My husband had two operations on his feet, and right now, he is in Apache Junction Health Center for his rehab. He has one more week to go, and we hope to come home. It is about two miles from my granddaughter, and he is living here right now because he cannot go down the steps of our home because his feet are too sore. The nurses are doing the best they can to do whatever the doctor says. They try very hard to follow his orders, but it is hard to get a hold of the doctor. Overall, the nursing staff is just really nice. The services are fine. They can improve on having a doctor available at all time, instead of just coming in once a week to see him. They are doing the very best they can. They take him to dialysis and bring him back. He is doing OK. They called me last night because his oxygen was too low, and he was having difficulty breathing. I am there in five minutes, they take care of him, they call the doctor, and he orders treatment. Right now, I am satisfied with the services.

Absolutely appalling care — avoid this facility at all costs. If you value the life, health, and dignity of your loved one, do NOT send them here. The care (or lack thereof) that our loved one received was beyond unacceptable — it was negligent and dangerous. We had to be there every single day to advocate for basic care. Nurses failed to assess him properly, medications were not administered on schedule (including pain meds), and calls for help were routinely ignored. Staff admitted their food has no nutritional value, communication within the facility was practically nonexistent, and phone calls — even from outside doctors — went unanswered for days. Despite showing obvious signs of vascular distress (purple toes, pain up the leg, cold to the touch), the staff continued pushing him into physical therapy. Only after a cardiologist stepped in and contacted a vascular specialist did anything happen — and even then, the vascular office struggled to get in touch with the facility. He was finally sent to the ER, where he needed an above-the-knee amputation, received multiple blood transfusions, and was found to have a fungal infection and severe anemia — all of which had been ignored for weeks. Administration showed concern only when he was sent to the hospital. We were told someone from the board would follow up — they never did. We followed up and were ignored. No accountability. No compassion. Just silence. Our loved one passed away due to the prolonged suffering and neglect endured at this facility. We are devastated, and we fear for the other patients still there. Please — there are better places. Don’t make the mistake we did.

Mother was at Apache Junction Health Center for ten days. It's both a long-term and rehab center. They had a nice rehab center with a lot of gym equipment, but we never got to experience that part of it. If I could rate somebody less than a "one," I would. I couldn't get my mother out of there fast enough. The day we brought my mother in there, she got in at 9:00 p.m. The next morning, I came in, and she was lying in bed all bruised and bleeding. I asked them what happened, and they said, "Oh, she fell, and we took her to the emergency room." However, they never called me or told me that they were taking my mother to the emergency room. I was very upset about that. She doesn't look like she was in the emergency room because the bandages were haphazardly put on her, and she had a bruise that went all the way around her leg from her hip to her toes. The whole leg was one big bruise, so we couldn't get her out of there fast enough. I would never recommend them. I can't believe they would take her to the emergency room and not tell me. I don't even know who authorized her treatment or anything. The facility was nice. It was a lot more homey looking. The rooms were large. They had a dining table and there were chairs. Especially during COVID, they were having to eat in their room, so they had that set up very nicely. The rooms were nice. They felt more welcoming. They were big rooms, and they also all had private baths. They were extremely limited on visitations, so I wouldn't get to see my mom very much. We were only allowed to see her once a day for thirty minutes. I didn't see any activities.
