my Dad. the nurses were such angels to me. consoling me at this difficult time. sending sympathy cards and helping to pack my moms belongings when it was very difficult to do. i am very grateful to evergreen nursing home for giving my mom the greatest care and happiness while she stayed there. again thank you to all staff and exspecially Lori in admissions for getting my mom there so quickly.

my mother is a patient at evergreen. i would like to thank all the proffessional staff at 1 schodack way section for their wonderful compassion and skills towards my moms care. because of their wonderful one on one care for her i still have her in my life today when at one point we all thought she was leaving us. her room is always very clean and organized and quite large. mom loves the activities and church gatherings. she also loves her nurses. they are our angels ! this is the 3rd nursing facility mom has been in and her final home as it is the perfect place for mom. thank you to all the staff at evergreen commons you are fantastic !!!

My family member was not given breakfast and the staff who "forgot" to bring the breakfast blamed the patient (who cannot speak up) Patients ring the bell and it takes about an hour to have someone come in the room. (The staff have even told us to ring the bell an hour before you need something) When someone does come in the room, you tell them your request and they say they will come back to assist you and they NEVER come back. Trying to get pain medicine is nearly impossible! Patients suffer greatly in pain at this facility. The staff complain all the time about working here. I heard a staff member say they are underpaid and understaffed. Most staff are very rude to both patients and visitors. I would not recommend this facility for care of a loved one.

My mom was here for rehab. She presses the call bell because she wanted to go for a walk and it took staff over an hour to come in. She wanted to get dressed and the aide told her she would be right back and 45 minutes later the aide never returned so my mom got herself dressed. When the aide came back into the room almost 2 hours later she yelled at my mom and told her that she should have waited for help. The food is always cold when they bring it to the patients. My mom ordered dinner the other night at 5pm and they finally brought her food into her at 6:30 and it was ice cold. My mom waited for over an hour for an aide to come in when she pressed the call bell because she had to go to the bathroom. When the aide finally came into to help my mother into the bathroom she had an attitude like my mom was bothering her. I would NOT recommend this place to anyone.

This place is the most disgusting dirty home ever. Staff is miserable my brother was there after his stroke had a feeding tube in had many falls went to the hospital not going to the bathroom emergency room doctor said that he was extremely dehydrated his system was shutting down how can someone be dehydrated with a feeding tube in he never made it out of the hospital he died I wouldn't send my own worst enemy there they don't care for their patient s all they want is money

Everything about Evergreen Commons in East Greenbush exceed my expectations. An initial stay for a dear friend included extensive rehabilitation services. I never thought my friend could be restored to level of capability again in her life. Evergreen Commons staff helped her so much that she functioned at a level that exceeded what she had experienced for the last 3 years. Sometime later she was admitted to the Alzheirmers unit under the direction of Brenda . I found the care and caring to far exceed my expectations. The room was beautiful but the real shining star was the staff who showed great patience, My friend smiled every time any of them came into the room. I saw them take her hand or give her a quick hug frequently. She was always clean and well dressed. She participated in social activities which she greatly enjoyed, especially the music programs they provided. Both she and all of her personal property were treated with respect and value. However, nothing could exceed the staff commitment as they tried to find foods she liked, worked to find a larger bed and a comfortable wheelchair for her use, always knew where she was when I came to visit and she was not in her room, and answer her call light promptly. No matter how busy they were they always greeted her with a friendly smile. I rarely observed the staff at the nursing station gabbing because they were always with the patients on the unit. All of the patients were cheerful and seemed genuinely happy there. And all staff members considered all residents as their own. I could ask anyone how she was and they knew, even if she was not one of their patients. I also received frequent phone calls even when there were only minor changes being made in her care which I found very comforting. My friend was very happy there and I was very pleased that she was in a good place where she was considered important and valued. The staff in the Alzheimer's were truly outstanding.

My mother was recently at Evergreen Commons for Rehab. It was a nightmare. They are severely understaffed and it broke my heart hearing residents calling for help and no one coming. Luckily we had enough family and friends to be there nearly around the clock. At her follow up Dr appointment today she told the doctor it "Was a house of horrors". She was only there 2 weeks, I can't imagine what is like for those who are there longer

THIS PLACE IS HORRIBLE!!!!! After being there about 24 hours my mother’s skin on her backside was broken!! She was not changed all night long her 1st night there. The 2nd day she wasn’t changed from theMymorning cleanup until I called someone at around 3:30pm. She will not be staying there!!

this nursing home never has enough help. just recently I went to visit my friend it was 10:30 he was not up or had breakfast and in his diaper all night until I got there uncalled for how would they like to go with no breakfast.

My sister has been in this place for five plus years and had noting but trouble Just recently the room was so cold like 30 degrees had to get the nurse to fix the heat because there was no knob I have been in her room when her machine goes off and no one comes for like 20 or more minutes I have tried to get her out of this place and once your in it is impossible to get move to another facility I have been trying can say most of the aids r very friendly they r over worked they need more aides

My mother suffered from Alzheimer's and was cared for at this facility at the end of her life and we could not have asked for a better experience. The hard working staff were always pleasant and available, my brother and I never felt that my mother was ever neglected; the room was clean and bright; the whole facility is in a very nice location. You have to realize that when your elderly parent is put into a nursing home, it's impossible for the care to exactly match the personal care at home--but for my family, Evergreen was a God send and we are very appreciative of how they cared for our mother when my brother was unable to keep her at home after she fell ill with pneumonia.

I visit my sister who has been in Evergreen Commons for over 5 years it has gone down hill ever since it was sold I visit her 2 or 3 times a week and the room she is in is always dirty things thrown on floor and still there when I go back next time. Her alarm goes off and it takes 15 minutes for someone to answer it. (I clocked it one time) I have been trying to get her out to a better place but it is very hard once a person is all ready in the home.

I currently have a parent in this facility, unfortunately. He has to wait at minimum one hour for anyone to answer his call button, and at times, up to 40 minutes for assistance in getting off the toilet. I have often gone down to the nurses station, only to find the help on their phones texting, chatting, or just sitting. Meanwhile, call bells are going off non- stop and it's as if they don't hear them. I would never again put a loved one in this facility. Lawmakers should fine this facility. To whomever owns this facility, if I am able to report this abuse, I fully intend to.

My grandmother was here before she went in to hospice. It breaks my heart to recall the terrible care she received here. They were mean, uncaring and gruff with her. Her bedding was always wet, and her diaper left deep welts on her frail body. I will never forget her saying "they won't let me eat". After my gram passed, and we collected her belongings, we noticed they were all labeled with the incorrect name. This place should be closed down.

My family member is in this facility, and she's on a special diet and she doesn't get what she's supposed to. She currently got a cold from here and sounds terrible on the phone, hasn't seen a doctor for her cough which sounds congested. She's put in the hall for hours after a meal because the staff is too busy doing other things!!! Her care is not what was expected! Don't send your family members to this facility, it's VERY poorly run!

My one hundred year old mother-in-law had the misfortune to go to Evergreen Commons for rehab after surgery to mend a broken hip. We - the family - chose this place because they would allow her personal aides to be present at all times during her stay. Her aides are excellent at what they do, and had a regimen set up to manage her late in life diabetes. The nursing supervisor was adamant and confrontational in not letting the aides do much of anything. They could not give her water in her room, she had to help herself. They could not feed her anything at any time, she must feed herself. During the nights, they would let the nursing station know that she was in pain. They would only give her tylenol which she could not get down because she was having trouble swallowing. The doctor there was hard to get in touch with during the first part of her stay. When finally we did get in touch, he would not change the sugar regimen which he had at no insulin unless the sugar level was above 250. At the high sugar levels she was lethargic. The therapy staff were great, as were several of the nurses and staff. It appears to me that the nursing supervisor, the head nurse, the night nursing staff and the doctor were road blocks to the well being of mom. Yes, she was 100 years old and needed aids to get around and do her daily functions. Her mind was good. There is no excuse for long waits to get pain medication and help in drinking water back in her room. I also feel the record keeping does not reflect the true sugar levels. A reading of 599 became 490 then 435. When the records were released, there was never a reading above 299. I strongly feel this place is understaffed and the management is only concerned with the bottom line profit. If you are diabetic, look elswhere.

They gave what I would say was average care up until my grandfather actually passed. Within minutes of him dying, they asked when he was going to get "picked up." When we said we were going to call funeral homes, they made it clear we needed to hurry. Then....a memory I will never forget. They had my deceased grandfather wheeled out in a body bag in front of his whole family and myself. Other residents also saw this. One resident started to cry

Three of my family members have gone to this facility. All three left in better conditions than they had seen in years. Great medical oversight and kind and caring therapy staff. One of my family members still visits all the friends he made there.

This nursing home should be closed down or have a complete over haul of its night staff. They hire young inexperanced aides and nurses who only want to party and have fun, patients are left alone in common areas for long periods , call buttons aren't answered for over thirty minutes some of the aides r mean and talk nasty, patients r given drugs to keep them calm without family notice (Xanax), Elderly who use a bathroom all day r put in diapers at night and wake soaking wet because the night staff is to busy on their phones or the computer, The facility as no security cameras anywhere in the building there for staff has the ability to do what ever they want, Staff nurses have no idea where their aides are it seems no one on night shift as to answer to anyone. My list goes on and on. If u have a loved one in here check on them often in the evening and what drugs their giving. Advice don't put your loved one in this facility RUN!! Do not judge this book by its cover.

Very understaffed. Patients have to wait for a long time when they call for help. Our family has observed our relative waiting for at least 30 minutes when she calls for assistance, and one time it was almost 90 minutes. Good thing she had diapers on. The food is good, and they have a lot of activities. The main issue is the lack of aides, which causes patients to have to wait for long periods of time for help.

I Think it is to under staff and residents have to wait to long for help when needed my mother didnt get the help she needed so got out of bed with the bed alarm on and was able to change her clothes and wander around room in which in this time frame she pulled out medical devices and tubes but no one heard the bed alarm and her room is next to nursing desk . How could this happen? Please do not send your family member here if you want them to get better

My mother re-habbed for a broken hip until taken to the ER fot treatment. This facility is grossly understaffed. 20-25 minute wait for the toilet not uncommon on weekends/holidays, little attention to detail, supervision of staff to see if they had done their job. Pills left in open container in dining room, linens on bed for over a week, no adhearence to dietary concerns - open ordering for menu choices. Not the place for frail individuals who need help with the simplest chores. Would give no stars, but system will not allow.

Over the years I have had two relatives become residents of Evergreen Commons; my grandfather and my Papa. In the case of my Papa I became experienced with both the assisted living element of the facility and, unfortunately, the dementia unit. My grandfather became a resident in the short-term rehabilitation area. In both cases, I found the nursing staff to be extremely caring and helpful. I remember one Halloween, the nurses helped the residents give out candy to visiting trick-or-treaters. All of the doctors I personally had experience with were very informative and made me feel at ease with my loved one's care and treatment. The grounds surrounding the facility are well kept and away from heavily trafficked roadways. The building itself is clean and welcoming. Overall, I would say that Evergreen Commons is a great option when looking for help in caring for your loved ones.
