Facility is not properly managed nor safe for anyone seeking long term care. Management is unable to keep qualified employees and takes no responsibility for residences care. Manager over promises and under delivers with residences’ living conditions, mental and physical recovery. Activities are provided but residences that are unable to get themselves to the activity are left out. I would not recommend this facility to anyone.

My mom has been a resident at Grace since March of this year. It was one of the hardest decisions for our family to make as my moms health had continued to decline and she was unable to walk. She has numerous health problems and we had been taking care of her over the last 2 years and we worried so much about where we would transition her for long term care. Grace has been such a wonderful place for her and an answer to our prayers about feeling that she is well cared for. The nurses genuinely love the residents and have been so helpful to us and kind to mom. We truly trust them with her care and that was our top priority when choosing a long term care facility for her. They welcome us to visit her anytime and we are welcome to take her out to family activities dinner, etc. If you are looking for a place to take care of your family member, I highly recommend Grace.

The rooms were just ok, need to be updated badly. My brother was there almost one week and he begged me to come pick him up after 3 days. He was almost bed ridden at first that is why he was there to receive physical therapy that he finally got on the fourth day after I called and complained. At first they were pretty nice until I started to see no one was doing their job, no bath or shower after 4 days, urine container so full and when he asked someone to empty it said they were going to get some gloves never came back, not to mention the gloves were right there on my brother's door. So after I had been there more than 30 minutes I went to find someone to take care of it so it had been over an hour, oh and did I mention it was sitting on his tray table where he eats because he couldn't reach to set it on the floor or anywhere else. I wouldn't take my worse enemy there. Then I had to call the head nurse (I think) her name was Tabitha, she was pretty nice the first time we spoke, the next time she was rude and blamed everything on my Brother defending everyone else because they were not doing their jobs!! Bottom Line DONOT EVER TAKE ANYONE YOU LOVE, OR hate TO GRACE LIVING CENTER -JENKS

Today was the first day of visitation after Covid. We called and made arrangements that my Granddaughter could come in wit her parents and finally meet her Great Grandmother. She is 7 months old, my Mom is 85. “Vella” refused to admit them. I hate to throw an absolute fit and I don’t like being that person, but for my Mother, my daughter and my Granddaughter I was. They finally got to go in, but not before Vella let me know she was going to report me, tried to frighten my Son-in-law by assuring him that his child would get desperately ill, and in general was horrible. My Mom has been there since 2007, and puts up with a lot of nonsense but this went beyond the pale. Thank God for the nurses who know and love my Mom, and allowed them. I’m giving the staff a high rating, but know it does NOT include Vella.

Lets start with the FOOD: it was so nasty, often times, my family would not eat and no alternate was offered, as required per CMS guidelines. STAFF. The Weekend staff was lazy, understaffed and inattentive. Male CNA's wore their scrubs half off their butt showing their underwear. THERAPY was rude and dismissed the patients requests and input for their Plan of Care, as required BY FEDERAL LAW. THERAPY embarrassed and humiliated the patient by scoffing at progress. The FACILITY is old. So old, there cannot be a riser placed on the toilet, thus requiring the patient to fail at therapy/toileting/ADL's and incur unnecessary pain and embarrassment. Insurance paid, so VALUE is not an issue, except it is obvious the facility is NOT spending money for adequate staffing and education. ACTIVITIES was totally absent. During the Pandemic, no visitors, no family, no church, so no stimulation, socialization or diversion for these patients. The patient requested assist with toileting and it was not provided in a timely manner, thus causing the patient humiliation and embarrassment. An extra blanket was requested and never provided, therefore the patient was uncomfortably cold at night. When attempted to deliver clean clothes to the patient, the bag sat at the front desk for 6 hours and not delivered to the patient, thus, they had to wear soiled clothing. The lack of caring at attention is appalling. They are using the pandemic/isolation as an excuse to not provide appropriate care as they know no one is there to observe. However, the State and Federal entities have been notified. The patient was moved room to room without consent of patient or consent of family. No Care plan meeting was ever done. Do better. We are the patients advocates now.

Whatever you do, don't take your relative or loved one to this place. My sister-in-law was treated badly. She fell out of bed one night and despite she and her room mate calling for help, no one came for six hours. her food was always cold and she didn't get enough therapy while she was there. She came home with bed sores on her rear and also a very bad urinary tract infection. If you love your family memeber please don't send them there.
